BK virus hemorrhagic cystitis (BKV-HC) is a complication after allogeneic hematopoietic stem cell transplant (AlloHCT) for which optimal management is uncertain. Intravenous (IV) and intravesicular (IVES) cidofovir have been used with varying degrees of success in small case series of six to 33 patients. While some series have investigated side effects, none have examined medication errors. A retrospective single-center case series of AlloHCT patients with BKV-HC given IV or IVES cidofovir (CDV) from 2018 to 2022 at an urban cancer center in Detroit, MI. Introduction

Injury by firearms among the pediatric population is a growing concern in the United States. This paper investigates demographic factors associated with pediatric cranial gunshot wounds (GSW).

Methods

A query of the KID databases from 1997-2019 was run for cranial GSW and craniotomy ICD-9 and ICD-10 codes. ANOVA and chi-squared tests were performed to analyze demographics and outcomes by injury type.

Results

Our query resulted in 2990 cranial GSW patients; 86.1% were male. Mean age was 16.48 years; accidental victims were the youngest and intentional the oldest (14.88±5.2, 17.2±2.3, p< 0.001). Blacks, Hispanics, and Asian-Pacific-Islanders (minorities) were most commonly victims of assault; Whites were most commonly victims of intentional GSW (p< 0.001). Victims mostly had Medicaid except for intentional victims who were more likely to have private insurance (p< 0.001). Assault was the predominant injury type in all regions, especially in the West (33.7%). The South had the most accidental (51.9%) and intentional (44.6%) injuries. In 1997 and 2019 the predominant GSW type was accidental; in all other years assault predominated. Accidental victims were most likely to undergo craniotomy and intentional were least likely (p< 0.001). Inpatient mortality was 46.4% with intentional victims having the highest rate (68.6%) then undetermined (60.3%) and law enforcement-related (57.7%).

Introduction: Urethral stricture treatment has high recurrence rates and adjunct injectable agents have been explored. Amniotic membranes (AM) promote apoptosis of pro-inflammatory cells, prevent differentiation of pro-fibrotic cells, and decrease scar formation. These tissues generated interest in reconstructive urethral surgery. Thus, we performed urethral dilation combined with micronized AM injection in urethral scar tissue for treatment of urethral stricture.

Materials and Methods: Adult males with strictures ≤12Fr in diameter and ≤2 cm in length, International Prostate Symptom Score (IPSS) ≥11 and maximum flowrate <15 ml/s. Reconstituted 100mg micronized AM was injected at the time of urethral dilation. Primary study end point was anatomical success (≥14Fr by cystoscopy) at 6 months. Secondary end points were questionnaires, flow rate, and post void residual. Outcomes assessed at baseline, 5 days, 14 days, 3 months, and 6 months post-injection. Safety was analyzed.

Since the COVID-19 outbreak in 2020, Great Lakes drownings have increased. The Great Lakes are a unique and understudied environment due to being cold, fresh water, and resuscitation protocols and guidelines being derived from warm, primarily salt water environments. We analyzed open sources materials to find records of Great Lakes drownings from 2021-2023 and analyze the data looking for trends in cold and warm water drownings.

Tendon Rupture and Tendonitis in Low-Profile Dorsal versus Volar Plating for Distal Radius Fractures: A Systematic Review and Meta-Analysis

Background: Dorsal plating of distal radius fractures has historically been associated with high rates of hardware removal, tendonitis, and tendon rupture. Newer generation low-profile dorsal plates are thinner (<1.6mm thick) with improved characteristics. We examine whether low-profile dorsal plates still have higher rates of tendon complications than volar locking plates.

Methods: We searched Ovid MEDLINE, Web of Science, and EMBASE for published literature describing tendon complications in association with plating of distal radius fractures. Intraosseous (IO) vascular access is most commonly used when critical patients need rapid establishment of vascular access. They have shown high rates of successful placement, with the proximal tibia showing the highest first-attempt success rates. Proper establishment of vascular access requires a needle properly sized to enter the bony cortex and stay there. This systematic review investigates the impact of social determinants of health (SDOH) on outcomes in shoulder arthroplasty (SA). SA, a common orthopedic procedure, has seen an increasing focus on SDOH influencing postoperative results. The study aims to comprehensively assess this impact on both surgical and patient-reported outcomes.

Through a systematic search of databases, 34 studies (2011-2021) met the inclusion criteria, involving 4,825,547 patients. Explored SDOH included insurance status, race/ethnicity, age, sex, income, social support, and veteran status. Outcomes considered were length of stay, American Shoulder and Elbow Surgeons scores, total SA rates, revision rates, complication rates, and discharge status.

External beam radiation therapy, a cornerstone in cancer treatment for 50% of patients, poses challenges due to its toxicity to both cancer and healthy cells. Seeking to mitigate side effects, the emerging FLASH radiation technology employs ultra-high dose rates, promising quicker treatments with reduced impact on surrounding normal tissue. Platinum, recognized as a radiation sensitizer, enhances the effectiveness of radiation therapy by increasing sensitization at higher energy levels through pair-production.

