The Immunology Of Inflammatory Bowel Disease, Hugh L. N. Mackechnie

Although several measurements of both cellular and humoral immunity are altered in inflammatory bowel disease (IBD), no direct pathogenic role can be confirmed in either ulcerative colitis (UC) or Crohn's disease (CD). No clear role has been shown for allergic, microbiological, or psychogenic factors, and no specific genetic susceptibility has been demonstrated. Several studies have shown some benefit from using immunosuppressive drugs to treat inflammatory bowel disease, such as sulfasalazine to treat ulcerative colitis and corticosteroids to treat Crohn's disease. Whether these benefits derive from the anti-inflammatory activity of these drugs or from their immunological effects is uncertain.

The occurrence of leukemia in Burkitt's lymphoma, with or without visceral or nodal tumefaction is uncommon, and its initial presentation as leukemia is even more unusual. Because it has a poor chemotherapeutic response and a grave prognosis, it is important to recognize this unusual leukemia correctly. Our report describes the clinical and pathologic findings of Burkitt's lymphoma cell leukemia in a five-year-old white boy who presented with abdominal distension, hepatosplenomegaly, and lymphadenopathy. Blood examination revealed normocytic normochromic anemia, erythroblastosis, slight leukocytosis, and the presence of numerous (24%) blasts. Strange Males Block Pregnancy In Lactating Pine Voles, Microtus Pinetorum, And Reduce Survival And Growth Of Nursing Young, Margaret H. Schadler

Strange (unfamiliar) males affect survival of progeny of reproductive female rodents in a variety of ways. In mice, strange males block pregnancy before implantation of embryos (Bruce, 1959) but only in females that are not lactating (Bruce and Parkes, 1961). In voles these males can block pregnancy both before and after implantation (Stehn and Richmond, 1975; Marks and Schadler, 1979; Schadler, 1981). Strange males have also been shown to kill nursing pups in lemmings (Mallory and Brooks, 1978) and mice (Labov, 1980 and vom Saal and Howard, 1982).

Zoonoses are infections transmitted by animals to man. Of the more than 50 such infections that are clinically significant, several are discussed in the articles by McIntosh and Madhavan and by Meyer and Adar in this issue of the Journal (pp. 7-13). These include brucellosis, leptospirosis, relapsing fever, Colorado tick fever, toxoplasmosis, psittacosis, and echinococcosis. The McIntosh and Madhavan article reviews the experience at Henry Ford Hospital with 60 patients, while the article by Meyer and Adar reports on an unusual case of echinococcosis of the spine and retroperitoneum that occurred at Haim Sheba Medical Center in Israel.

Zoonoses are diseases of animals that can be transmitted to humans. We reviewed the experience with zoonotic illnesses treated at Henry Ford Hospital in the past ten years. Included in this report are cases of brucellosis, leptospirosis, relapsing fever, Colorado tick fever, toxoplasmosis, psittacosis, and pasteurellosis. This report describes the clinical and epidemiologic aspects of zoonoses in 60 patients in a large urban general hospital.

Echinococcosis is a zoonosis transmitted from sheep dogs to humans. It occurs most frequently in sheep and cattle-raising countries, and children who come in contact with sheep dogs are the most likely victims. The liver and the lungs are the most common sites of the hydatid cysts. Their presence in other organs is less common, and primary hydatidosis of the retroperitoneum is extremely rare. In the case reported here, a 65-year-old man had secondary retroperitoneal echinococcosis that recurred three times in 23 years. On the third occasion, marsupialization was performed on a large cyst found in the right retroperitoneum.

