Interprofessional Education And Healthcare Simulation Symposium, Janet Willhaus
Interprofessional Education And Healthcare Simulation Symposium, Janet Willhaus
Janet Willhaus
Interprofessional Education (IPE) has been recognized by various international professional societies (e.g. World Health Organization and Institute of Medicine) and accreditation organizations as foundational to achieving safe, high quality, accessible, patient-centered care. An Interprofessional Education Collaborative (IPEC) formed in 2010 to develop common domains and again in 2011 to define domain competencies for interprofessional education.
Furthermore, in 2012 the Society for Simulation in Healthcare (SSH) and the National League for Nursing (NLN), supported in part by the Josiah Macy Jr. Foundation, collaborated to explore the advancement of IPE and interprofessional practice (IPP) via the instrument of healthcare simulation using the …

Amresh Shrivastava (ed), Megan Kimbrell and David Lester Handbook of Suicide From Global Perspective
Preface by Professor N. Sartorius, MD, PhD
Suicide is the cause of death for nearly one million people a year. Death by suicide is often reported as being due to other causes to avoid stigmatization and other negative consequences of suicide for the family: it can therefore be assumed that the number of people who commit suicide is much higher than this number. Attempts of suicide …

Selective Breeding For 50 Khz Ultrasonic Vocalization Emission Produces, Howard Cromwell
Howard Casey Cromwell
Ultrasonic vocalizations (USVs) are emitted by rodents and can signal either negative or positive affective states in social and nonsocial contexts. Our recent work has utilized selective breeding based upon the emission of 50 kHz USVs in response to standard cross species hand play—namely experimenters ‘tickling’ rats. Previous work has shown that high-tickle responsive animals (i.e., rats emitting abundant 50 kHz USVs) are gregarious and express enhanced positive emotional behaviors relative to animals exhibiting low 50 kHz USVs. The present study extends this work by examining the developmental profile of play behavior and the suppression of play behavior by predator …

The Publication Of Ethically Uncertain Research: Attitudes And Practices Of Journal Editors, Carla Angelski, Conrad Fernandez, Charles Weijer, Jun Gao
Charles Weijer
Background
Publication of ethically uncertain research occurs despite well-published guidelines set forth in documents such as the Declaration of Helsinki. Such guidelines exist to aide editorial staff in making decisions regarding ethical acceptability of manuscripts submitted for publication, yet examples of ethically suspect and uncertain publication exist. Our objective was to survey journal editors regarding practices and attitudes surrounding such dilemmas.
Methods
The Editor-in-chief of each of the 103 English-language journals from the 2005 Abridged Index Medicus list publishing original research were asked to complete a survey sent to them by email between September-December 2007.

Background: Recommendations for antibiotic treatment of acute otitis media (AOM) have changed over the years, and today many experts recommend initial observation. However, antibiotic prescribing should be considered in children aged <2 years or if AOM is accompanied by discharging ear. Objectives: To investigate the quality of treatment of AOM in general practice and to explore the influence of selected GP and patient characteristics on antibiotic prescribing. Methods: During the winter>2008, a prospective registration of patients diagnosed with AOM was conducted in general practice in Lithuania, Kaliningrad, Spain, Argentina, Sweden and Denmark. Some 1175 patients diagnosed with AOM were registered. Information about age and sex of the patient, duration of symptoms (days), temperature >38.5°C, ear discharge and the antibiotic treatment given was recorded. Results: Danish GPs had the lowest antibiotic prescription rate for AOM [72.7% (95% confidence interval (CI) = …2>

Genetic testing for inherited cancer susceptibility is an increasingly valuable tool in medical oncology. The identification of mutations in the BRCA1 and BRCA2 genes, for example, leads to specific clinical recommendations for screening or surgery that can improve survival.1 However, genetic testing can also identify variants of uncertain significance (VUS), changes in the genetic code that have an unknown effect on disease risk. Roughly 7% of women tested for BRCA1 and BRCA2 have one of these variants.2 Test results with VUS increase, rather than decrease, uncertainty for the patient in a situation with particularly high stakes and present a challenge …

A Nursing Perspective On Simulation And Interprofessional Education (Ipe): A Report From The National League For Nursing’S Think Tank On Using Simulation As An Enabling Strategy For Ipe, Janet Willhaus
Janet Willhaus
The concept of interprofessional teamwork and education is not new. Health professionals and commissions have been recommending a team approach to health care for decades. In the 1950’s, Silver (1958) supported changes to medical education and advocated for a team approach for preventative and therapeutic family care. In the 1960’s, Szasz (1969) found little integration in learning among the health professions and proposed a plan for how interprofessional education (IPE) might be accomplished. The first Institute of Medicine (IOM) conference in 1972 produced a report that discussed the importance of establishing substantive relationships between educational programs for the health professions …

Stigma of Mental Illness-1: Clinical Reflections Amresh Shrivastava*, Megan Johnston**, Yves Bureau*** ABSTRACT Although the quality and effectiveness of mental health treatments and services have improved greatly over the past 50 years, therapeutic revolutions in psychiatry have not yet been able to reduce stigma. Stigma is a risk factor leading to negative mental health outcomes. It is responsible for treatment seeking delays and reduces the likelihood that a mentally ill patient will receive adequate care. It is evident that delay due to stigma can have devastating consequences. This review will discuss the causes and consequences of stigma related to mental …

