Increased access to a variety of foods in low-and-middle-income countries (LMICs) has led to greater autonomy in food choice decision-making. Autonomy allows individuals to make decisions through negotiation of considerations in ways that are consistent with basic values. The aim of this study was to identify and describe how basic human values drive food choice in two diverse populations with transitioning food environments living in the neighboring East African countries of Kenya and Tanzania. The Los Angeles Diversion, Outreach, and Opportunities for Recovery (LA DOOR) program is designed by the Los Angeles City Attorney's Office (LACA) to provide a comprehensive, health-focused, preventative approach that proactively engages individuals at elevated risk of returning to LACA on a new misdemeanor offense. In this study, the authors document the findings of a process and outcome evaluation of Cohort 2 of the LA DOOR program.

手冊源起
香港面對人口高齡化,醫療和福利系統無可避免面臨持續的挑戰。照顧者因壓力問題而引發的家庭悲劇時有發生,反映照顧者自身亦極需要社會的重視和支援。
現時照顧者人數眾多,當中不乏配偶、父母、子女、女婿、媳婦,對長者照顧及社會無酬的付出,確實值得社會的認同和肯定。然而,儘管本港為長者及照顧者提供不少支援服務,由於不同的原因,照顧者似乎未有善用現有的服務,常常需要一力承擔所有照顧的責任,這無疑增加了他們的壓力。
照顧者需求多樣性
照顧者有不同種類,所需要的服務或支援亦有所不同。例如:「新手照顧者」剛開始接觸照顧工作,需要盡快提升日常護理和相關的疾病知識,以及獲得合適的離院/復康服務支援。「隱蔽照顧者」一直承擔照顧工作但沒有接受任何支援,因此,他們需要的是服務提供者/鄰里的主動接觸,並被轉介合適的照顧服務,以分擔他們部分照顧工作。「高危照顧者」幾乎長時間被照顧的工作佔據了其生活,他們需要的是喘息空間和暫託服務,甚至可能是情緒支援。即使「資深的照顧者」也可能需要喘息或暫託的支援服務去減輕壓力;而結束了照顧工作的「畢業照顧者」,無論是情緒的支援,還是重新規劃財務與就業的安排,都有非常實際的需求。基於照顧者的不同需要,照顧者的支援和服務也是多樣化,如何讓不同種類的照顧者能夠輕易了解現有的社區支援服務、可考慮甚麼因素選取相關服務、如何獲取所需的支援包括申領經濟支援或預早準備長期護理開支、如何自強增值等,都是這本手冊希望解答的問題。
手冊的目的
編製這本手冊的目的就是要減少照顧者四處尋找資訊的麻煩,提供一個整全且易於瀏覽的資訊來源。本手冊主要有兩大目標:其一,幫助照顧者認識不同的服務和資源,積極尋求援助;其二是一站式提供豐富的資訊服務,提升照顧者的能力和平衡身心健康。
手冊的意義
此手冊的社會意義是期望提高大眾對照顧者需要的關注,並引導更多的行動以支援照顧者。如果您的親友是照顧者,又或您認識的朋友甚至鄰居正面對照顧家人的困難,您可以主動伸出援手,又或利用這本手冊作為啟示,向他們介紹相關的服務。
另外,這本手冊可以視作為支援照顧者的工具,讓照顧者明白在承擔照顧家人的重任時並不需要孤軍奮戰,也可以善用社區提供的各種資源和服務。
手冊架構
本手冊由四個主要部分組成,每部分都專注於特定的主題,旨在提供照顧者的全方位支援和資訊,幫助他們在面對挑戰時能夠有所依賴,並從中獲得力量:
第一部分主要提供基礎知識,其中包括不同種類的照顧者的介紹、四種長者常見疾病(包括認知障礙、中風、老年抑鬱和癌症)的病徵說明、以及照顧者可能面臨的壓力與「喘息服務」的重要性。第二部分專注介紹長者常用的社區及健康服務。我們按照地區將服務機構進行了分類,羅列「認知障礙、中風、老年抑鬱、癌症」常用的服務機構,此外,我們還提供家居安全、樂齡科技租賃服務,以及財務支援等相關建議。第三部分將焦點重新放回照顧者身上,提供了支援照顧者的社福機構介紹、照顧者同路人的心得分享,以及照顧者的培訓資訊,旨在幫助照顧者在面對困難時能自強不息。最後,在本手冊的結尾部分,我們特別邀請了四位專家學者就照顧者政策提出見解和建議,為加強照顧者支援服務提供新思路。
如果按持份者來分類,手冊的四個部分的重點如下:
第一部分:照顧者的基礎知識
◎ 針對預備照顧者、新手以及隱蔽照顧者,讓他們了解照顧者的角色和需要;照顧體弱長者的基礎知識,並提供應對壓力的建議,包括自我照顧和喘息服務等。
第二部分:照顧者支援
◎ 針對所有護老者,提供長者常用的社區服務、健康服務和支援計劃,包括暫託、住宿、護送及陪診、善別支援、情緒支援、復康用品津貼及租賃的支援計劃,以及財務策劃師提供的理財貼士。
第三部分:照顧者自強
◎ 針對照顧者,資訊包括不同的照顧者支援組織、同路人和畢業照顧者的經驗心得、培訓課程,以及實用的電子資訊平台推介。
第四部分:對照顧者支援政策的看法和展望
◎ 針對服務提供者、政策倡議者及制定者,提供專家學者對照顧者服務及政策的未來發展方向的看法和建議,以期引發更多關於「如何為照顧者提供更好支持」的討論。

