Introduction: Children growing up in arid and semi-arid regions of Sub-Saharan Africa (SSA) face heightened risks, often resulting in poor developmental outcomes. In Kenya, the arid and semi-arid lands (ASAL) exhibit the lowest health and developmental indicators among children. Despite these risks, some children grow up successfully and overcome the challenges. Adolescents across the United States are struggling with their mental health in record numbers, often without access to adequate support. This paper examines the role of schools as critical access points for all students to receive mental health interventions. I propose the implementation of a school-wide, student-led mental health program that every student participates in during homeroom, with the additional support of a dedicated mental health space. This study aims to introduce a measurement tool into the literature that can reveal the factors affecting the joy of life in older adulthood. Participants included 266 individuals aged 55 and over living in Izmir, Turkey. The process of developing the scale includes the following stages: problem identification, item pool creation, expert opinion, form shaping, pilot application, and finalizing the scale. The exploratory factor analysis revealed that the scale consists of 6 different dimensions, and the variance explained by these dimensions was found to be 65.184%. The Drone Medical Package Delivery for Improved Transportation and Better Patient Outcomes planning and prototyping grant involves five partners, including Accomack-Northampton Planning District Commission (A-NPDC), Riverside Health System, Old Dominion University (ODU), Virginia Institute for Spaceflight & Autonomy (VISA), Virginia Innovation Partnership Corporation (VIPC), and DroneUp. Background: Despite efforts to increase recruitment and enrollment, the physical therapy profession lacks Black students and clinicians. Racially marginalized students have higher rates of academic difficulty during physical therapy education (PTE) than White students. Mental health and belongingness have been proposed as prerequisites for reaching academic achievement and attaining one’s full potential. Comparative Analysis Of Gut Microbiota In Post-Mortem Chronic Alcoholics Versus Minimal To No Alcohol Users In The Thai Population, Laiba Pervez

Chronic alcohol consumption induces long-term disruption of the gut microbiota, contributing to inflammation, metabolic imbalance, and impaired intestinal integrity. This study investigated 24 postmortem gut microbiota samples from the Thai population, including 12 chronic alcoholic and 12 non-alcoholic cadavers, focusing on cecal and fecal samples to identify site-specific microbial signatures of alcohol-associated dysbiosis. Following DNA extraction, the V3–V4 regions of the 16S rRNA gene were sequenced on the Illumina MiSeq platform. Data were processed in QIIME for OTU clustering and taxonomic profiling. Alpha (Shannon, Chao1, ACE) and beta (Bray–Curtis) diversity were analyzed with PERMANOVA (p< 0.05) while LEfSe, cladogram, and STAMP analyses identified differentially abundant taxa.. At the phylum level, alcoholic cecal samples (AC) showed a decrease in Firmicutes (59.07%) and increases in Proteobacteria (5.54%), Fusobacteria (8.48%), and Actinobacteria (2.78%) compared with controls (NAC). The rise in Actinobacteria was statistically significant. The class Bacilli was also elevated, reflecting a more oxygenated and inflammatory intestinal environment. At the genus level, Paeniclostridium was enriched in alcoholics, while the butyrate-producing Coprococcus was reduced, suggesting impaired SCFA metabolism and gut barrier function. In fecal samples (AF vs NAF), phylum-level differences were minimal, although Clostridium sensu stricto 1 was notably higher in alcoholics, consistent with inflammation-associated dysbiosis. When comparing gut regions, distinct spatial heterogeneity was observed. Within the alcoholic group, Firmicutes were more abundant in cecum, while Actinobacteria were lower compared to feces. Among controls, Actinobacteria and Bacteroidetes were enriched in feces, whereas Firmicutes predominated in the cecum. Alpha diversity analysis using ACE and Chao1 indices showed significant differences between cecal and fecal samples of the control group. Overall, postmortem gut microbiota retained distinct alcoholassociated signatures, with the cecum preserving clearer microbial patterns than feces. These findings highlight the cecum’s potential as a reliable site for detecting alcohol-related dysbiosis in forensics.

