Spetzler-Martin Grade IV arteriovenous malformations (AVMs) are challenging due to high risks associated with both treatment and natural progression. This study compares the outcomes of microsurgical resection and stereotactic radiosurgery (SRS) in high-grade AVMs, analyzing obliteration rates, complications, and functional outcomes. A retrospective cohort of 96 patients treated with either microsurgical resection (33 patients) or SRS (63 patients) was analyzed. Propensity-score matching was employed to account for baseline variables such as AVM size (cm), preoperative embolization and rupture status. Primary endpoints included AVM obliteration, complication rates, and modified Rankin Scale (mRS) scores. After matching, 31 patients per group were analyzed. …

INTRODUCTION: Internal Medicine residents in The United States of America (USA) need to demonstrate competency in the Accreditation Council for Graduate Medical Education (ACGME) core areas to meet training requirements. Two of these core competencies are professionalism and interpersonal/communication skills. Medical humanities can positively influence trainee development by cultivating these skills. There is limited literature on the use of art-based programs to enhance communication and professionalism skills among residents aside from the original FRAME workshop. Nurses often work in stressful environments. These stressors can have many causes and may vary between specialties. Some examples of stressors are death and dying, conflict with others, and workload. One scale that analyzes these stressful factors is the Expanded Nursing Stress Scale (ENSS). This survey uses the ENSS to examine stressors experienced by Registered Nurses and Licensed Practical Nurses who work in a hospital setting and continues by asking nurses if these factors cause continued stress at home. Nurses are essential in early detection, education, symptom management, treatment administration, and care coordination for patients with multiple sclerosis (MS). This role requires both theoretical and practical knowledge. This study evaluates the knowledge level among nurses regarding multiple sclerosis in Morocco. We conducted this cross- sectional study on 326 nurses in Morocco using a purposive sampling method, which involves selecting nursing specialties that have direct contact with patients with MS, aiming to target the most relevant professionals for the study. One of the most common challenges faced by people living with human immunodeficiency virus/acquired immune deficiency syndrome (HIV/AIDS) is a low quality of life, particularly during the coronavirus disease 2019 (COVID-19) pandemic, as they are considered a vulnerable group. This study aimed to examine the relationship between resilience, social support, and quality of life among people living with HIV/AIDS during the pandemic. 682 studies from 2012 and 1682 studies from 2022 were included. Around a quarter of studies were interventional and the remainder being observational and primary outcomes of effectiveness were more common than efficacy. Key study design aspects were frequently unreported, including sample size justification, registration, randomisation technique and funders. Our evidence map demonstrated that predominantly, investigations were of implants and were in a cardiovascular setting. Clinical investigations were broadly similar between 2012 and 2022, though there was a reduction in the proportion of cardiovascular studies, a move in the share of studies coming out of Europe and North America towards …

Ten years have passed since the last regional assessment of the state of Fogarty International Center-funded research ethics education and challenges facing research ethics in Latin America and the Caribbean basin. Program directors and faculty from four international research ethics education programs met in Spring 2024 to discuss their progress, challenges, and future priorities. The themes that emerged from these discussions were the structural difficulties faced by regional ethicists, the use of pedagogical innovations to mitigate different barriers faced by fellows, and the need to anticipate future challenges for the region including climate change.

