Background: There is a brisk, reflex autonomic response to low blood glucose in which surges of catecholamines are released to enhance glycogenolysis, gluconeogenesis, and lipolysis. In addition to providing metabolic substrates, the cardiovascular responses to this surge in norepinephrine and epinephrine include an increase in heart rate and cardiac contractility. How these changes in heart rate and contractility are subjectively sensed and perceived as …

Enhancing State-Of-The-Art Motor Imagery Classification With Reinforcement Learning, Anton Shepelev
Enhancing State-Of-The-Art Motor Imagery Classification With Reinforcement Learning, Anton Shepelev
USF Tampa Graduate Theses and Dissertations
One of the key obstacles to the rapid adoption of non-invasive Brain-Computer Interfaces (BCIs) for Motor Imagery (MI) is the low signal-to-noise ratio, and the substantial data requirements which can be mentally taxing for users. EEGNet, a compact Convolutional Neural Network (CNN), has long been considered the state-of-the-art (SOTA) for MI classification, demonstrating strong performance even with limited data. However, recent studies advocate for integrating Deep Reinforcement Learning (RL) to further enhance classification accuracy by dynamically optimizing feature extraction and decision-making processes. Despite this potential, practical implementations remain scarce due to challenges in stabilizing RL training and adapting it to …
A Long Non-Coding Erna Forms R-Loops To Shape Emotional Experience-Induced Behavioral Adaptations, Rose Marie Akiki
A Long Non-Coding Erna Forms R-Loops To Shape Emotional Experience-Induced Behavioral Adaptations, Rose Marie Akiki
MUSC Theses and Dissertations
Emotional experiences often evoke neural plasticity that supports adaptive changes in behavior, including maladaptive plasticity associated with mood and substance use disorders. These adaptations are supported in part by experience-dependent activation of immediate-early response genes, such as Npas4. We discovered that a conserved, long non-coding enhancer RNA (lnc-eRNA) transcribed from an activity-sensitive enhancer produces DNA-RNA hybrid R-loop structures that support 3D chromatin-looping of the enhancer and proximal promoter and stimulus-induced, rapid Npas4 gene induction. We also show that this Npas4 lnceRNA and its R-loops are required for the development of behavioral adaptations produced by chronic psychosocial stress or cocaine exposure, …
Comparison Of Treatments For Lumbar Disc Herniations & Long-Term Pain Relief, Becca Pollack
Comparison Of Treatments For Lumbar Disc Herniations & Long-Term Pain Relief, Becca Pollack
D.U.Quark
A lumbar disc herniation (LDH) occurs when the intervertebral disc pushes out from the disc space and irritates the surrounding nerves. LDHs are typically diagnosed via MRI scan, and the most common symptoms are low back pain and shooting leg pain, along with leg weakness/numbness. Although many different treatments are available to treat LDHs, few papers have compared the efficacy of treatment beyond 12 months. This review evaluates the differences between popular LDH treatments, like epidural steroid injections and microdiscectomies, at reducing leg and back pain, as well as comparing newer treatments’ efficacy at treating pain. Research comparing treatments should …

Faculty Scholarship
Purpose of Review: Advances in HIV testing, prevention, and treatment, alongside increased awareness and harm reduction efforts for substance use disorder (SUD) have improved care and treatment access over the past decade. However, racial and ethnic minorities with SUD and HIV or at risk for HIV still face disproportionately high health disparities. Understanding and addressing the reasons behind these disparities is crucial. Recent Findings: Structural and systemic barriers continue to negatively impact minoritized communities, due to lack of access to care, mistrust, and feelings of ostracization. Disconnected systems for HIV and SUD treatment complicate combined care. Delays in HIV diagnosis …

Miller, Seethal A. Jacob, Sana Saif-Ur-Rehman, Andrew O'Brien, Sophie Lanzkron
Cardeza Foundation for Hematologic Research
Introduction SCD remains the most common inherited blood disorder. Due to a lack of strong evidence, the management of sickle cell is often anecdotal, local to individual centers, states, and countries. Even in areas of practice with high-level data to improve practice, implementation has not been consistent. The historical lack of an agreed-upon national registry has compounded this problem and resulted in a lack of comparative effectiveness data to identify areas of deficiency and improve practice. These barriers have resulted in centers developing local policies and procedures, often with limited communication with other centers, resulting in disparate and inconsistent care. …

Walden Dissertations and Doctoral Studies
In this staff education project, I addressed the critical need to improve hand hygiene compliance among healthcare workers in behavioral health settings. Poor adherence to hand hygiene protocols is directly linked to increased healthcare-associated infections (HAIs), patient morbidity, and higher healthcare costs in behavioral health environments. The practice-focused question was: How does a hand hygiene intervention program, grounded in the theoretical domains framework, impact hand hygiene compliance among behavioral healthcare workers in inpatient psychiatric units? The purpose of the project was to evaluate the effect of a hand hygiene intervention program on compliance rates among behavioral healthcare workers in inpatient …
Social Support And Treatment Outcomes Among Multi-Drug Resistant Tuberculosis And Persons Living With Hiv In Nigeria, Abiodun Olusegun Hassan
Social Support And Treatment Outcomes Among Multi-Drug Resistant Tuberculosis And Persons Living With Hiv In Nigeria, Abiodun Olusegun Hassan
Walden Dissertations and Doctoral Studies
An estimated 40% of people with tuberculosis (TB) are missed due to underdiagnosis, and among those previously treated TB cases, about a quarter develop drug-resistant TB in Nigeria. The emergence of multi-drug-resistant TB (MDR-TB) is stalling the effort towards TB’s control and eradication. Few studies have examined the impact of the psychosocial effect and support systems for TB, particularly for MDR-TB. In this quantitative, cross-sectional study, the predictive relationship between social support given to MDR-TB patients and their treatment outcomes was examined using secondary data of 594 MDR-TB cases enrolled between January 2018 and December 2021. The theoretical framework for …