การศึกษานี้เป็นการวิจัยกึ่งทดลองแบบวัดเฉพาะหลังการทดลองมีกลุ่มเปรียบเทียบ (Posttest-only Control Group Design) โดยมีวัตถุประสงค์เพื่อประเมินผลของโปรแกรมการส่งเสริมความสุขสบายต่อคุณภาพการฟื้นตัวหลังการผ่าตัดศัลยกรรมกระดูกในเด็กวัยเรียน กลุ่มตัวอย่างเป็นเด็กอายุ 7–12 ปี จำนวน 40 ราย ที่เข้ารับการผ่าตัดใหญ่ศัลยกรรมกระดูกส่วนรยางค์และพักรักษาในหอผู้ป่วยกุมารศัลยกรรมกระดูก สถาบันสุขภาพเด็กแห่งชาติมหาราชินี โดยแบ่งเป็นกลุ่มทดลอง 20 ราย ซึ่งได้รับโปรแกรมการส่งเสริมความสุขสบาย และกลุ่มควบคุม 20 ราย ซึ่งได้รับการพยาบาลตามปกติ เครื่องมือวิจัยประกอบด้วย (1) โปรแกรมการส่งเสริมความสุขสบายแบบองค์รวมที่พัฒนาตามแนวคิดความสุขสบายของ Kolcaba (1992) (2) แบบประเมิน Children’s Comfort Daisies และ (3) แบบประเมินคุณภาพการฟื้นตัวหลังผ่าตัด 15 (Quality of Recovery–15: QoR–15) เครื่องมือทั้งหมดผ่านการตรวจสอบคุณภาพ โดยมีค่าดัชนีความตรงตามเนื้อหา (CVI) เท่ากับ 1.00, 1.00 และ 0.96 ตามลำดับ และแบบประเมิน QoR–15 มีค่าความเที่ยง Cronbach’s alpha เท่ากับ 0.86 การวิเคราะห์ข้อมูลใช้สถิติ Independent t-test ที่ระดับนัยสำคัญทางสถิติ .05 ผลการวิจัยพบว่า เด็กในกลุ่มทดลองมีคะแนนคุณภาพการฟื้นตัวหลังผ่าตัดเฉลี่ยในช่วง 48 และ 72 ชั่วโมง สูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p < .001) โดยที่ 48 ชั่วโมงหลังผ่าตัด กลุ่มทดลองมีคะแนนเฉลี่ย 130.80 (SD = 16.43) ขณะที่กลุ่มควบคุมมีคะแนนเฉลี่ย 101.45 (SD = 21.13) และที่ 72 ชั่วโมงหลังผ่าตัด กลุ่มทดลองมีคะแนนเฉลี่ย 140.90 (SD = 10.76) ขณะที่กลุ่มควบคุมมีคะแนนเฉลี่ย 118.20 (SD = 17.19) ผลลัพธ์ดังกล่าวแสดงให้เห็นว่าโปรแกรมการส่งเสริมความสุขสบายแบบองค์รวมมีประสิทธิผลในการส่งเสริมคุณภาพการฟื้นตัวหลังการผ่าตัดศัลยกรรมกระดูกในเด็กวัยเรียนอย่างมีนัยสำคัญทางสถิติ

ปัจจัยทำนายพฤติกรรมการป้องกันวัณโรคในผู้สัมผัสร่วมบ้านของผู้ป่วยวัณโรค, นงนภัส เกษรทอง

