Background: Excessive posterior tibial slope (PTS) has been associated with a higher risk of graft failure after anterior cruciate ligament reconstruction (ACLR). Although anterior closing wedge osteotomy (ACWO) can reduce the PTS, it may also change the coronal alignment and patellar height. Purpose: To elucidate the radiological outcomes after infratubercle ACWO, specifically to evaluate its influence on perioperative changes in patellar height.
Methods: Patients who underwent infratubercle ACWO with combined ACLR with a minimum follow-up of 3 months were included. Surgery was indicated when the PTS was greater than 12°. Radiological evaluation included measurements of the hip-knee-ankle angle (HKA), PTS, …

This doctoral capstone project was in partnership with Seattle’s community boating center, Sail Sand Point (SSP). SSP’s mission is “to bring the joy and life-enhancing benefits of sailing and small boats to people of all ages, abilities, and backgrounds”. This project aimed to develop adaptive sailing programming to increase sailing access to individuals with disabilities. This included expanding the partnership with Outdoors For All, an adaptive outdoor recreation organization in Seattle. Through this partnership we hosted their summer camp for five sailing days with five additional days planned for after the completion of this project, demonstrating program sustainability and longevity. …

Introduction
Physical, sensory, and social aspects of the classroom environment directly influence students’ behavior, learning and academic achievement. Both teachers and occupational therapy practitioners (OTPs) approach the design, adaptation, and use of the classroom from their respective knowledge base (Wintle, et al, 2022). Grandisson et al. (2020) noted that OTPs can often know very little about the school personnel, their pedagogy, the school culture, or the classroom routines. This can lead to a disconnect between OTPs recommendations for classroom environment modifications and teachers’ design and use of the same space.

Dysregulation of immune homeostasis is fundamental to the development of autoimmune disorders. Fibroblastic reticular cells (FRCs) within lymph nodes play crucial roles in forming a microenvironment conducive to immune cell interactions and regulating autoreactive T cells driving autoimmune diseases. Studies have shown the potential of FRCs as a form of personalized cell therapy to correct dysregulated immune responses. Successful translational efforts, however, require the cells to be formulated as injectable suspensions while retaining their native architecture, which is essential for their regulatory functions. With suicide being a leading global cause of death, there is a need to explore the ethical complexities surrounding suicide and preventive measures. This dissertation questions the morality of suicide by examining the historical and philosophical dimensions across cultures and religions. The text argues for an approach that acknowledges rational suicide while developing the ethics of paternalistic measures for suicide prevention of vulnerable populations.

Coaching in Context (CinC) is an emerging intervention approach used by occupational therapy practitioners (OTPs). A growing body of literature has found CinC to be effective when used by OTPs with various clients. Background: School-based occupational therapy practitioners (OTPs) experience alarming levels of burnout and poor work satisfaction related to excessive caseload (Garfinkel & Seruya, 2018; Stephenson, 2019). The workload model has the potential for positive results (AOTA, 2014), as it compensates an OTP for all tasks performed, not just student-facing time (Garfinkel & Seruya, 2018; Corley et al., 2023). A 2022 federal policy change allowed occupational therapists (OTs) to open home health cases in a process known as the Start of Care or SOC, a responsibility previously limited to nurses, physical therapists, and speech-language pathologists. The American Occupational Therapy Association (AOTA) recognizes the opportunity for increased professional value and service utilization and called upon practitioners to advocate such and to educate home health agencies about the change. Background: Endovascular treatment with the woven endobridge (WEB) device has been widely utilized for managing intracranial aneurysms. However, predicting the probability of achieving adequate occlusion (Raymond-Roy classification 1 or 2) remains challenging.
Objective: Our study sought to develop and validate a predictive calculator for adequate occlusion using the WEB device via data from a large multi-institutional retrospective cohort.

Extracellular vehicles (EVs) are an emerging class of drug carriers and are primarily reported to be internalized into recipient cells via a combination of endocytic routes such as clathrin-mediated, caveolae-mediated and macropinocytosis pathways. Ischemic stroke is the second leading cause of death worldwide, and the lack of effective treatment options is an unmet clinical need for stroke survivors. Ischemic stroke-induced oxygen-glucose deprivation (OGD) in the affected brain tissue leads to mitochondrial dysfunction and adenosine triphosphate (ATP) depletion in the brain endothelial cells (BECs) lining the blood-brain barrier (BBB). ATP deprivation in the BECs dysregulates actin cytoskeleton dynamics, resulting in BBB breakdown. Restoration of blood flow (i.e., reperfusion) upon the first-line stroke treatment further leads to BBB breakdown and widens the paracellular spaces between BECs. …