Hydrocephalus causes neurological effects and high intracranial pressure (ICP) in the pediatric condition. Studies observing the relationship between ICP and cerebrospinal fluid (CSF) production in an electrical resistance model have been performed, but less has been done to define this relationship in living organisms. This study explored CSF flow dynamics as a consequence of hydrocephalus severity utilizing a novel model.

Introduction and Objectives: Female sexual dysfunction (FSD) following spinal cord injury (SCI) presents with abnormalities in sexual desire, arousal, orgasm, pain, distress, and dissatisfaction. FSD negatively impacts quality of life and is often refractory to available treatment options. Previously, we reported preliminary benefits of intra-detrusor micronized amniotic membrane (AM) on detrusor dysfunction following SCI. Introduction: In view of the pathophysiology of chronic radiation cystitis (CRC) that is mainly caused by activation of an inflammatory cascade leading to chronic inflammation, vascular damage and fibrosis, amniotic membrane (AM) based therapy has emerged as a potential therapeutic approach for radiation-induced tissue injury due to its immunomodulatory, vasculogenic and anti-fibrotic properties. Background: Workplace violence (WPV) is defined as physical violence or verbal threats of physical violence, regardless of whether an employee sustains an injury. Emergency department (ED) staff are at high risk of exposure to WPV, which can lead to burnout, job dissatisfaction, and secondary stress. Effective preparation for and response to WPV incidents can help mitigate these negative effects. Since the COVID-19 pandemic, national vaccination rates show a dangerous decline with Detroit being amongst the lowest with only 41.3% of adolescents completing the vaccine series. The Vaccine Ambassador program was created to mitigate the decline in vaccination rates through the education and empowerment of youth via train-the-trainer model of education.

Background: Active surveillance (AS) allows patients with low-risk prostate cancer (LRPC) to be monitored for disease progression, thereby avoiding or delaying invasive curative treatment (CT) (i.e., surgery or radiation) and related side effects. Here we compare quality of life (QOL) two-years after diagnosis between men who underwent CT overall and by race.

Methods: Black and white men with LRPC were recruited through population-based cancer registries in metropolitan Detroit and Georgia. Participants were grouped by their treatment type (CT or AS). Background: Glioblastoma Multiforme (GBM) is an aggressive and malignant brain tumor known for its rapid progression and unfavorable prognosis. The standard treatment options primarily involve radiation and temozolomide, however recurrence is inevitable.

B7H3/CD276, an immunomodulatory transmembrane glycoprotein, demonstrates tumor-specific expression, with limited presence in normal healthy tissue, making it a promising and attractive treatment target in GBM. AbbV155 is an antibody drug conjugate that inhibits BCL-XL and specifically targets B7H3 (CD276) on GBM cells with the goal of selectively eliminating viable tumor cells.

Osteocytes serve as a sensory network that responds to dynamic loading of the skeleton and play a crucial role in regulating bone mass while maintaining bone quality [1]. Osteocytes’ induction of bone formation in response to loading is extremely age dependent, such that clinical studies have shown exercise to be less and less effective as we age [2,3]. As a result, modifying osteocytes’ mechanosensitivity may enable older adults to better capitalize on the anabolic nature of exercise to increase bone mass and reduce fracture risk. Introduction:

Insula and amygdala have a large network of connections to other cortical regions. Seizures originating from these structures may become evident only after propagation to other structures. High degree of attention must be paid to auras and seizure semiology in patients with medically intractable epilepsy.

Methodology:

We have described a series of three adult male patients with focal intractable epilepsy who did not have structural lesions on Magnetic Resonance Imaging (MRI). Patients underwent MRI, Positron Emission Tomography (PET) scan, scalp electroencephalography (EEG), and stereo EEG implantation.

Increased knowledge of medical conditions can positively modify health behaviors, such as more participation in cancer screening and better adherence to medications. However, the readability of educational materials often does not match the patients’ reading abilities. Mindfulness based interventions (MBI) reduce the severity of anxiety and depression among a variety of populations. While the psychological benefits of MBIs are increasingly recognized, less is known about the underlying neural correlates—particularly during childhood and adolescence, a time when many psychopathologies begin to emerge. Here, we provide the first systematic review on the neural correlates of MBI in youth. We reviewed the PubMed database for original articles examining the neural correlates of MBI in youth (0-18 years) leveraging neuroimaging. Background: Individuals with sickle cell disease are restricted from certain physical activities due to the increased risk of complications including fractures secondary to osteopenia. However the exact incidence and outcomes of fractures amongst these patients is unknown.

Vaccine hesitancy among youth with human immunodeficiency virus (HIV) was evaluated to determine if the pandemic affected vaccine acceptance. A retrospective chart review was conducted to analyze vaccine rates over a five-year (pre-post COVID) in one university-based clinic. Results indicated a variation in acceptance rates between vaccines, with the MenACWY, MenB, tetanus, and HPV having the highest acceptance rates and COVID-19 and annual flu vaccine having the lowest acceptance rates. Vaccine hesitancy did not correlate with HIV treatment hesitancy nor with aversion to needles. Negative messaging around vaccines from a variety of sources appeared to be the primary factor in …