การศึกษาครั้งนี้เป็นการวิจัยแบบบรรยายเชิงทำนาย เพื่อศึกษาความสามารถของปัจจัยคัดสรรในการทำนายพฤติกรรมการป้องกันวัณโรคในผู้สัมผัสวัณโรคร่วมบ้าน โดยปัจจัย ประกอบด้วย 1) ความรู้เกี่ยวกับวัณโรค 2) ทัศนคติต่อวัณโรค 3) แรงจูงใจในการป้องกันวัณโรค 4) การรับรู้ความสามารถของตนเองในการป้องกันวัณโรค และ 5) การรับรู้อุปสรรคเกี่ยวกับการป้องกันวัณโรค กลุ่มตัวอย่างเป็นผู้สัมผัสวัณโรคร่วมบ้าน จำนวน 111 คน ซึ่งเลือกอย่างเจาะจงจากโรงพยาบาล 3 แห่งในเขตกรุงเทพมหานคร เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามชนิดตอบด้วยตนเอง วิเคราะห์ด้วยสถิติเชิงพรรณนาและการถดถอยพหุคูณ ผลการวิจัย พบว่า พฤติกรรมการป้องกันวัณโรคในผู้สัมผัสวัณโรคร่วมบ้านโดยรวม อยู่ในระดับดี (Mean = 44.2, SD = 6.5) ปัจจัยที่สามารถทำนายพฤติกรรมการป้องกันวัณโรคอย่างมีนัยสำคัญทางสถิติ ได้แก่ ทัศนคติต่อวัณโรค (b = .374, p<.05) การรับรู ้ความสามารถของตนเอง ในการป้องกันวัณโรค (b = .244, p <.05) และ การรับรู ้อุปสรรคเกี ่ยวกับการป้องกันวัณโรค (b = .166, p <.05) ปัจจัยดังกล่าวสามารถร่วมทำนายพฤติกรรมการป้องกันวัณโรคได้ร้อยละ 32.8 อย่างมีนัยสำคัญทางสถิติ (R2 = .328, p <.05) อย่างไรก็ตามในการศึกษานี้ ความรู ้เกี ่ยวกับวัณโรค และแรงจูงใจในการป้องกันวัณโรค ไม่สามารถทำนายพฤติกรรมการป้องกันวัณโรคในผู ้สัมผัสวัณ โรคร่วมบ้าน

การวิจัยแบบกึ่งทดลองนี้มีวัตถุประสงค์ เพื่อศึกษาผลของโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชันต่อพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม เบาหวาน ความดันโลหิตสูง ไขมันในเลือด 1 โรคขึ้นไป ที่มีอายุตั้งแต่ 35 – 80 ปี คัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 23 คน จับคู่ทั้งสองกลุ่มให้มีลักษณะคล้ายคลึงกันในเรื่องอายุ จำนวนโรคร่วม จำนวนกลุ่มยาที่ใช้รักษาโรค เปรียบเทียบข้อมูลพื้นฐานด้วยสถิติ Chi-square test และ Fisher’s Exact Test กลุ่มตัวอย่างทั้งสองกลุ่มมีคุณลักษณะพื้นฐานที่ไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติในทุกตัวแปร (p > .05) กลุ่มควบคุมจะได้รับการพยาบาลปกติ ในขณะที่กลุ่มทดลองได้รับโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชัน เครื่องมือที่ใช้ในการวิจัย ได้แก่ แบบสอบถามพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารของผู้ป่วยที่มีภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม และแบบสอบถามประเมินความรอบรู้ด้านสุขภาพเกี่ยวกับพฤติกรรมการจัดการตนเองด้านการบริโภคอาหาร ทดสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน ได้ค่าดัชนีความตรงตามเนื้อหาเท่ากับ 0.92 และ 1.00 และค่าความเที่ยงเท่ากับ 0.91 และ 0.73 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และสถิตทดสอบค่าที ผลการวิจัยสรุปได้ดังนี้ 1. คะแนนเฉลี่ยพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม กลุ่มทดลองหลังได้รับโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชันสูงกว่าก่อนได้รับโปรแกรม ฯ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. คะแนนเฉลี่ยพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม ที่ได้รับโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชันสูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง เพื่อศึกษาผลของโปรแกรมสนับสนุนการให้ข้อมูลแบบรูปธรรม-ปรนัยต่อความวิตกกังวลของสมาชิกครอบครัวของผู้ป่วยโรคหลอดเลือดสมองครั้งแรกในห้องฉุกเฉิน โดยกลุ่มตัวอย่างในการศึกษาครั้งนี้คือ สมาชิกครอบครัวของผู้ป่วยโรคหลอดเลือดสมองครั้งแรกที่นำส่งผู้ป่วยมารับบริการที่ห้องฉุกเฉินของโรงพยาบาลสรรพสิทธิประสงค์ แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 30 คน โดยคัดเลือกตามแบบเฉพาะเจาะจงตามเกณฑ์ที่กำหนด โปรแกรมรูปธรรม-ปรนัย และแบบสอบถาม ผ่านผู้ทรงคุณวุฒิ 5 ท่าน ได้ค่าความเที่ยง 0.84 โปรแกรมการสนับสนุนการให้ข้อมูลแบบรูปธรรม–ปรนัยต่อความวิตกกังวล มีเครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล ประกอบด้วย ข้อมูลทั่วไปของผู้ป่วยโรคหลอดเลือดสมอง และแบบประเมินความวิตกกังวลขณะเผชิญ (The State Anxiety Inventory (STAI) Form Y) วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สถิติทดสอบ Independent t-test และสถิติ paired t-test ผลการวิจัย: ผลการวิจัยพบว่าสมาชิกครอบครัวของผู้ป่วยโรคหลอดเลือดสมองที่ห้องฉุกเฉิน ในกลุ่มทดลอง ภายหลังได้รับโปรแกรมคะแนนความวิตกกังวลตํ่ากว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติ (p < .05) และคะแนนความวิตกกังวลของสมาชิกครอบครัวของผู้ป่วยในกลุ่มทดลองภายหลังได้รับโปรแกรมตํ่ากว่าในสมาชิกครอบครัวของผู้ป่วยได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติ (p < .05)