BACKGROUND: Converging evidence suggests that elevated inflammation may contribute to depression. Yet, the link between peripheral inflammation and neuroinflammation in depression is unclear. Here, using data from the UK Biobank, we estimated associations among depression, C-reactive protein (CRP) as a measure of peripheral inflammation, and neuroinflammation as indexed by diffusion basis spectral imaging-based restricted fraction (DBSI-RF).
METHODS: DBSI-RF was derived from diffusion-weighted imaging data (N = 11,512) for whole-brain gray matter (global-RF), and regions of interest in the bilateral amygdala (amygdala-RF) and hippocampus (hippocampus-RF), and CRP was estimated from blood (serum) samples. Importance: It remains unclear why lesions in some locations cause epilepsy while others do not. Identifying the brain regions or networks associated with epilepsy by mapping these lesions could inform prognosis and guide interventions.

Alzheimer’s disease (AD) is the most common cause of dementia. In the United States, AD affects about 6.2 million Americans and is projected to reach 13.8 million by 2050. Currently, there are no effective therapies for AD and therapies targeting epigenetic mechanisms have shown to be effective in improving cognitive performance in animal models of AD. However, until now no study has reported sex-dependent mechanisms leading to differential expression of genes that contribute to changes in AD pathology. Using descriptive and quality descriptive (QD) analysis, emergency department nurses from around the country completed a modified version of the Physician Readiness to Manage Intimate Partner Violence Survey (PREMIS) tool to gain insight on their attitudes, knowledge, and preparedness about working with patient survivors of intimate partner violence (IPV) regarding their willingness to and comfort in engaging in IPV screening practices. In total, 1,523 responses were received from emergency department nurses currently or formerly in practice.

In this dissertation, I argue that the discussion surrounding the role of values in the conceptualization of mental disorders is reproduced in critical debates surrounding the issue of reliability and validity in a psychiatric nosology. In so doing, I oppose the conventional separation of the demarcation problem and the crisis of validity in psychiatric classification, arguing instead that they are inextricably linked. After drawing out the intersection between these literatures in the conceptual history of the DSM and its much-maligned transition to an etiologically neutral taxonomy of mental disorders, I consider Carl Hempel’s proposed conciliation between descriptive and theoretical systematization …

Background:
Learning to effectively debrief with student learners can be a challenging task. Currently, there is little evidence to support the best way to train and evaluate a debriefer's competence with a particular debriefing method.
Purpose:
The purpose of this study was to develop and test an asynchronous online distributed modular training program with repeated doses of formative feedback to teach debriefers how to implement Debriefing for Meaningful Learning (DML).

PURPOSE: The Audio-Visual Assisted Therapeutic Ambience in Radiotherapy (AVATAR) system was the first published radiation therapy (RT)-compatible system to reduce the need for pediatric anesthesia through video-based distraction. We evaluated the feasibility of AVATAR implementation and effects on anesthesia use, quality of life, and anxiety in a multicenter pediatric trial.
METHODS AND MATERIALS: Pediatric patients 3 to 10 years of age preparing to undergo RT at 10 institutions were prospectively enrolled. Children able to undergo at least 1 fraction of RT using AVATAR without anesthesia were considered successful (S). UNLABELLED: Mammographic percentage of volumetric density is an important risk factor for breast cancer. Epidemiology studies historically used film images often limited to craniocaudal (CC) views to estimate area-based breast density. More recent studies using digital mammography images typically use the averaged density between craniocaudal (CC) and mediolateral oblique (MLO) view mammography for 5- and 10-year risk prediction. The performance in using either and both mammogram views has not been well-investigated. Excerpt: Hearing loss is a major worldwide health issue affecting an estimated 1.5 billion people. Causes of hearing loss include genetics, chemicals, medications, lifestyle habits such as smoking, and noise. Noise is probably the largest contributing factor for hearing loss. Noise arises from the workplace, ambient environment, and leisure activities. The easiest noise sources to control are workplace and environmental. Workplace noise is unique in that the employer is responsible for the noise and the worker. Also, workers may be exposed to much higher levels of noise than they would accept elsewhere. BACKGROUND: The aim of this study was to examine the sports’ related oral health behavior and the effect of sports drinks on the salivary volume and acidity of 6 to 14 years old children.
METHODS: Sixty-eight children with a median age of 8 years old from Waterford Tennis Association camp participated in the study. Each child was randomly assigned to either the control group that consumed water or the experimental group that consumed a sports drink. Objectives
Postoperative oral health-related quality of life is procedure-dependent and may vary during early healing. There is scarce evidence on patient-reported outcome measures (PROMs) after extraction and guided bone regeneration (GBR) or on the clinical parameters influencing PROMs. This prospective observational study aimed to evaluate PROMs during the first 2 weeks following extraction and GBR and correlate them with clinical parameters.
Methods
Patients undergoing extraction and GBR (bone graft and resorbable membrane) at a single tooth-bound site were recruited. PROMs (pain, swelling, difficulty of mouth opening, and OHIP-14) were recorded immediately preoperatively, and at 2, 7, and 14 days postoperatively. …

Condylar hyperplasia (CH) is a rare condition characterised by excessive unilateral growth of the mandibular condyle after cessation of growth on the contralateral side causing facial asymmetry, being more prevalent in the second and third decades.