Mothers’ characteristics influence children’s health and development. We conducted a cross-sectional study to assess the influence of mothers’ education level, employment status, and exposure to domestic violence on diarrhea among children under 5 years old using secondary data from 2010, 2015, and 2020 Rwanda Demographic Health Surveys. Hypomethylating agents (HMA) are indicated in the treatment of higher-risk myelodysplastic syndromes (MDS) and chronic myelomonocytic leukemia (CMML). The combination of hypomethylating agents with venetoclax (Ven) has demonstrated promising results in these diseases, although randomized clinical trials are needed for validation. In this retrospective study, we compared two matched cohorts of patients with MDS or CMML: one receiving oral decitabine-cedazuridine (DEC-C, n = 73) and one receiving DEC-C and Ven (DEC-C-Ven, n = 51), in three contemporary clinical trials. Neurofibromatosis type 1 (NF1) is an inherited disorder that predisposes individuals to malignant peripheral nerve sheath tumors (MPNSTs), a highly aggressive sarcoma with limited treatment options and poor prognosis. This study explores the potential of targeting the interaction between Galectin-1 and Ras as a novel therapeutic strategy for MPNSTs. Through molecular docking, we identified critical residues involved in the Galectin-1 and H-Ras interaction. We developed LLS30, a compound designed to target this Ras-binding pocket on Galectin-1, and tested its efficacy. LLS30 effectively disrupted the Galectin-1/Ras interaction, causing Ras delocalization from the plasma membrane and inhibiting Ras signaling. In vitro experiments …

BACKGROUND: In recent years, it has become increasingly common to incorporate the patient perspective into drug development and regulatory decision making.
OBJECTIVE: This study aimed to measure and quantify patient preferences (priorities and trade-offs) for attributes that characterize current and emerging refractory chronic spontaneous urticaria (rCSU) treatments.
METHODS: Adult patients with self-reported rCSU symptoms completed an online discrete choice experiment survey. The survey included 10 questions that asked respondents to choose between 2 hypothetical rCSU treatment profiles having similar attributes with varying levels. Spetzler-Martin Grade IV arteriovenous malformations (AVMs) are challenging due to high risks associated with both treatment and natural progression. This study compares the outcomes of microsurgical resection and stereotactic radiosurgery (SRS) in high-grade AVMs, analyzing obliteration rates, complications, and functional outcomes. A retrospective cohort of 96 patients treated with either microsurgical resection (33 patients) or SRS (63 patients) was analyzed. Propensity-score matching was employed to account for baseline variables such as AVM size (cm), preoperative embolization and rupture status. Primary endpoints included AVM obliteration, complication rates, and modified Rankin Scale (mRS) scores. After matching, 31 patients per group were analyzed. …

Hypomethylating agents (HMA) are indicated in the treatment of higher-risk myelodysplastic syndromes (MDS) and chronic myelomonocytic leukemia (CMML). The combination of hypomethylating agents with venetoclax (Ven) has demonstrated promising results in these diseases, although randomized clinical trials are needed for validation. In this retrospective study, we compared two matched cohorts of patients with MDS or CMML: one receiving oral decitabine-cedazuridine (DEC-C, n = 73) and one receiving DEC-C and Ven (DEC-C-Ven, n = 51), in three contemporary clinical trials. Dengue Hemorrhagic Fever (DHF) is an infectious disease that poses a public health problem in tropical regions and is influenced by climatic factors such as temperature, rainfall, humidity, and wind speed. In 2023, Malang Regency recorded 1,018 cases with an incidence rate (IR) of 38.2 per 100,000 population. This study aims to analyze the relationship between climatic factors and the DF IR for the 2022–2024 period using an ecological design and Spearman’s correlation analysis. Global hepatic DNA methylation change has been linked to human patients with metabolic dysfunction-associated steatotic liver disease (MASLD). DNA demethylation is regulated by the TET family proteins, whose enzymatic activities require 2-oxoglutarate (2-OG) and iron that both are elevated in human MASLD patients. We aimed to investigate liver TET1 in MASLD progression. Depleting TET1 using two different strategies substantially alleviated MASLD progression. Knockout (KO) of TET1 slightly improved diet induced obesity and glucose homeostasis. Intriguingly, hepatic cholesterols, triglycerides, and CD36 were significantly decreased upon TET1 depletion. Consistently, liver specific TET1 KO led to improvement of MASLD progression. Mechanistically, TET1 promoted …