The rehabilitation of children with cerebral palsy (CP) has progressively evolved from therapist-centered, impairment-focused models to child- and family-centered approaches that emphasize real-world participation. This narrative review summarizes conceptual and practical shifts in CP rehabilitation, highlighting the distinction between motor capacity and real-world performance. Accelerometry is a critical objective tool for assessing lower and upper limb physical activity. Devices such as StepWatch and ActiGraph provide ecologically valid data on lower limb activity levels, step counts, and energy expenditure. Climate change and the rising frequency of extreme weather events (EWEs) pose significant challenges to vulnerable populations, particularly individuals with spinal cord injury (SCI). These patients are at greater risk due to impaired thermoregulation, limited mobility, poor sensation, and dependence on caregivers or adaptive equipment, increasing their exposure to climate-related hazards and disrupting healthcare access during disasters. Although climate change impacts on the general population are well-documented, research on specific disability groups like SCI patients remains limited. Background: Adhesive capsulitis causes pain and limited mobility. Hydrodilatation is an effective, minimally invasive treatment, but the optimal volume is unclear. This study compares the pain and function improvement from large-volume (40 mL) versus small-volume (10 mL) ultrasound-guided hydrodilatation in patients with adhesive capsulitis.

Methods: This study was conducted at Phramongkutklao Hospital in Bangkok, Thailand, with Thai adults aged 20 and older who had adhesive capsulitis. Oropharyngeal dysphagia is a potentially life-threatening condition, particularly affecting individuals with neurological diseases, head and neck cancers, and age-related issues. Its prevalence in the general population ranges from 2.3% to 16%, with much higher rates among patients with stroke, Parkinson’s disease, and traumatic brain injury. Dysphagia is prevalent in geriatric and long-term care settings, where over 50% of elderly residents may experience swallowing difficulties. Given its high prevalence and serious consequences, early detection through dysphagia screening in at-risk populations is essential. Methods: Fifteen healthy volunteers with no symptoms or signs of neuropathy in either upper limb were recruited for this double-blind, placebo-controlled study over a 10-month period. A lidocaine patch or a placebo patch was applied to the volar aspect of the wrist during sleep. The treatment procedure was repeated for 14 consecutive nights. Nikethamide is classified a Drug Class 1, Penalty Class A substance by the Association of Racing Commissioners International (ARCI) and as a Horseracing Integrity and Safety Authority (HISA) SO or banned Stimulant that lacks FDA approval. Purpose

The global Assessing long-teRm Outcomes in dupiluMAb (AROMA) registry study aims to characterize the long-term, real-world use of dupilumab in patients with chronic rhinosinusitis with nasal polyps (CRSwNP). This paper reports interim analysis of the baseline characteristics for the first 303 patients enrolled in AROMA.

Methods and materials

AROMA is currently ongoing in adult patients with CRSwNP who initiated dupilumab for up to 36 months. This interim analysis describes baseline demographics; systemic/oral corticosteroid (SCS/OCS) and antibiotic use; disease burden; healthcare resource utilization; and medical history. All outcomes are summarized descriptively.

Objective

(1) To investigate weight as an alternative guide for trach tube choice. (2) Evaluate complications 3-month posttracheostomy and associations with patient and trach tube factors.

Study design

Retrospective cohort study.

Setting

Single academic medical center.