Purposes: Study the influence of changing CBCT radiation doses (using ultra low dose and high dose systems) on soft tissues thickness measurements and compare the results with the direct measurements method. Methods:9 fresh pig mandiblue were utilized for each of two CBCT scan systems (Planmeca Promax 3D MID). Nine disposable needles were inserted into the gingival tissue of each jaw until reaching resistance from the underlying bone. For at least the past decade, one in ten infants born in the United States have delivered prematurely or before the 37th week of gestation (March of Dimes, 2024b). As a result, there are a variety of both short- and long-term effects on the infant that include both physical and neuromotor developmental delays requiring follow-up after discharge (March of Dimes, 2024b). Abstract
Background: Compassion fatigue (CF) among nurses is a significant concern affecting both individual well-being and patient care outcomes. Findings from a 2020 national survey found two-thirds of nurses (62%) struggle with CF and even a high rate for those nurses under the age of 25 years. Through a review of current literature and case studies, the effectiveness of these interventions in reducing stress, enhancing resilience, and improving overall job satisfaction among healthcare professional is examined. PURPOSE:
In the phase III HIMALAYA study (ClinicalTrials.gov identifier: NCT03298451) in unresectable hepatocellular carcinoma (uHCC), the Single Tremelimumab Regular Interval Durvalumab (STRIDE) regimen significantly improved overall survival versus sorafenib, and durvalumab monotherapy was noninferior to sorafenib. Patient-reported outcomes (PROs), a secondary outcome from HIMALAYA, are reported here.
METHODS:
Participants were randomly assigned to receive STRIDE, durvalumab, or sorafenib. PROs were assessed (preplanned secondary outcome) using the European Organization for Research and Treatment of Cancer 30-item Quality of Life Questionnaire and the 18-item HCC module. The purpose of this scholarly project was to identify and mitigate the barriers of medication adherence in an underserved population of Hispanic patients diagnosed with Type II diabetes through a nurse-driven telephone follow-up program. The program aims to assess and offer solutions available in the clinic to barriers of medication adherence. The project was implemented at the non-profit Christ Clinic located in Katy, TX. About 94% of the population are Spanish-speaking Hispanics and 200% below the federal poverty level. The patients do not have insurance due to their immigration status. HER2-positive and triple-negative breast cancers (TNBC) are difficult to treat and associated with poor prognosis. Despite showing initial response, HER2-positive breast cancers often acquire resistance to HER2-targeted therapies, and TNBC lack effective therapies. To overcome these clinical challenges, we evaluated the therapeutic utility of co-targeting TrkA and JAK2/STAT3 pathways in these breast cancer subtypes. Here, we report the novel combination of FDA-approved TrkA inhibitors (Entrectinib or Larotrectinib) and JAK2 inhibitors (Pacritinib or Ruxolitinib) synergistically inhibited in vitro growth of HER2-positive breast cancer cells and TNBC cells. Parkinson's disease (PD), which is the second most common neurodegenerative disorder, is characterized by progressive movement impairment and loss of midbrain dopaminergic neurons in the substantia nigra. Although mutations in TMEM230 are linked to familial PD, the pathogenic mechanism underlying TMEM230-associated PD remains to be elucidated. To explore the effect of TMEM230 depletion in vivo, we created TMEM230 knockout rats using CRISPR-Cas9 technology. TMEM230 knockout rats did not exhibit any core features of PD, including impaired motor function, loss of dopaminergic neurons in the substantia nigra, or altered expression of proteins related to autophagy, the Rab family, or vesicular trafficking. …

Purpose: T cells modified with chimeric antigen receptors (CARTs) have demonstrated efficacy for hematologic malignancies; however, benefit for patients with CNS tumors has been limited. To enhance T cell activity against GD2+ CNS malignancies, we modified GD2-directed CART cells (GD2.CARTs) with a constitutively active interleukin (IL)-7 receptor (C7R-GD2.CARTs).
Methods: Patients age 1-21 years with H3K27-altered diffuse midline glioma (DMG) or other recurrent GD2-expressing CNS tumors were eligible for this phase I trial (ClinicalTrials.gov identifier: NCT04099797). Phytochromes (Phys) are a divergent cohort of bili-proteins that detect light through reversible interconversion between dark-adapted Pr and photoactivated Pfr states. While our understandings of downstream events are emerging, it remains unclear how Phys translate light into an interpretable conformational signal. Here, we present models of both states for a dimeric Phy with histidine kinase (HK) activity from the proteobacterium Pseudomonas syringae, which were built from high-resolution cryo-EM maps (2.8-3.4-Å) of the photosensory module (PSM) and its following signaling (S) helix together with lower resolution maps for the downstream output region augmented by RoseTTAFold and AlphaFold structural predictions. The mental health of older adults is of national concern. There is some uncertainty regarding access to care and whether mental and behavioral health care will be readily available in primary care settings. This paper explores the concepts of integrated delivery of care and continuity of care for older adults. The definitions of the two concepts vary in detail, but are similar in purpose. There is an array of strategies presented that outline a continuity of care that involves an integrated delivery system. The remarkable increase in longevity is the pivotal triumph of public health over the last two centuries. Yet, even as we experience the benefits of increased longevity, we continue to face rampant ageism – or the prejudice and discrimination directed at individuals perceived as 'old.' Worst of all, we know that ageism is associated with increased medical costs, poorer health outcomes, and increased mortality. Efforts to combat ageism have been insufficient. Adult family caregiving is a national public health issue which is more prominently recognized since the COVID pandemic. The National Strategy to Support Family Caregivers (2022) highlights more than three hundred federal government actions in support of family caregivers and more than 150 actions that can be addressed at other levels of government with their partnerships to build a caregiver support system. Background: Although falls are preventable, falls among older adults remain a public health concern. Research indicates that over one-third of persons over 65 fall. Approximately 3 million report to emergency rooms due to falls annually. Falls are attributed to health conditions such as cognitive impairment, poor eyesight, arthritis, and polypharmacy.
Purpose: This study adapted the Teach-Back/Show-Back method for older persons in adult daycare and faith-based settings to assess their understanding of falls prevention techniques. Hoarding in late life represents a major barrier to aging in place, particularly for rural-dwelling older adults. One risk factor for hoarding disorder in late life may be underlying personality factors. However, there has not yet been an investigation of late life hoarding and possible maladaptive personality factors from the continuous five-factor model perspective in a sample of individuals diagnosed with hoarding disorder. Abstract
Introduction. Chronic obstructive pulmonary disease (COPD) is the sixth leading cause of death in the US and approximately 10% of people 65 years and older have COPD. This study examines the association of geographic-level risk factors with chronic obstructive pulmonary disease (COPD) prevalence using spatial-based analyses with the goal of identifying risk factors associated with county-level COPD and to identify communities at risk.
