Toward A Standardized Language To Describe Perioperative Nursing, Victoria Steelman, G. Bulechek, J. Mccloskey
Toward A Standardized Language To Describe Perioperative Nursing, Victoria Steelman, G. Bulechek, J. Mccloskey
Victoria J. Steelman
The Nursing Interventions Classification (NIC) is a standardized language that identifies all interventions performed by nurses. Each nursing intervention has a label name, a conceptual definition, and a set of defining activities. The NIC provides a standardized nomenclature for automated databases, describes and measures nursing's contribution to health care, facilitates nursing education, supports clinical decision making, plans resource allocation, and facilitates nursing research. Perioperative nurses can use the NIC to describe and document their contributions to surgical patients' outcomes.

Relaxing To The Beat: Music Therapy In Perioperative Nursing, Victoria Steelman
Victoria J. Steelman
1. Music therapy is a research-based nursing intervention that can be used in perioperative nursing to reduce anxiety and prevent postoperative complications. 2. Music therapy provides patients with some degree of control and the opportunity to participate in their own care. 3. The use of headphones allows the patient to listen to the music without the distraction of other sounds.

An Approach To Managing Latex Allergy In The Health Care Worker, B. Muller, Victoria Steelman, P. Hartley, T. Casale
Victoria J. Steelman
IgE-mediated sensitivity (true allergy) to latex occurs in well-defined groups. The common feature is a high degree of repeated exposure to latex. Approximately one percent of the total U.S. population and five to 17 percent of health care workers are affected. The objective of this article is to provide a rational approach to managing latex allergy in health care workers. Information is provided on the basis of a review of recent literature (primarily since 1986) and on the basis of work by the University of Iowa Hospitals and Clinics Multidisciplinary Latex Allergy Committee. This article summarizes a proactive, hospital-wide approach …

OBJECT: The risk of surgical site infection (SSI) after craniotomies or craniectomies in patients in whom contaminated bone flaps have been reimplanted has not been determined. The objectives of this study were to identify the prevalence of bone flaps with positive cultures--especially those contaminated with Propionibacterium acnes--to assess the risk of SSI after reimplanting (either during the initial operation or subsequently) bone flaps with positive cultures, and to identify risk factors for SSI following the initial craniotomies or craniectomies. METHODS: The authors conducted a retrospective review of cases in which patients underwent craniotomy/craniectomy procedures between January and October 2007 in …
Case Report Of Latex Aerosolization From A Transesophageal Echocardiogram Machine, B. Muller, Victoria Steelman
Case Report Of Latex Aerosolization From A Transesophageal Echocardiogram Machine, B. Muller, Victoria Steelman
Victoria J. Steelman
Aerosolized natural rubber latex proteins produce latex sensitization and can cause acute allergic reactions in susceptible individuals. The objective of this study is to describe measures that should be taken to ensure a latex-safe hospital environment. A case of latex-induced anaphylaxis prompted a survey of air quality in acute care areas of a major tertiary health care center that had eliminated the use of powder-free latex gloves years earlier. Six air samples were collected using pre- and postcalibrated sampling pumps operating at 2.7 L/minute. Samples were collected in duplicate on three-piece 37-mm Teflon filters in open-faced cassettes and tested for …

The Iowa Model of Research-Based Practice to Promote Quality Care was originally developed and implemented at the University of Iowa Hospitals and Clinics. Since its inception, the model has been used in numerous settings, and users of the model have provided feedback for improving it. This article discusses how the original model is used and describes the revised model; The Iowa Model of Evidence-Based Practice to Promote Quality Care. Copyright © 2001 by W.B. Saunders Company

Issues In Sterilization And Disinfection, Victoria Steelman
Issues In Sterilization And Disinfection, Victoria Steelman
Victoria J. Steelman
Urology nurse managers and practitioners play a key role in minimizing the risk of infection to patients. It is essential that managers understand the processes of sterilization and disinfection, precautions, recommendations, and related issues to establish appropriate policies and procedures. It is equally important for practitioners to understand sterilization and disinfection and the rationale for practice guidelines. This area is dynamic, with new products and technology being introduced into the market each year. Along with these innovations will be additional information, and possibly revisions in recommendations. Therefore, urology nurses must remain current with scientific literature and national standards.
Prion Diseases--An Evidence-Based Protocol For Infection Control, Victoria Steelman
Prion Diseases--An Evidence-Based Protocol For Infection Control, Victoria Steelman
Victoria J. Steelman
Prion diseases are fatal, infectious, neurodegenerative disorders with special implications for infection control in the OR. The causative agent is highly resistant to disinfection and sterilization processes and has been transmitted during health care interactions. It is important to use evidence gained through research and case reports to minimize risk of infection. This article describes an infection control protocol developed for identifying high-risk patients, providing perioperative care, decontaminating the OR, and protecting health care personnel. This protocol provides multidisciplinary team members with a guideline for preventing transmission of these fatal diseases.
Creutzfeld-Jakob Disease: Recommendations For Infection Control, Victoria Steelman
Creutzfeld-Jakob Disease: Recommendations For Infection Control, Victoria Steelman
Victoria J. Steelman
Creutzfeld-Jakob disease, an infectious, progressive, degenerative neurologic disorder, has a presumably long incubation period but a rapid, fatal course. Brain tissue at autopsy resembles that seen in spongioform encephalopathies of other species. Creutzfeld-Jakob disease is transmitted by a proteinaceous infectious agent, or "prion." Epidemiologic patterns remain uncertain; various studies have reported conflicting risk factors in different populations, and genetic susceptibility may be involved. Although natural transmission routes are still unclear, both iatrogenic and nosocomial transmissions have been identified. Transmission has occurred through contaminated electrodes, contaminated biologic products from cadaveric brains, and infected donor tissues, including dura mater and corneas. Because …

Ethylene Oxide. The Importance Of Aeration, Victoria Steelman
Victoria J. Steelman
Residual EO on instruments and supplies can cause serious patient injuries. It is imperative that these items be adequately aerated before they are used. Removing instruments prematurely from an aerator is, indeed, "a blatant disregard for patient safety." The perioperative nurse has the responsibility and accountability for patient protection. Preventive measures and alternatives to using improperly aerated instruments have been described. If these measures do not resolve the problem, the surgery must be postponed until adequately aerated instruments are available.

This article describes the Iowa Model of Research in Practice, a heuristic model used at the University of Iowa Hospitals and Clinics for infusing research into practice to improve the quality of care. The components of the model are presented with examples. The impact of the model on patient, staff, and fiscal outcomes is delineated.

Creating more sophisticated models that describe and value nursing care will enable more sophisticated management of nursing resources. The authors used the Nursing Interventions Classification (NIC) as the basis for describing actions performed by nurses. Nursing experts evaluated each intervention in order to assign the time required for each task and the minimum level of education required to perform each task. Fifty-four percent of the interventions were determined to require fewer than 30 minutes. Over 70% of the interventions in the NIC were deemed to require basic RN education to per-form them safely and appropriately. With the time and skill …
