Acute Onset Of Bulbar And Ocular Paralysis: An Isolated Case, With Differential Diagnosis, Aziz Aghdassi, Robert D. Teasdall

The diagnosis of botulism should be considered in all patients with the acute onset of cholinergic dysfunction and muscle weakness. The pupils are usually dilated, paralysis of ocular and bulbar muscles invariably occurs along with weakness of limb and axial muscles. This diagnosis is supported by the presence of a neuromuscular block and confirmed by the detection of toxin. Although the clinical presentation in this patient supported this diagnosis and a neuromuscular defect was present, no botulinum toxin was found. Not until Clostridium botulinum, type B organisms were cultured was the diagnosis of "probable botulism" made in this patient. Myasthenia …

Salicylate Effect On Platelets And Vascular Thrombosis In Rheumatoid Arthritis, Gilbert B. Bluhm, Jeanne M. Riddle, Donald G. Pica, Gordon D. Langejans

Platelet surface activation was surveyed by electron microscopy (EM) in 40 patients with classical rheumatoid arthritis (RA) without clinical evidence of recent intravascular coagulation. Increased platelet surface activity (PSA) was noted in 8 of 25 females and 8 of 15 males (16 of 40 or 40%). Abnormal PSA failed to correlate with either the presence of rheumatoid nodules or the titer of rheumatoid factor. Hyperactive platelet populations, however, did tend to correlate with a serum urate level above 5 mg% (7 of 12 vs 9 of 28). Neither "low" nor "anti-inflammatory" levels of serum salicylate appear to afford protection from …

Quinidine-Induced Hepatitis And Thrombocytopenia, Mohsin Alam, Wolf F. C. Duvernoy, Sol D. Pickard, Paul L. Aronsohn

Quindlne-induced hepatitis has received more attention only recently whereas quinidine-induced thrombocytopenia is well recognized. Over the past two years the authors saw four patients with quinidine-hepatitis accompanied by malaise, anorexia, fever in two patients and marked elevation of liver enzymes in all. Two patients had associated quinidine-induced thrombocytopenia of 81,000 and 6,600 platelets / cu mm, respectively. Liver biopsy in two patients revealed small foci of hepatocellular necrosis, Kupffer cell hyperplasia and Kupffer cells containing lipochrome. In one patient noncaseous granulomatous lesions were seen. Symptoms subsided and laboratory tests returned to normal rapidly on withdrawal of quinidine. In one patient, …

Diagnostic Ultrasound In Gynecology, Manojkumar H. Shah, Chang Y. Lee, James J. Karo, Bruce H. Drukker

The accuracy of diagnostic ultrasound in 484 gynecologic patients was evaluated. In 97% of them, the ultrasonic findings were either diagnostic or confirmatory of the final clinical condition. The ultrasonic procedure was useful in the evaluation of pelvic or adnexal masses, confirmation and assessment of early pregnancy, location of lost Intrauterine contraceptive devices, diagnosis of tubo-ovarian abscess and hydatidiform mole. The ultrasonic study aided in developing a precise surgical approach as well as reducing the number of operative procedures performed only for diagnosis. The Incidence Of Retrolental Fibroplasia, R. Michael Nisbet

With the increased use of oxygen to treat the respiratory distress syndrome, retrolental fibroplasia was encountered in more infants. One hundred consecutive premature infants were screened for ocular disease and a diagnosis of retrolental fibroplasia was made in six infants. A profile of a "high risk" infant emerged as one with a birth weight less than 1,500 grams who suffers from the respiratory distress syndrome and multiple apneic spells. During the examinations many other ocular abnormalities were found which prove the value of such screening.

X-linked cataracts are a rare group and appear as a complication of other disease. Changes in the lens are difficult to visualize and the family history may alert the examiner to the possibility of cataractous changes. Biofeedback refers to a special type of operant conditioning (ie, a category of learning) in which subjects acquire control over physiological processes through the feedback of information. Its methodology is based upon principles of behavioral science developed in the laboratory through experimentation. It has been conclusively demonstrated that humans (and other animals) can learn to control such actions as EKG parameters, blood pressure, body temperature, EEG, individual motor units, and glandular secretions. Clinically, biofeedback has been applied to a wide variety of psycho-physiologic disorders in which a measurable response requires alteration. The purpose of this investigation was to determine if performance on a language comprehension task, varying in number of syntactical units (i.e., grammatical complexity) was affected by altered rates of speech. A total of twenty-four language disordered children, aged 7 years, 8 months, through 9 years, 8 months, who were enrolled in language/learning disorders classrooms in the Portland Public Schools served as subjects. The Assessment of Children's Language Comprehension (Foster et al., 1972) test was administered to each subject via audio-tape at one expanded (100 wpm), one normal (150 wpm), and two compressed rates (200, 250 wpm) of speech.

During the last decade, the literature on media exposure has been dominated by a single theoretical system of personality functioning—the social learning model. The model emphasizes that the disinhibition of already-learned behavior, the learning of new behaviors, and the establishment of patterns of personality result from observing the behavior of other people (models) and from vicarious reinforcement upon viewing those models successfully engaging in rewarded sequences of behavior.

