Ectopic parathyroid tumors are uncommon, accounting for only 6% of parathyroid adenomas, and even fewer cases are attributed to parathyroid carcinomas. While ectopic parathyroid carcinoma in the anterior mediastinum is a rare condition, the occurrence of recurrent skeletal fractures in the presence of mild hypercalcemia is even rarer. In this report, we present the case of a 52-year-old man with a medical history of hypertension, chronic kidney disease, and a previous right-sided intertrochanteric fracture of the femur one year earlier. Introduction: The newly emergent COVID-19 virus reached pandemic levels in March 2020. By the middle of August 2020, there were over 1 million deaths attributed to COVID-19 in the U.S., with those in rural areas outpacing urban counterparts. Prior to emergency approval of the Pfizer, Moderna and Johnson & Johnson vaccine formulations, mitigation efforts addressing individual behavior were challenging. However, even with the entrance of these three new vaccines, herd immunity was not achieved in rural areas, as vaccine uptake remained low there. Introduction: In the opioid epidemic, the U.S. faces a significant public health crisis, with some areas of the country, such as rural and Appalachian regions, suffering more than others. The differential regional impact of the crisis in Kentucky—a state with both non-metropolitan/metropolitan and Appalachian/Non-Appalachian statuses—has not yet been documented despite such knowledge being essential to the success of overdose prevention efforts.
Purpose: This study compares all-cause, drug- and opioid-related mortality between counties in different regions of Kentucky: Appalachian non-metropolitan, Appalachian metropolitan, non-Appalachian non-metropolitan, and non-Appalachian metropolitan.

Introduction: Carbapenem-resistant Enterobacterales (CRE) are considered urgent, antibiotic-resistant threats in the U.S. and are of global concern. Active collaboration between public health authorities and healthcare facilities and providers will be necessary to prevent and contain these organisms.
Purpose: To describe the epidemiology of CRE in Kentucky and to discuss challenges and successes with building and sustaining an effective prevention and containment program.
Methods: Retrospective descriptive summary of CRE isolates reported by healthcare providers, facilities, and laboratories in Kentucky from 2013 through 2020. Data available from case reporting forms and laboratory testing are summarized.

Introduction: Success of opioid overdose interventions involving first responders is dependent on the comfort level that bystanders have with first responders and their willingness to call for assistance. Positive or negative experiences with first responders following witnessing an overdose may influence a person’s willingness to call a first responder for assistance in the future.

Objective: Tramadol and 3,4-methylenedioxy-methamphetamine (MDMA) use over an extended period is linked to deficits in memory encoding and retrieval. We aimed to determine the neurotoxic effects of co-administration of MDMA and tramadol (TRAM) on hippocampal function and investigate the potential of FMRFamide in attenuating resulting alterations in the Wistar rat model. Methods: Thirty adult male Wistar rats were grouped into six (n=5): Control, FMRFamide, TRAM, MDMA, TRAM+MDMA, and TRAM + MDMA + FMRFamide groups. Somatostatin is a paracrine modulator of insulin secretion and beta cell function with pleotropic effects on glucose homeostasis. The mechanism of somatostatin-mediated communication between delta and beta cells is not well-understood, which we address in this study via the ciliary somatostatin receptor 3 (SSTR3). Primary cilia are membrane organelles that act as signaling hubs in islets by virtue of their subcellular location and enrichment in signaling proteins such as G-protein coupled receptors (GPCRs). Down syndrome (DS) is the most common chromosomal disorder diagnosed in the United States since 2014. There is a wide range of intellectual severities, with the average IQ of individuals with DS at approximately 50 and adults without intellectual delay at approximately 70-130. Individuals with DS vary from mild to severe cognitive impairment, depending on the phenotypic penetration on the 21st chromosome, with the average cognitive capacity equivalent to a cognitive functioning of an 8- to 9-year-old child. However, there is limited data available on fungal empyema from developing countries regarding risk factors, treatment, and outcome. Background: Obesity and overweight pose significant challenges in developing countries, underscoring the need for evidence-based prevention strategies. This study focuses on assessing the prevalence and associated factors of overweight and obesity in the rural areas of Jamnagar district, Gujarat, India.
Methods: Data collection encompassed six tehsils within Jamnagar district, with primary health center selection via simple random sampling. Individual selection employed the consecutive sampling approach. Statistical analysis was conducted using SPSS software. Background: This study aimed to examine and compare the age, body mass index (BMI), physical activity (PA), and quality of life (QOL) of third-trimester Turkish pregnant women with and without a diagnosis of gestational diabetes mellitus (GDM).
Methods: A comparative study was conducted in the obstetrics clinic of a university hospital in Turkey, and it included 210 women (GDM women = 105 and non-GDM women = 105). Personal Information Form, International Physical Activity Questionnaire-Short Form (IPAQ), and World Health Organization Quality of Life-Short Form-Turkish (WHOQOL-Bref-TR) were used.

