When pediatric rheumatology patients are discharged from a hospital, they are given a complex discharge plan which involves responsibilities in self-managing their care at home. This creates an overwhelming time for patients and families who may have questions or concerns during this time. In order to address this care gap and provide continuity of care, this quality improvement project implemented a nurse-led telehealth discharge visit intervention to 15 pediatric rheumatology patients during the transition from hospital discharge to outpatient care. With limited treatment options, cachexia remains a major challenge for patients with cancer. Characterizing the interplay between tumor cells and the immune microenvironment may help identify potential therapeutic targets for cancer cachexia. Herein, we investigate the critical role of macrophages in potentiating pancreatic cancer induced muscle wasting via promoting TWEAK (TNF-like weak inducer of apoptosis) secretion from the tumor. Specifically, depletion of macrophages reverses muscle degradation induced by tumor cells. Macrophages induce non-autonomous secretion of TWEAK through CCL5/TRAF6/NF-κB pathway. TWEAK promotes muscle atrophy by activating MuRF1 initiated muscle remodeling. Notably, tumor cells recruit and reprogram macrophages via the CCL2/CCR2 axis …

At present, there is no internationally accepted set of core outcomes or measurement methods for epilepsy clinical practice. Therefore, the International Consortium for Health Outcomes Measurement (ICHOM) convened an international working group of experts in epilepsy, people with epilepsy and their representatives to develop minimum sets of standardized outcomes and outcomes measurement methods for clinical practice that support patient–clinician decision-making and quality improvement. Consensus methods identified 20 core outcomes. Measurement tools were recommended based on their evidence of strong clinical measurement properties, feasibility, and cross-cultural applicability. Background: This survey described the compensation of neonatal intensive care unit (NICU) registered dietitian nutritionists (RDNs) in the United States and examined correlates of higher salaries within this group. Methods: A cross-sectional online survey was completed in 2021 by 143 NICU RDNs from 127 US hospitals who reported hourly wage in US dollars (USD). We used initial bivariate analyses to assess the relationship of selected institution-level and individual-level variables to hourly wage; the rank-sum test for binary variables; bivariate regression and Pearson correlation coefficients for continuous variables; the Kruskal–Wallis test for categorical variables. Buprenorphine is a semi-synthetic long-acting partial µ-opioid receptor (MOR) agonist that can be used for chronic pain as a sublingual tablet, transdermal patch (Butrans®), or a buccal film (Belbuca®). Buprenorphine’s unique high receptor binding affinity and slow dissociation at the MOR allow for effective analgesia while offering less adverse effects compared to a full agonist opioid, in particular, less concern for respiratory depression and constipation. It is underused in chronic pain and palliative care due to misconceptions and stigma from its use in opioid use disorder (OUD). Pulmonary tuberculosis (PTB) elimination efforts must consider the global growth of the ageing population. Here we used TB surveillance data from Texas, United States (2008–2020; total n = 10656) to identify unique characteristics and outcomes in older adults (OA, ≥65 years) with PTB, compared to young adults (YA, 18–39 years) or middle-aged adults (40–64 years). We found that the proportion of OA with PTB increased from 15% in 2008 to 24% in 2020 (trend p < 0.05). Diabetes was highly prevalent in OA (32%) but not associated with adverse outcomes. Death was 13-fold higher in OA compared to YA and was 7% at the time of diagnosis which suggests diagnostic delays. However, once TB was suspected, we found no differences in culture, smear, or nucleic acid detection of mycobacteria (although less lung cavitations) in OA. During treatment, OA had less drug-resistant TB, few adverse reactions and adhered with TB treatment. We recommend training healthcare workers to ‘think TB’ in OA, for prompt treatment initiation to diminish deaths. Furthermore, OA should be added as a priority group to the latent TB treatment guidelines by the World Health Organization, to prevent TB disease in this highly vulnerable group.

Acetaminophen (APAP) overdosing is a major cause of acute liver failure worldwide and an established model for drug-induced acute liver injury (ALI). While studying gene expression during murine APAP-induced ALI by 3'mRNA sequencing (massive analysis of cDNA ends, MACE), we observed splenic mRNA accumulation encoding for the neutrophil serine proteases cathepsin G, neutrophil elastase, and proteinase-3 - all are hierarchically activated by cathepsin C (CtsC). This, along with increased serum levels of these proteases in diseased mice, concurs with the established phenomenon of myeloid cell mobilization during APAP intoxication.