Methods

Patients < 2 years old who underwent tracheostomy placement from 2017 to 2022 were identified. Associations between trach characteristics, trach size/length, chest x-ray (CXR) and intraoperative endoscopy measurements, and complications within 3 months posttracheostomy placement were evaluated.

Results

In total, 68 patients were included in the study, with a median age of 4 months (range: 1 day to 1.74 years) and weight of 4.1 kg (range: 1.8-11.6 kg) at the time of the procedure. Chronic rhinosinusitis (CRS) and allergic rhinitis (AR) are common comorbid sinonasal conditions. CRS is classically divided into two distinct phenotypes: CRS with nasal polyposis (CRSwNP) and CRS without nasal polyposis (CRSsNP). [Introduction] A 32-year-old woman with no significant past medical history presented to the emergency department with 11 days of fever, fatigue, chills, and new-onset fullness in her left flank. She denied dysuria, abdominal pain, or vaginal bleeding. Vital signs showed fever and mild hypotension, and the physical examination revealed left-sided abdominal distension. Laboratory results demonstrated leukocytosis and a lactate of 3.2 mmol/L. She appeared nontoxic but was persistently febrile despite initial fluids and antibiotics. Computed tomography—shown in Figure 1, and ultrasound—shown in Figure 2, were obtained and revealed the diagnosis.

Objective: Congenital heart defects (CHDs) are a leading cause of infant morbidity and mortality partly due to low prenatal detection rates. We evaluated whether an artificial intelligence (AI) system can improve the detection of CHDs on fetal ultrasound exams among both general OBGYNs and MFM specialists. Study Design: The AI system analyzes all grayscale 2D ultrasound cines of an exam and detects 8 morphological findings associated with severe CHDs. Objective: Late-onset fetal growth restriction (LO-FGR), defined as onset 3 32 weeks gestation, is the more common subgroup of FGR. The umbilical artery Doppler is usually unaffected. There are no subgroup-specific guidelines for monitoring pregnancies affected by LO-FGR. We compared the utility of the cerebroplacental Doppler ratio (CPR) to the Biophysical Profile (BPP), a widely endorsed FGR monitoring tool, for the prediction of adverse perinatal outcomes (APO). Study Design: This is a retrospective cohort study of LO-FGR, defined as estimated fetal weight < 10th%, from 2013-2018. Using logistic regression analysis, we determined whether low CPR < 5th% and BPP score £ 6 significantly independently predicted the composite APO defined as one or more complications: cord pH < 7.20, 5-minute Apgar score of < 5, operative or cesarean delivery secondary to non-reassuring fetal heart tracing, assisted respiration at birth, NICU admission, and/ or perinatal death. Secondarily, we also evaluated prediction of severe neonatal morbidity (SNM) defined in Table 1 (legend). Results: There were 233 cases of LO-FGR of which 76 (34%) had a low CPR percentile and 28 (13%) had a low BPP score. There were 22 (9.9%) cases with SNM of which 5 (2.1%) had a low CPR percentile and 4 (1.8%) had a low BPP score. A low CPR was associated with a significantly higher rate of individual and composite APO than cases with normal CPR percentile (p < 0.001) [Table 1]. Both a low CPR percentile and a low BPP score significantly predicted APO overall. CPR appeared to be a better predictor of the APO, controlling for gestational age at testing [Table 2]. Only BPP significantly predicted SNM: CPR (OR = 4.64, p = 0.13) and BPP (OR = 4.11, p = 0.038). Conclusion: A low CPR percentile was a significant independent predictor of perinatal complications in LO-FGR, including abnormal fetal heart rate tracing, smaller birthweight percentile, and NICU admission. CPR appeared superior to BPP overall. CPR should be considered as a fetal surveillance tool in LO-FGR.

This communication reviews the matter of the clinical, pharmacological, and regulatory significance of a 4.95 nanogram/ml identification of trichlormethiazide in urine from a horse six days after administration of a compounded trichlormethiazide/ dexamethasone preparation. This combination of dexamethasone and trichlormethiazide has historically been widely used in bovine and equine medicine to reduce local inflammation and edema. The horse was administered an oral formulation of trichlormethiazide 400 mg /dexamethasone 10 mg per packet under veterinary supervision on days 7 and 6 before the race in question.
