This Doctor of Nursing Practice (DNP) project focused on enhancing provider knowledge and supporting the integration of FreeStyle Libre continuous glucose monitoring (CGM) technology in a family medicine and occupational health clinic. A gap in provider understanding of CGM technology and its role in diabetes management was identified. The intervention targeted a diverse group, including medical assistants, management, clerical staff, occupational health specialists, and medical providers. A pre- and post-education knowledge assessment was conducted, and results showed an increase in provider understanding. The pre-intervention average score was 76.52 (SD = 2.64), which increased to 98 (SD = 3.34) post-intervention. While …

The NurseOneTap Project is a staff education initiative designed to improve nursing documentation accuracy and efficiency through the implementation of a culturally sensitive electronic health record (EHR) system. This project addressed a critical gap in nursing practice—transitioning from paper-based documentation to EHR systems—by providing structured educational interventions to enhance staff proficiency, confidence, and documentation accuracy. A pre- and post-evaluation methodology was employed to assess improvements in system navigation, real-time data access, workflow optimization, and documentation accuracy. Results demonstrated a 54.5% increase in staff proficiency, a 50% improvement in documentation accuracy, and a 76% rise in staff confidence in using the …

In critical illness, all elements of gut function are perturbed. Dysbiosis develops as the gut microbial community loses taxonomic diversity and new virulence factors appear. Intestinal permeability increases, allowing for translocation of bacteria and/or bacterial products. Epithelial function is altered at a cellular level and homeostasis of the epithelial monolayer is compromised by increased intestinal epithelial cell death and decreased proliferation. Gut immunity is impaired with simultaneous activation of maladaptive pro- and anti-inflammatory signals leading to both tissue damage and susceptibility to infections. Introduction: Elevated circulating IL-6 levels are associated with poorer outcomes after stroke, and increased serum IL-6 levels are linked to a higher risk of stroke. IL-6 binds to soluble IL-6 receptors (sIL-6R) and subsequently to ubiquitously expressed gp130, initiating proinflammatory trans-signaling. This study tested the hypothesis that inhibiting IL-6 trans-signaling by administering soluble (s) gp130 improves long-term functional outcomes in young mice after stroke.

Active avoidance is a core behavior for human coping, and its excess is common across psychiatric diseases. The decision to actively avoid a threat is influenced by cost and reward. Yet, threat, avoidance, and reward have been studied in silos. We discuss behavioral and brain circuits of active avoidance and the interactions with fear and threat. BACKGROUND: This article examines predictors of subsequent total knee arthroplasty (TKA) within 6 months of nonoperative intervention based on (1) patient demographics; (2) knee osteoarthritis (OA) severity; and (3) various nonoperative treatments (cryoneurolysis with superficial or deep genicular nerve block, intra-articular [IA] hyaluronic acid injections, nonsteroidal inflammatory drug injections, IA-corticosteroids injections, or IA-triamcinolone extended-release [IA-TA-ER] injections). Tau is a microtubule-associated protein that undergoes liquid-liquid phase separation (LLPS) to form condensates under physiological conditions, facilitating microtubule stabilization and intracellular transport. LLPS has also been implicated in pathological Tau aggregation, which contributes to tauopathies such as Alzheimer's disease. While LLPS is known to promote Tau aggregation, the relationship between Tau's structural states and its phase separation behavior remains poorly defined. Here, we examine how oligomerization modulates Tau LLPS and uncover key distinctions between monomeric, oligomeric, and amyloidogenic Tau species. Data indicate that earlier initiation of anti-tumor necrosis factor alpha (anti-TNF-α) biologic medicines may prevent progression to irreversible bowel damage and improve outcomes for patients with inflammatory bowel disease (IBD), particularly Crohn's disease. However, the high cost of such therapies may restrict access and prevent timely treatment of IBD. Biosimilar anti-TNF-α medicines may represent a valuable opportunity for cost savings and optimized patient outcomes by improving access to advanced therapies and allowing earlier anti-TNF-α treatment initiation. Diabetes is a major public health issue, particularly in underserved communities with limited healthcare access. This study evaluates the effectiveness of the Comprehensive Care Model (CCM) in improving diabetes management at three Area Health Education Center (AHEC) sites in the Rio Grande Valley (RGV). A retrospective analysis was conducted on 858 patients, categorized as prediabetic (A1C 5.7-6.4) or diabetic (A1C ≥6.5). Of these, 54 patients in each group had confirmed follow-up A1C values after a minimum of three months. NM_000141.5: FGFR2 c.1032G>A is a pathogenic variant that causes Crouzon syndrome through activation of a new donor splice site. This clinical report highlights the intrafamilial variability that can exist with this specific variant. The proband is a 4-year-old boy who initially presented with concern for seizures. Computed tomography and magnetic resonance imaging revealed pancraniosynostosis and Chiari 1 malformation. Papilledema on fundoscopic exam and copper beaten appearance of the skull on imaging both suggested the presence of elevated intracranial pressure. The current study examines the association between language preference, social determinants of health (SDOH) risk factors, viral load, and HIV care outcomes among individuals of Hispanic ethnicity in the United States. More than half (54%) of all participants reported having two or more unmet needs. Hispanic individuals were more likely to be at risk in 2 or more domains than their non-Hispanic counterparts. Food insecurity was the most common risk factor reported and when present, 51% of patients also reported financial resource strain. Surprisingly, when language preference was analyzed, English-preferring Hispanic patients were at higher risk of financial resource strain, …