Leadership potentials and personality profiles of 27 associate degree and 20 baccalaureate degree nursing students were compared, using the California Psychological Inventory (CPI) and the Managerial Key for the CPI. The mean values on each of the 18 scales of the CPI and the mean values of the Managerial Score were compared and revealed no significant difference (p=.05) between the two groups of nursing students, and were within normal range, indicating average social and intellectual adjustment. Though the associate degree students had a higher mean value on the Managerial Score than the baccalaureate students the difference was not significant (p=.05). …

A sample of 27 cases treated with CHG were selected from the files of Rocky Mountain Data Systems, Inc. and Dr. B. Fletcher. The cases were divided into two groups according to forces used during treatment to initiate and continue distal molar movement. Group I consisted of 15.cases which had been treated by Dr. Ricketts using heavy cervical traction (250-500 grams); Group II included 12 cases which Dr. Fletcher had treated with very light cervical traction (120-240 grams).

The purpose of this study was to determine: (a) the level of disagreement/agreement (role consensus) between skilled nursing facility administrators and consultant dietitians on the activities, responsibilities, and guilities which compose the ideal role (role expectations) of the consultant dietitian; (b) the extent to which administrators and consultant dietitians perceive discrepancies/consistencies between ideal (role expectations) and actual (reality) performance of the dietitian at present; and (c) the common deterrents, which consultant dietitians perceive as the basis for the dichotomy between ideal (role expectations) and actual (reality) role performance.

A comparison was made of ultrasonic filing versus hand filing using three solutions to flood the canals of 24 extracted human maxillary central incisors. A modified PR-30 Cavitron insert was used in a Cavitron 1010 unit for the ultrasonic filing. The clinical crown was removed with a diamond saw and water spray at the cemento-enamel junction. The two major groups were ultrasonic filing versus hand filing; the three subgroups were solution/solvents - 10% EDTA, 2.0% NaOCl, and distilled water.

The purpose of this study was to compare the performance of "learning disabled" (LD) children and children with no identified learning problems on tests of central auditory function. Katz's Staggered.Spondaic Word (SSW) test and Willeford's Tests of Central Auditory Function (TCAF) were administered.

The groups consisted of twenty-one LD children (experimental) and twenty-one children with no identified learning problems (control). The groups were matched for age, sex, socioeconomic background, and IQ. None of the subjects had a peripheral auditory impairment.

An experimental study was conducted with twenty-one breast-feeding primiparas during the initial four-week post-partal period to test the effect of in-home community health nursing assessment and counseling on the breast-feeding success rate. It was hypothesized that mothers receiving assessment and counseling regarding physical, informational and psychosocial needs, would be more successful than those who were not counseled.

Criteria for success were: (1) breast-feeding at four weeks postpartum, (2) an adequate milk supply as evidenced by no need for formula supplementation to keep the baby satisfied, (3) an infant weight gain of one to two pounds during the four-week period, and …

Human placental alkaline phosphatase was purified by butanol extraction, methanol precipitation and chromatography on DEAE-cellulose. The pH optimum was found to be 10.9. The enzyme was competitively inhibited by di-ethyl-p-nitrobenzyl-phosphonate and copper sulfate. An increase in enzymatic activity occurred in the presence of the phosphate ion (HPO4) and ATP.

The enzyme was crystallized from a 40% saturated solution of ammonium sulfate and shown to be homogeneous by sedimentation analysis. Homogeneity of the enzyme was also demonstrated using poly-acrylamide-gel-electrophoresis. At a pH of 7.5% the electrophoretic pattern yielded a single band and showed no significant contamination.

Endodontic therapy was evaluated as a possible control or prevention modality of radiation type dental caries in dogs. Two adult male dogs with permanent dentitions had endodontic therapy performed on alternating teeth in each jaw. The nontreated teeth served as controls.

The dogs were irradiated with 7000 rad Cobalt-60 radiation fractionated over an eight week period. The dogs were killed at 60 and 182 days post-irradiation. The teeth were removed from the jaws and immersed in methylene blue dye, sectioned, and the dye penetration pattern photographed. Diffusion of the dye into the tooth structure was quantified and statistically analyzed.

During the researcher's experience as a public health nurse, she became aware of two types of depressed women. The first type is the withdrawn, weeping, self-isolating woman while the second type cited numerous physical complaints which prevented her from caring for her home and children. Both exhibited non-responsive behavior to her children's needs.

The purpose of this study was to determine whether or not low socio-economic latino mothers displaying non-responsive behavior had levels of depression different than low socio-economic latino mothers displaying responsive behavior.

An investigation was conducted to evaluate the application of Xeroradiography for imaging the Temporomandibular Joint using the Updegrave and Weinberg positioning boards and to compare these images to those obtained by conventional radiography using the same positioning boards.

A cadaver head in which three spherical radiopaque implants had been placed was used in this study. A General Electric Model 1000 (Dental Unit) was the source of radiation. The conventional radiographs were produced on Dupont Cronex 2DC film and processed by a Phillips 810 automatic processed by a Xerox "125 System".
