Revista de la Facultad de Medicina Humana

Introduction: Solitary fibrous tumors (SFTs) are rare mesenchymal neoplasms that, although typically develop in the visceral pleura, occasionally occur in the intracranial cavity. Furthermore, they are characterized by high rates of metastasis and recurrence. Case Report: We present the case of a 59-year-old male patient with a 3-month history of headache and bradyphrenia. Computed tomography revealed a neoformative tumor infiltrating the nasal cavity, ethmoid sinuses, and anterior cranial fossa, involving the left frontal lobe. The patient underwent two exploratory craniectomies, during which diagnoses suggestive of high-grade glial neoplasia and SFT were made. For precise diagnosis, immunohistochemistry was performed, which was …

First Case Of Thanatophoric Dysplasia Type 1 In The Peruvian Andes With A Pathogenic Variant In The Fgfr3 Gene, Carlos Hugo Torres Salinas, Yesenia Ledesma Porras

Revista de la Facultad de Medicina Humana

Thanatophoric dysplasia type 1 is a form of lethal skeletal dysplasia, characterized by axial-appendicular skeletal disproportions as well as short stature, frontal bossing, cloverleaf skull, narrow chest, femoral bowing, and micromelia. These phenotypic characteristics are the result of pathogenic variants in the fibroblast growth factor receptor 3 (FGFR-3) gene, located on chromosome 4p16.3 For its study, obstetric ultrasound, physical examination and radiographic findings are important. However, the diagnosis must be confirmed by genetic study in order to discover new variants or associations, as well as to reveal its actual casuistry in a certain region.

Objective: Identify associated factors with emergence delirium in children in the Post-Anesthetic Recovery Unit of a Pediatric Health Specialized Institute in Peru. Methods: A cross-sectional study was carried out in children from 2 to 6 years old, physical status classification I and II, in elective procedures under anesthesia. The main variable was the presence of emergence delirium evaluated with the Pediatric Anesthesia Emergence Delirium scale greater than or equal to 10 points. To identify the factors associated with delirium, the Poisson regression model was applied and a p value < 0.05 was considered significant. Results: Of the 150 children, it was found that the incidence of emergence delirium occurred in 10.6%. Of these, 81.4% were under 5 years of age, pain was significantly associated in 37.5% with PR = 3.63, 95%CI [1.20, 10.1] and it was observed that 68, 8% required palliative care. Conclusion: Pain was associated with delirium of postanesthetic emergence in children. Prevention and treatment of emergence delirium should focus on postoperative pain control and careful monitoring of patients after surgery.

Methods: To strengthen sexual health knowledge among medical students in Miami, we conducted a brief workshop in February 2020 and March 2021. The workshop provided an introductory overview of the physiology …

Leadership Coaching: Enduring Lessons From Mythology, Raymond L. Forbes May 2024

Leadership Coaching: Enduring Lessons From Mythology, Raymond L. Forbes

All Faculty and Staff Scholarship

The juxtaposition of leadership coaching and mythology together in the same sentence is probably enough in itself to set traditionalists on edge. How could a look at the myths of cultures thousands of years old possibly benefit a leadership coach working in a modern, complex, technologically-oriented society? While much of the current literature on leadership coaching takes a non-historical, rational-empirical approach to the subject, a great deal of practical, useful insight can be gained from assessing lessons derived from mythology. The content of leadership coaching typically focuses on the resolution of professional and organizational issues in the present and on …

A Behavioral Intervention Checklist To Reduce Re-Hospitalizations In Residents With Mental Illness, Jeffery Davis

Doctor of Nursing Practice Projects

Skilled nursing facilities are an essential component of the continuum of care and, with the rapidly aging population, represent an increasing cost to the healthcare system. The policies beginning in the 1960s promoting the deinstitutionalization of mental health institutions have resulted in increasing numbers of individuals with serious mental illnesses now residing in skilled nursing facilities. The complex needs of individuals with mental illness have been associated with an even greater cost of care, deficient quality, and safety concerns. This quality improvement project analyzed the impact of an evidenced-based, structured behavioral intervention checklist on rehospitalization rates in individuals with mental …

Asthma is the most prevalent disease in childhood and disproportionately affects African American (AA) populations in terms of prevalence and frequency of exacerbations. Air quality and environmental factors are strongly associated with the diagnosis and management of asthma and AA children are more likely to live in areas with poor air quality. An air quality flag program was implemented in a daycare located in a county with a predominantly AA population with a high prevalence of asthma diagnoses. The goal of the intervention was to increase staff awareness of air quality and the impact it can have on asthma diagnosis …

Los Intérpretes Médicos Son Un Derecho, No Un Privilegio, Morgan Uesseler May 2024

Los Intérpretes Médicos Son Un Derecho, No Un Privilegio, Morgan Uesseler

World Languages and Cultures Senior Capstones

This essay explores medical interpretation as a right, not a privilege. It argues that medical interpretation is a vital service that should be required in all healthcare settings, not just in those that are funded by the government. Themes addressed include the difference between interpretation and translation, face-to-face interpretation versus telecommunication, liabilities and risks associated with untrained and uncertified medical interpreters, and the federal law Title VI of the Civil Rights Act of 1964. Challenges experienced during any medical session are underscored, while giving special focus to limited English proficient Hispanic patients whose healthcare literacy is in Spanish, not English.

Minimally Invasive Tissue Extraction: Continued Growth Through Simulation For Obgyn Residents, Angela Frankel, Md, Mph, Melanie Hotz, Md, Patrick Teefey, Md

Department of Obstetrics and Gynecology Posters

Background

  • Minimally invasive surgery often requires tissue morcellation for extraction of large surgical specimens.1
  • Surgeon comfort with tissue morcellation in gynecologic surgery is vital in providing a minimally invasive (MIS) approach.
  • However, OBGYN residents often feel underprepared to perform this skill independently.2
  • While practicing tissue morcellation in the OR is ideal, simulation has been shown to improve resident technical skill without risking increased operative time or associated morbidity.3
  • This intervention seeks to build upon a prior simulation exercise, with the ultimate goal of improving OB-GYN resident self-reported comfort with minimally invasive tissue extraction.