The Effects Of Word Length, Articulation, Oral-Motor Movement, And Lexicality On Gait: A Pilot Study, K. Davie, Janis Cardy, J. Holmes, M. Gagnon, A. Hyde, M. Jenkins, Andrew Johnson
Andrew M. Johnson
Previous research has suggested that articulatory demands are important predictors of the impact of dual-task interference on spatial-temporal parameters of gait. In this study, we evaluated the effects of word length, oral-motor movement, articulation, and lexicality, within a verbal task, on a continuous gait task. Fifteen healthy young women participated in a study in which two word lengths (monosyllabic and bisyllabic) were crossed with four levels of secondary task complexity (no dual-task, non-speech movement, spoken non-word, and spoken word). Spatial and temporal parameters of gait were measured using a 23' instrumented carpet. Results indicated a significant multivariate main effect for …

Succeeding In The First Year Of Practice: Heed The Wisdom Of Novice Nurses, Genevieve Chandler
Genevieve E. Chandler
The transition from student to nurse has been described as traumatic, confusing, and shocking. The difficulties encountered by the graduates have led to the premature termination of their first position, and sometimes they leave nursing altogether. To coach new nurses in preparation for their first year of practice using an appreciative inquiry framework, this study focused on the new graduates' perspective of the processes that enabled them to successfully integrate into their new role. From the analysis of 36 interviews, three themes were identified: "They were there for me," "There are no stupid questions," and "Nurturing the seeds." New nurses …

Oral Nifedipine Versus Intravenous Labetalol For Acute Blood Pressure Control In Hypertensive Emergencies Of Pregnancy: A Randomised Trial, Siti Zawiah Omar
Siti Zawiah Omar
Objective To compare oral nifedipine with intravenous labetalol in their rapidity to control hypertensive emergencies of pregnancy. Design A double-blind randomised trial. Setting A university hospital in Malaysia. Population Pregnant women with severe gestational hypertension >= 160/110 mmHg who required immediate treatment. Methods Patients were randomised to receive nifedipine (10 mg tablet, orally, up to five doses) and intravenous placebo saline injection or intravenous labetalol injection (in an escalating dose regimen of 20, 40, 80, 80 and 80 mg) and a placebo tablet every 15 minutes until the target blood pressure of <= 150/100 mmHg was achieved. Crossover treatment was effected if the initial treatment regimen was unsuccessful. Main outcome measure The time taken to achieve a blood pressure of <= 150/100 mmHg. Results The median time taken to achieve target blood pressure was 30 minutes (interquartile range, IQR 22.5-67.5 minutes) versus 45 minutes (IQR 30-60 minutes) for nifedipine and labetalol, respectively (P = 0.59). Repeated measures analysis of variance indicated that in the first hour both systolic (F = 87.6, P < 0.001) and diastolic (F = 55.8, P < 0.001) blood pressure significantly decreased, but there was no difference between the nifedipine and labetalol groups for both systolic (F = 0.12, P = 0.74) and diastolic (F = 0.92, P = 0.34) blood pressure trends over time. Crossover treatment was required in 20% of women from each group. Conclusions Oral nifedipine and intravenous labetalol regimens are similarly effective in the acute control of severe hypertension in pregnancy.
Certified Professional Co-Active Coaches: Why They Enjoy Coaching, Courtney Newnham-Kanas, Don Morrow, Jennifer Irwin
Certified Professional Co-Active Coaches: Why They Enjoy Coaching, Courtney Newnham-Kanas, Don Morrow, Jennifer Irwin

The evidence-base for the practice of coaching continues to flourish, despite the fact that very little is known about the practitioners (i.e. the coaches) themselves. It is of value to understand how coaches perceive their practice. Such information can be utilized to create a common knowledge-base about coaches that can be used, in turn, to track trends and forward research that evaluates coaching services. As the use of Co-Active coaching in facilitating behaviour change continues to rise it becomes important to learn more about Certified-Professional Co-Active Coaches (CPCC). Therefore, the purpose of this study is to evaluate what CPCCs enjoy …

Stigma Of Mental Illness-2: Non-Compliance And Intervention, Amresh Srivastava

Stigma of Mental Illness-2: Non-compliance and Intervention Amresh Shrivastava*, Megan Johnston**, Yves Bureau*** ABSTRACT The consequences of stigma are preventable. We argue that individual attention should be provided to patients when dealing with stigma. Also, in order to deal with the impact of stigma on an individual basis, it needs to be assessed during routine clinical examinations, quantified and followed up to observe whether or not treatment can reduce its impact. A patient-centric anti-stigma programme that delivers the above is urgently needed. To this end, this review explores the experiences, treatment barriers and consequences due to stigma. We also offer …

Background: Discussion of the influence of money on bioethics research seems particularly salient in the context of research on the ethical, legal, and social implications (ELSI) of human genomics, as this research may be financially supported by the ELSI Research Program. Empirical evidence regarding the funding of ELSI research and where such research is disseminated, in relation to the specific topics of the research and methods used, can help to further discussions regarding the appropriate influence of specific institutions and institutional contexts on ELSI and other bioethics research agendas. Methods: We reviewed 642 ELSI publications (appearing between 2003 and 2008) …

Effects Of Olanzapine On Muscarinic M3 Receptor Binding Density In The Brain Relates To Weight Gain, Plasma Insulin And Metabolic Hormone Levels, Katrina Weston-Green, Xu-Feng Huang, Jiamei Lian, Chao Deng
Xu-Feng Huang
The second generation antipsychotic drug (SGA) olanzapine has an efficacy to treat schizophrenia, but can cause obesity and type II diabetes mellitus. Cholinergic muscarinic M3 receptors (M3R) are expressed on pancreatic β-cells and in the brain where they influence insulin secretion and may regulate other metabolic hormones via vagal innervation of the gastrointestinal tract. Olanzapine's M3R antagonism is an important risk factor for its diabetogenic liability. However, the effects of olanzapine on central M3Rs are unknown. Rats were treated with 0.25, 0.5, 1.0 or 2.0 mg olanzapine/kg or vehicle (3×/day, 14-days). M3R binding densities in the hypothalamic arcuate (Arc) and …