The prognostic impact of measurable residual disease (MRD) in acute myeloid leukemia (AML) is unequivocal; however, the optimal time point for achieving undetectable MRD is unclear. We retrospectively studied patients with newly diagnosed (ND) AML who achieved remission with frontline intensive chemotherapy and had MRD assessed by flow cytometry after induction (time point 1 [TP1]) and after cycles 2 or 3 (TP2). Cases were grouped into MRD negative (Neg)/Neg, positive (Pos)/Neg, or Pos/Pos at TP1 and TP2, respectively. This study aimed to examine how perceived parental mental and physical health status was associated with child physical activity, screen time, and sleep. Participants were 6- to 17-year-old minors (N = 20,156; 47.9% female) from the 2022 US National Survey of Children’s Health. Dependent variables were children’s physical activity frequency, screen time, and sleep. Independent variables were parents’ physical and mental health status. Using the Actor-Partner Interdependence Model, two adjusted path models were constructed. Post-surgical spinal infection occurs in up to 20 % of patients, despite aggressive peri-operative antibiotic treatments. To improve prophylaxis, we have designed and evaluated an ultrasound-activated prophylactic antibiotic release system to combat post-surgical bacterial survival. Polylactic acid (PLA) clips (1 cm³) were 3D-printed with an interior reservoir (0.8 cm³) for carrying drug payload, specifically vancomycin (VAN). Under IACUC approval, clips were surgically implanted into the spines of sheep (n = 9) and swine (n = 2) by removing the spinous process at several levels of the lumbar spine. The Woven EndoBridge (WEB) device is FDA-approved for the treatment of bifurcation aneurysms. Despite its wide popularity, it has been under scrutiny for its association with potential aneurysm recanalization and retreatment due to device shape modification. Enteric pathogen rotavirus (RV) primarily infects mature enterocytes at the tips of the intestinal villi; however, the role of secretory Paneth and goblet cells in RV pathogenesis remains unappreciated. Atoh1 knockout mice (Atoh1cKO) were used to conditionally delete Paneth, goblet, and enteroendocrine cells in the epithelium to investigate the role of secretory cells in RV infection. Unexpectedly, the number of infected enterocytes and the amount of RV shedding in the stool were greatly decreased following secretory cell deletion. Tumors are complex ecosystems of interacting cell types. The concept of cancer hallmarks distills this complexity into underlying principles that govern tumor growth. Here, we explore the spatial distribution of cancer hallmarks across 63 primary untreated tumors from 10 cancer types using spatial transcriptomics. We show that hallmark activity is spatially organized, with the cancer compartment contributing to the activity of seven out of 13 hallmarks, while the tumor microenvironment (TME) contributes to the activity of the rest. Background: Antiangiogenics combined with immune checkpoint blockade have become standard of care for recurrent endometrial cancer after standard platinum-based chemotherapy. To dissect mechanisms and define biomarkers associated with clinical outcomes to these combinations, we applied multidimensional immune monitoring to peripheral blood specimens collected from a randomized phase 2 trial of nivolumab with or without cabozantinib in 75 evaluable patients with recurrent endometrial cancer (NCI ETCTN 10104, NCT03367741). This trial demonstrated superiority of the combination to nivolumab alone.