Embryo implantation is a critical event in the establishment of pregnancy, and implantation failure is a major cause of pregnancy loss in women. Coordinated, cell-type specific responses to the ovarian steroid hormones, estrogen, and progesterone, within the endometrium underlie successful embryo implantation and pregnancy establishment. In this study, we utilized a glandular epithelium (GE) specific Cre recombinase mouse line that is only active in the adult (Prss29-Cre) to determine the biological role of progesterone receptor (PGR) in uterine glands during pregnancy. Introduction: Four years of managing a housing network for people with disabilities prompted a rethinking of the financial management of the services offered. Clients with different needs require diverse professional staff for their home care. Intestinal microbes can beneficially impact host physiology, prompting investigations into the therapeutic usage of such microbes in a range of diseases. For example, human intestinal microbe Limosilactobacillus reuteri strains ATCC PTA 6475 and DSM 17938 are being considered for use for intestinal ailments, including colic, infection, and inflammation, as well as for non-intestinal ailments, including osteoporosis, wound healing, and autism spectrum disorder. While many of their beneficial properties are attributed to suppressing inflammatory responses, we postulated that L. reuteri may also regulate intestinal hormones to affect physiology within and outside of the gut. The rise of transcatheter aortic valve replacement (TAVR) over the past two decades has substantially changed the lifetime management of patients with aortic valve disease. As the indications for TAVR expand to include younger and lower-risk patients, the proportion of patients who subsequently require reintervention for failed transcatheter heart valves (THVs) will increase. The two primary options for reintervention are redo TAVR and TAVR explant followed by surgical aortic valve replacement (SAVR). Breast cancer is the most prevalent cancer in women worldwide. Aberrant epigenetic reprogramming such as dysregulation of histone acetylation has been associated with the development of breast cancer. Histone acetylation modulators have been targeted as potential treatments for breast cancer. This review comprehensively discusses the roles of these modulators and the effects of their inhibitors on breast cancer. In addition, epigenetic reprogramming not only affects breast cancer cells but also the immunosuppressive myeloid cells, which can facilitate breast cancer progression. INTRODUCTION: Social determinants of health (SDoH) and health literacy have been demonstrated to significantly impact health outcomes. As part of a study of diabetic ketoacidosis (DKA) treatment from the emergency department (ED), we assessed the burden of SDoH and health literacy among patients with DKA to identify potentially modifiable risk factors in the development of DKA.

Background: Archery requires precision, strength, stamina, and attention, yet musculoskeletal injury data is scarce. The study aimed to profile the musculoskeletal injuries and the associated risk factors among Malaysian archers. Methods: A cross-sectional approach was used to recruit 165 elite archers. The Standardized Nordic Questionnaire (SNQ), adapted risk factors questionnaire, Pittsburgh Sleep Quality Index (PSQI), and Depression Anxiety Stress Questionnaire (DASS-21) were used to screen participants. Results: Shoulder (drawing arm) (54.5%), shoulder (bow arm) (56.4%), upper back (38.8%), lower back (38.2%), and neck (37.6%) musculoskeletal injuries were common. As future RNs preparing to enter the workforce in a few short months, we chose to explore the effects of nurse-to-patient ratios, specifically acuity-based staffing assignments. The Summit Medical Center’s 4th and 5th floor medical-surgical units currently utilize a fixed numerical assignment system, not considering the complexity of individual patients when assigning patients to nurses. This form of assignment-making poses potential problems with unequal workloads, contributes to an increase in nurse burnout, and potentially compromised care. Continuous renal replacement therapy (CRRT) is a complex, high-risk intervention used in intensive care units (ICUs) for critically ill patients with acute kidney injury and hemodynamic instability. ICU nurses are responsible for initiating, monitoring, troubleshooting, and discontinuing CRRT while providing continuous bedside care (Richardson & Whatmore, 2015). However, variability in training frequency and competency validation remains a significant gap, potentially contributing to errors and adverse patient outcomes.