Racial Differences In The Incidence Of Steroid Diabetes In Renal Transplant Patients, Francis Dumler, Hajime Hayashi, Jay Hunter, Nathan W. Levin

We have studied the development of steroid-induced diabetes in a population of 143 renal allograft recipients who were nondiabetic before transplantation. Steroid-induced diabetes developed In 9.8% of patients. However, in blacks its incidence was significantly higher than in whites (17.3% vs 5.5% respectively; p < .01). The development of steroid-induced diabetes was not associated with a higher frequency of HLA-B8 or HLA-Bw15 in either race. In black graft recipients, HLA-B14 was significantly more frequent (p < .001) among those who developed steroid-induced diabetes than in insulin-dependent diabetic (Type I) and nondiabetic recipients. The clinical course of patients with steroid-induced diabetes has been similar to that of noninsulin-dependent diabetics (Type II).

Fine needle aspiradon biopsy is a simple and accurate means of obtaining a tissue diagnosis for a thyroid nodule. Our experience with 3,000 such biopsies and 242 surgically proven cancers has enabled us to characterize the cytologic presentations of the common "nodular" thyroid diseases. We can readily identify nodular goiter and papillary, poorly differentiated follicular, medullary, anaplastic, and metastatic cancers. Benign follicular neoplasms and well-differentiated follicular cancers cannot be separated with great accuracy, but useful predictions can be made. Care is required to recognize the cellular abnormalities of autoimmune thyroiditis and to diagnose the coexistence of this disease with malignant …

Two patients developed progressive muscle weakness in adult life. The initial diagnosis of polymyositis was made in both cases, but subsequent studies and the distribution of muscle involvement suggested facioscapulohumeral myopathy. Other investigators have proposed that this syndrome may result from either genetic or acquired causes. In our patients, the disorder was probably a sequela of polymyositis.

Aquatic toxicity tests were conducted on 11 organic compounds considered hazardous to water resources. The toxicity of each compound was evaluated using embryo-larval stages of two to eight fish and amphibian species. Exposure was initiated at fertilization and maintained through 4 days posthatching. The animal test species exhibited varying degrees of sensitivity to the selected toxicants. Combined frequencies for mortality and teratogenesis at 4 days posthatching gave LC50 ranges of 3.66 to 8.25 mg/L for benzene, 1.16 to 22.42 mg/L for carbon tetrachloride, 0.11 to 1.20 mg/L for chlorobenzene, 2.03 to > 68 mg/L for chloroform, 3.01 to 5.56 mg/L …

Antibody raised against the purified human erythrocyte glucose transporter specifically precipitated four proteins from normal and Rous sarcoma virus-transformed chicken embryo cells: a major protein of Mr 41,000 and minor proteins of Mr 68,000, 73,000, and 82,000. The Mr 41,000 and 82,000 proteins were found only in a membrane fraction, not in the soluble fraction, and displayed a heterogeneous mobility on NaDodSO4/polyacrylamide gel electrophoresis, suggesting glycosylation. The Mr 41,000 and 82,000 proteins were increased in amount after malignant transformation in direct proportion to the increase in hexose transport rate, and the increase was dependent on the expression of the src …

Millions of dollars are spent annually on special educational programs for children whose severe behavior disorders prevent them from participating in the regular school setting despite average or above average intellectual capacity. A growing body of research indicates that some of these behavior disorders are related to nutritional problems. (Pfeiffer and Iliev 1972; Kittler 1973; Mayron 1979; and Buckley 1977), and many clinicians support the view that no matter what the etiology of behavioral disorders, nutritional programs can improve the baseline data on medical, social and intellectual achievement, and on personality measures (Palmer 1978). Meaning In Work: Toward A Clinical Approach To Work Dissatisfaction, Shimshon M. Neikrug
The Journal of Sociology & Social Welfare
This paper deals with issues relating to the meaning of work for the worker and clinical implications for the social worker intervening in the field of industrial and occupational social work. The problems of work dissatisfaction and alienation in work are analyzed. The author presents two concepts, work as play and logotherapy, as being useful in the clinical intervention in work related problems. It is concluded that the absence of work dissatisfaction need not be the goal of intervention, rather the clinical encounter can strive for meaningfulness, growth, and self-discovery.