Background: Sitravatinib, an oral multi-kinase inhibitor targeting VEGFR, TAM, and MET, has been shown to resensitize the tumor microenvironment to immune checkpoint inhibitors (ICI) by reducing immune-suppressive myeloid cells in metastatic clear cell RCC (ccRCC). ICI is the standard first-line (1L) treatment of metastatic ccRCC, and there is unmet need for improved treatment outcomes after progression on ICI. We hypothesized that sitravatinib plus nivolumab would revert an immunosuppressive tumor microenvironment (TME) to improve clinical outcomes.
Methods: In this investigator-initiated, phase II, multicenter trial (NCT04904302), patients with progressive metastatic ccRCC after 1-2 lines of treatment were enrolled into 3 …

IMPORTANCE: The US Centers for Medicare & Medicaid Services launched the Accountable Care Organization (ACO) Realizing Equity, Access, and Community Health (REACH) payment model in January 2023. In contrast to prior ACO initiatives, such as the Medicare Shared Savings Program (MSSP), ACO REACH includes equity-focused measures and payment adjustments, including an equity plan and financial risk adjustment for ACOs with higher proportions of underserved beneficiaries. However, it is unknown whether these changes have incented participation from organizations that serve beneficiaries from marginalized communities.
OBJECTIVE: To compare characteristics between participants in ACO REACH with those in MSSP and the broader pool …

The MECOM (MDS1 and EVI1 complex locus) gene, located at 3q26.2, encodes an oncogenic transcription factor implicated in multiple signaling pathways. Rearrangements involving MECOM/3q26.2, including inversions, translocations, insertions and cryptic chromosomal changes, are observed in myeloid neoplasms and are associated with high-risk disease features and poor clinical outcomes. The translocation t(3;12)(q26.2;p13.1) is a rare genetic event, resulting in a fusion of the MECOM gene at 3q26.2 with the ETV6 gene at 12p13.1. To date, only 78 cases of hematologic neoplasms harboring t(3;12) have been reported in the English literature, primarily as case reports or case series. T(3;12) has …

Accurately genotyping structural variant (SV) alleles is crucial to genomics research. We present a novel method (kanpig) for genotyping SVs that leverages variant graphs and k-mer vectors to rapidly generate accurate SV genotypes. Benchmarking against the latest SV datasets shows kanpig achieves a single-sample genotyping concordance of 82.1%, significantly outperforming existing tools, which average 66.3%. We explore kanpig's use for multi-sample projects by testing on 47 genetically diverse samples and find kanpig accurately genotypes complex loci (e.g. SVs neighboring other SVs), and produces higher genotyping concordance than other tools. Kanpig requires only 43 seconds to process a single sample's 20x …

Fluorescent proteins are indispensable molecular tools for visualizing biological structures and processes, but their limited photostability restricts the duration of dynamic imaging experiments. Yellow fluorescent proteins (YFPs), in particular, photobleach rapidly. Here, we introduce mGold2s and mGold2t, YFPs with up to 25-fold greater photostability than mVenus and mCitrine, two commonly used YFPs, while maintaining comparable brightness. These variants were identified using a high-throughput pooled single-cell platform, simultaneously screening for high brightness and photostability. Compared with our previous benchmark, mGold, the mGold2 variants display a ~4-fold increase in photostability without sacrificing brightness. mGold2s and mGold2t extend imaging durations across diverse modalities, …

Histone proteoforms, often presenting multiple co-occurring post-translational modifications (PTMs), are central to chromatin regulation and gene expression. A proteoform is a specific form of a protein that includes variations arising from genetic changes, alternative RNA splicing, proteolytic processing, and PTMs. Genome-indexed histone proteoforms define the histone code, influencing cellular phenotype by dictating DNA interacting partners. Understanding the dynamics of histone proteoforms is essential for elucidating chromatin-based regulatory mechanisms. Advances in middle-down and top-down proteomics enable accurate identification and quantitation of thousands of proteoforms in a single run. However, the resulting data complexity presents significant challenges for analysis and visualization. Here, …

BACKGROUND: Same-day testing and treatment of curable sexually transmitted infections (STI) is a strategy to reduce infection duration and onward transmission. South African primary healthcare facilities often lack sufficient waiting spaces. This study aimed to assess the proportion of, and factors influencing, pregnant women waiting for on-site STI test results before and after the installation of clinic-based waiting rooms. METHODS: We conducted an observational quality improvement study at 5 public primary healthcare facilities in South Africa from March 2021 to May 2023. The intervention was the installation of a waiting room in two clinics. Three clinics were used as comparators: …

Celebration of Research
Batten disease is a rare, pediatric, neurological disorder that is the result of a mutation in one of 13 CLN genes. Batten disease has an incidence rate of 1 out of 100,000 births worldwide with CLN3 being the most common form. Being a genetic disorder, gene therapy is an ideal treatment approach for Batten disease. Diagnosis of disease, however, typically does not occur until disease has progressed, when the ideal therapeutic window for gene therapy has already passed. Additionally, the durability of viral-delivered gene therapy over time is poorly understood. AF38469, a small molecule inhibitor of sortilin, was developed for …