Purpose: The purpose of this study was to assess the oral health education and dental care access of families of children with Down syndrome in New Mexico.
Methods: The data were gathered from a survey available to members of the Rio Grande Down Syndrome Network for 14 days containing 13 questions.
Results: The most reported age range for the individuals in care were 0-14 years at 41.6%. Most individuals with Down syndrome receive dental services every six months (62.5%) at a regular dental office (54.2%). Caregivers reported the individuals are taking 1-2 prescription medications (54.2%) and not aware of …

Alcohol use disorder (AUD) is a complex and widespread disease with limited pharmacotherapies. Preclinical animal models of AUD use a variety of voluntary alcohol consumption procedures to recapitulate different phases of AUD, including binge alcohol consumption and dependence. However, voluntary alcohol consumption in mice is widely variable, making it difficult to reproduce results across labs. In the central nervous system, astrocytes are the primary source of glutathione (GSH), an important and abundant non-protein thiol antioxidant. GSH biosynthesis takes place in the cytosol and involves two sequential steps, the first catalyzed by glutamate cysteine ligase (GCL) and the subsequent by glutathione synthetase (GSS). The process relies heavily on the availability of cysteine, which is obtained through the action of the glutamate/cystine antiporter, System xc-. This transporter imports cystine into the cells, where it is rapidly reduced to cysteine intracellularly for GSH synthesis. Health disparities, “preventable differences in the burden of disease, injury, violence, or opportunities to achieve optimal health,” are infamous for their effects within both general healthcare and occupational therapy settings (Centers for Disease Control and Prevention [CDC], 2008). Though existing literature establishes a need for the widespread adoption of anti-discrimination measures, specific recommendations for occupational therapy schools remain limited. Small-animal models and reverse genetics systems are powerful tools for investigating the molecular mechanisms underlying viral replication, virulence, and interaction with the host immune response in vivo. Rotavirus (RV) causes acute gastroenteritis in many young animals and infants worldwide. Murine RV replicates efficiently in the intestines of inoculated suckling pups, causing diarrhea, and spreads efficiently to uninoculated littermates. Because RVs derived from human and other non-mouse animal species do not replicate efficiently in mice, murine RVs are uniquely useful in probing the viral and host determinants of efficient replication and pathogenesis in a species-matched mouse model. PURPOSE: Virtual reality (VR) has an increasing breadth of utilization, from at home entertainment to a tool used in professional athletic and clinical settings. A review of literature shows altered visual performance (VP) post-VR gameplay in children and adult athletes, particularly in acuity, accommodation and convergence, but has been limited to the immediate effects on vision. The sample consisted of 42 college students from Bethel University aged 18+ or who had an approved parent consent form. Beta-lactone synthetases are a class of enzymes which produce chiral beta-lactones, four-membered cyclic esters, from racemic beta-hydroxy acid substrates. Chiral beta-lactones are used as chemical synthons and as bioactive compounds, such as the FDA-approved drug, Orlistat. However, substrate specificity of stand-alone beta-lactone synthetases are poorly understood, and their use in chemoenzymatic synthesis of chiral bioactive compounds remains unexplored. This research focused on the organic synthesis of beta-hydroxy acids to probe the substrate specificity of this class of enzymes and to generate potentially bioactive compounds against essential tuberculosis enzymes. This study aimed to investigate the relationship between ankle mobility and balance in a young, healthy population, filling a gap in existing literature primarily focused on older adults. The findings revealed no significant correlation between ankle mobility and balance measures in the college-aged subjects. While previous research in elderly populations suggested a link between ankle mobility and balance, this study suggests that such a relationship may not hold true for younger individuals. During my Nursing Assistant internship, I transitioned from classroom learning to real-world caregiving. Guided by an instructor, I further developed practical skills. At Immaculate Homes Care, I faced challenging and new situations, and as a provider, I administered medications and interacted with residents regularly. This experience reinforced the importance of empathy, documentation, and patient care. It also prepared me for my career and boosted my confidence in making decisions.