The Relationship Of Educational And Employment Experience In Psychiatric Nursing To The Attitudes Of Psychiatric Nursing Students And Graduates Toward Mental Illness, Margaret Elaine Lobdell

In a non-experimental descriptive-comparative study with correlational components, the relationship of educational and employment experience in psychiatric nursing to the attitudes of psychiatric nursing students and graduates toward mental illness was investigated. The first sample consisted of eight female students enrolled in the second year of a Registered Psychiatric Nursing program in a province in western Canada. The second sample consisted of 10 female graduates of the same program who had been employed in psychiatric settings for up to two years after completion of the program.

การศึกษาผลทางพยาธิสภาพของอวัยวะสืบพันธุ์ แม่โคเนื้อที่เป็นโรคบรูเซลโลซีส เพื่อต้องการผลทาง Histo-pathological lesion ของระบบอวัยวะสืบพันธุ์ เนื่องจากการทําลายของเชื้อ Brucella spp. ที่มีผลโดยตรงต่อความสมบูรณ์พันธุ์ของ แม่โคเนื้อ และเพื่อเปรียบเทียบผลทางพยาธิวิทยาของโรคบรูเซลโลซิสกับค่าทาง Antibody agglutination titer ของโคเนื้อที่เป็นโรคบรูเซลโลซีล เพื่อเน้นถึงความรุนแรงของโรคนี้ที่มีต่อการผลิตปศุสัตว์โคเนื้อทางด้านเศรษฐกิจ โรคบรูเซลโลซีสหรือโรคแท้งติดต่อ (Contagious abortion disease) นับว่าเป็นอุปสรรคต่อการพัฒนาปรับปรุงเกี่ยวกับสมรรถภาพความสมบูรณ์พันธุ์ของปศุสัตว์ (Animal fertility) อย่างมากประการหนึ่ง จัดเป็น specific venereal disease ที่สำคัญที่สุดในทางโรคปศุสัตว์ นอกจากมีผลทางด้านการผลิตปศุสัตว์ โดยเฉพาะโคนม โคเนื้อ กระบือ และสุกร อันมีผลทําให้เกิดอาการแท้งลูก เป็นหมันผสมติดยาก ตลอดจนมีโรคแทรกซ้อนต่าง ๆ ในทางระบบสืบพันธุ์แล้วยังมีผลต่อสมรรถภาพการให้น้ำนมอย่างในโค ทําให้ลูกสัตว์ที่คลอดจากแม่ที่เป็นโรคนี้มีสุขภาพอ่อนแอมากมีอัตราการตายภายหลังคลอดสูง และที่สำคัญที่สุดอีกประการหนึ่งคือ สามารถติดต่อถึงคนได้โดยทําให้คนเป็นโรค Undulant fever นับว่าเป็น Zoonotic disease ที่มีผลต่อ Public health มาก

ศึกษาการตรวจท้องแม่สุกรโดยการใช้คลื่นเสียงจากความถี่สูงในแม่สุกรพันธุ์ผสม 2 สายเลือด (ลาจไวท์แลนด์เรส) จำนวน 172 ตัวจากฟาร์มเอกชน 2 แห่งในจังหวัดนครปฐม โดยใช้เครื่องตรวจแม่สุกรในท่ายืนบริเวณตำแหน่งเหนือเต้านมคู่ที่ 3 และ 4 (นับจากข้างหลัง) ขึ้นมาประมาณ 3 นิ้ว ส่องทิศทางของเครื่องสู่แนวล้นหลัง ตรวจหลัง จากผสมตามธรรมชาติ 19 - 30 วัน (2 ตัว) 31 - 60 วัน (115 ตัว) และ 61 - 110 วัน (15 ตัว) ผลการตรวจท้องแม่สุกรโดยวิธีพบว่าเปอร์เซ็นต์ความถูกต้อง 90. 17%, 94. 78% และ 100% ตามลำดับ แสดงให้เห็นว่าเราสามารถนำวิธีนี้มาใช้ใน การตรวจการตั้งท้องของสุกรได้