Hyperkalemia refractory to standard temporization measures can be life-threatening, and urgent hemodialysis is often utilized as a final resort. Our patient presented with hyperkalemia that was multifactorial in etiology, with acute kidney injury complicated by adrenal insufficiency. Her hyperkalemia was refractory to temporization and excretion agents, and hemodialysis was being considered. Given a recent infection, surgery, and borderline hypotension with low adrenocorticotropic hormone, there was a concern for adrenal insufficiency. However, a full investigation for secondary adrenal insufficiency via magnetic resonance imaging could not be conducted as the patient suffered from claustrophobia. Development of the affective domain is linked to increasing self-awareness of internal dialogue, such as biases and assumptions, which can affect the ability of nurses to engage in person-centered care and meet the identified patient outcomes. Through visual thinking strategies (VTS) paired with art-based pedagogy to examine grief, loss, and bereavement concepts in a hospice and palliative care nursing virtual education course, affective domain development was advanced. Background: Despite the high prevalence of neurological complications and mortality associated with extracorporeal cardiopulmonary resuscitation (ECPR), neurologically-focused animal models are scarce. Our objective is to review current ECPR models investigating neurological outcomes and identify key elements for a recommended model.
Methods: We searched PubMed and four other engines for animal ECPR studies examining neurological outcomes. Inclusion criteria were: animals experiencing cardiac arrest, ECPR/ECMO interventions, comparisons of short versus long cardiac arrest times, and neurological outcomes.
Results: Among 20 identified ECPR animal studies (n = 442), 13 pigs, 4 dogs, and 3 rats were used. Only 10% (2/20) included both sexes. …

OBJECTIVE: The objective of this systematic review and meta-analysis was to assess the efficacy of exercise in neuropathic pain following traumatic spinal cord injuries.
METHODS: The search was conducted in MEDLINE, Embase, Scopus, and Web of Science by the end of 2022. Two independent researchers included the articles based on the inclusion and exclusion criteria. A standardized mean difference was calculated for each data and they were pooled to calculate an overall effect size. Proponents suggest these estimates can be obtained at high frequencies and assist with manipulating loads according to session-by-session fluctuations. SARS-CoV-2 appears to induce diverse innate and adaptive immune responses, resulting in different clinical manifestations of COVID-19. Due to their function in presenting viral peptides and initiating the adaptive immune response, certain Human Leucocyte Antigen (HLA) alleles may influence the susceptibility to severe SARS-CoV-2 infection. Objectives: Understanding speech-in-noise (SiN) is a complex task that recruits multiple cortical subsystems. Individuals vary in their ability to understand SiN. This cannot be explained by simple peripheral hearing profiles, but recent work by our group (Kim et al. 2021, Neuroimage) highlighted central neural factors underlying the variance in SiN ability in normal hearing (NH) subjects. The present study examined neural predictors of SiN ability in a large cohort of cochlear-implant (CI) users. Psoriatic arthritis has been known to cause many inflammatory clinical features, including synovitis. In the past, synovitis has been primarily treated with NSAIDs, corticosteroid injections, and synovectomy when needed. This case report outlines a 49-year-old female with synovitis caused by psoriatic arthritis who did not respond to conventional treatment options and was apprehensive to surgery. She was offered a platelet rich plasma (PRP) injection as alternative therapy to a synovectomy. We study the effect of community access to mental health and substance use treatment on police officer safety, which we proxy with on-duty assaults on officers. Police officers often serve as first-responders to people experiencing mental health and substance use crises, which can place police officers at risk. Combining agency-level data on police officer on-duty assaults and county-level data on the number of treatment centers that offer mental health and substance use care, we estimate two-way fixed-effects regressions and find that an additional four centers per county (the average annual increase observed in our data) leads to a 1.3% reduction …

Seizures are common in neonates, but there is substantial management variability. The Neonatal Task Force of the International League Against Epilepsy (ILAE) developed evidence-based recommendations about antiseizure medication (ASM) management in neonates in accordance with ILAE standards. Six priority questions were formulated, a systematic literature review and meta-analysis were performed, and results were reported following the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) 2020 standards. Childhood lead poisoning is one of the most preventable environmental health hazards in history. While childhood lead poisoning rates have decreased substantially since the 1970s, mounting evidence suggests that chronic, low-level exposure in early childhood can have long-lasting impacts on children. In Nevada, there are over 218,000 children under the age of 6 but only 3% are tested for lead – making Nevada one of the lowest testing states in the United States (U.S.).