Urban farming can enhance the lives of communities and help reduce food scarcity. This paper presents a conceptual prototype of an efficient urban farming community that can be scaled for a single apartment building or an entire community across all global geoeconomics regions, including densely populated cities and rural, developing towns and communities. When deployed in coordination with smart crop choices, local farm support, and efficient transportation then the result isn’t just sustainability, but also increasing fresh produce accessibility, optimizing nutritional value, eliminating the use of ‘forever chemicals’, reducing transportation costs, and fostering global environmental benefits.

BACKGROUND: Locally advanced non-small cell lung cancer (LA-NSCLC) treated with the programmed death-ligand 1 inhibitor durvalumab has been associated with significant rates of pneumonitis, which has led to higher rates of discontinuation of therapy in real-world populations. Thus far there has been no consensus in the literature on the impact of pneumonitis on survival.
METHODS: This is a retrospective cohort study of veterans receiving durvalumab between 12/5/2017 and 4/15/2020. Participants were identified using VINCI data services. Osteoporosis represents a significant global public health issue, particularly among the aging population. Its incidence reaches 18.3% of the total population, with the highest prevalence observed in elderly postmenopausal women. A key factor in osteoporosis is the decreased expression level of estrogen receptor alpha (ERα), attributed to its degradation by the ubiquitin ligase protein complex Cullin4B (CUL4B), DNA damage binding 1 (DDB1), and aryl hydrocarbon receptor (AhR), collectively known as CUL4BAhR. Acanthus ilicifolius L contains compounds exhibiting antiosteoporosis activity, primarily by inhibiting osteoclastogenesis via RANKL. Rosella, also known as Hibiscus sabdariffa L., is a plant that contains anthocyanin compounds in its calyx, which are known for their antioxidant properties. These compounds give the plant a captivating red color and can be used as natural dyes. This study aimed to exploit anthocyanins in rosella calyx as natural dyes and antioxidants in lip cream product. The formulations employed extracts from rosella calyx, a viscous extract from maceration in 96% ethanol (F1) and a commercially available powdered rosella extract (F2). This study was conducted to evaluate the subchronic toxicity of the leaf extract of ki hampelas. Dragon fruit, known as Hylocereus undantus (HU), has traditionally been utilized in Banyuwangi, East Java, Indonesia, for addressing gout and rheumatism. Despite its empirical use, there is a lack of reported pharmacological research on this plant. This study aimed to explore the properties of HU extract in treating hyperuricemia, inflammation, and pain. Adenine-potassium oxonate-induced mice were employed to evaluate anti-hyperuricemia activity, measuring uric acid, blood urea nitrogen (BUN), and creatinine serum levels. AIMS: Mutations in the DSP gene encoding desmoplakin, a constituent of the desmosomes at the intercalated discs (IDs), cause a phenotype that spans arrhythmogenic cardiomyopathy (ACM) and dilated cardiomyopathy. It is typically characterized by biventricular enlargement and dysfunction, myocardial fibrosis, cell death, and arrhythmias. The canonical wingless-related integration (cWNT)/β-catenin pathway is implicated in the pathogenesis of ACM. The β-catenin is an indispensable co-transcriptional regulator of the cWNT pathway and a member of the IDs. We genetically inactivated or activated β-catenin to determine its role in the pathogenesis of desmoplakin cardiomyopathy.

Background: Lichen amyloidosis (LA) is characterized by linear rows of firm, pigmented, grouped, hyperkeratotic papules that can form a plaque, usually occurring on the shins and forearms, with intense itch as the prominent symptom.
Case Illustration: A 57-year-old female complained of brown spots on her shins and arms that gradually thickened six years ago. The lesions were brown, multiple, discrete, slightly scaly papules forming hyperpigmented plaques. The result of the histopathology examination showed an acanthotic epidermis with hyperkeratotic foci and eosinophilic amorphous mass deposits in the papillary dermis with brown pigments. Background: Lengthy therapeutic regimens in leprosy are prone to defaults, contributing to more antibiotic-resistant Mycobacterium. Intervening patients’ medical habits midway would be more complicated than understanding their perception of undergoing such regimens in advance. We study the factors considered relevant to leprosy patients’ compliance to construct a novel self-reporting questionnaire that can predict the patients’ adherence before initiating multidrug therapy.
