Chronic Idiopathic Thrombocytopenic Purpura: Effective Preoperative Preparation And Long-Term Results Of Splenectomy, James C. Gruenberg, Melvin A. Block, Ellis J. Van Slyck, Joseph P. Abraham

A retrospective review of 98 patients seen at Henry Ford Hospital from 1953 through 1977 demonstrated that splenectomy for chronic idiopathic thrombocytopenic purpura provided a good response which usually was sustained on long-term follow-up (72% at 15 years). Although splenectomy for this condition had a low mortality, morbidity was significant in patients older than 40 years. When compared to patients whose response to splenectomy was sustained, patients who relapsed had significantly lower platelet counts preoperatively both when they were untreated (mean: 9,194 per cc versus 18,524 per-cc) and/or when they were treated with steroids (mean: 85,647 per cc versus 142,590 …

Effect Of Levamisole On The Incidence Of Spontaneous Mammary Tumors In C3h Mice, Robert H. Page, Duilio A. Valdivia, Robert W. Talley, Robert A. Huseby

Spontaneous mammary carcinoma in C3H mice is preceded by premalignant hyperplastic alveolar nodules. When a primary tumor is evident, examination of the other mammae will reveal a number of microscopic precancerous lesions in various stages of transformation to overt malignant tumors. In an effort to inhibit the development of additional malignant tumors after surgical removal of the first apparent tumor, we treated female C3H/HeJ mice with subcutaneous injections of levamisole (0.6 mg per kg of body weight) on a twice weekly regimen. Acid Phosphatase: Clinical Utility Of The First Tumor Marker, Christopher J. Dipaolo, Jan Rival

Acid phosphatase was the first tumor marker to be measured in blood. Although it is used as a marker of prostatic carcinoma, serum levels are also elevated in many diseases of prostate, bone, and hematologic tissue. While serum acid phosphatase concentration is elevated in patients with prostatic carcinoma or infarction, it may also be increased merely with benign hypertrophy and after prostate massage. Recent studies demonstrate that levels are not significantly increased after routine rectal examination. Although of limited use as a screening test, the assay may be useful in staging, prognostication during hormonal therapy, and in making clinical decisions. …

The Central Surgical Association assembled for the first time in Detroit and Ann Arbor, Michigan in February, 1941. On the 40th anniversary of this meeting, the Association met again in the same geographical region, in Dearborn. Selected papers from the first twenty years have been chosen as highlights. Criteria for choosing a few from many significant papers were the originality of the material, its time-proven value, and the subsequent impressive careers of the authors. Compression of the duodenum by the root of the mesentery is a rare pathological entity that has caused much controversy because it has been mistakenly identified as the "superior mesenteric artery syndrome" or the "vascular compression of the duodenum." We discuss the pathological anatomy and the diagnosis of the disease, and report an illustrative case. Some authors report many cases of the syndrome, while others doubt its existence. Before surgery is considered, the diagnosis should be established beyond doubt and a prolonged period of observation, medical treatment, and nutritional support should be provided. Rupture Of Mycotic Aneurysm During Pregnancy: Another Hazard Of Drug Abuse, Jayanta Kumar Dirghangi, Mohammed R. Ansari

Widespread parenteral drug abuse has led to an increase in mycotic aneurysms in the area of drug injection. We report an unusual case of ruptured mycotic aneurysm of the right external iliac artery in a pregnant woman who was a heroin addict. In the third trimester, she developed a large hematoma that resulted in intra-uterine fetal death, premature labor, and hypovolemic shock. This case presented a difficult diagnostic and therapeutic dilemma that was confused with the syndrome of ruptured utero-ovarian veins.

Data on actual labor time can be utilized as an effective management tool. With labor cost representing about 50 percent of direct cost in food service, it is essential to examine how labor time is being utilized.

A continuous time study was conducted to determine the total labor time for the production of eight vegetarian entrees. The two work areas observed were the ingredient assembly area and the cooks' production area. This labor time was recorded by Work Functions to find the dis- tribution of labor time. The lack of scientific information regarding caring beliefs, expectations, and behaviors in the Chinese community of Hong Kong was the problem underlying this descriptive study. A knowledge of caring behaviors as they routinely occur within a culture can provide a basis for the development of professional nursing care that is meaningful for the people of that culture. The purpose of the study was to obtain the opinions of the residents of a Hong Kong Chinese village regarding current beliefs and practices relating to touch, surveillance, health counseling, and special ethnocare techniques. Two rapid palatal expansion appliances, acrylic-free and acrylic-reinforced, were compared for their effect on the cross-sectional area of the palate. Two groups, twenty-five patients in each, were selected so that they would match as closely as possible with regards to age and sex distribution. The pre-treatment and post-treatment maxillary models were duplicated, and this duplicate model was then trimmed in a specific manner to allow the cross-sectional area between the first permanent molars to be compared. The results indicate that there is no difference, at the five percent level of significance, between these two appliances. It is concluded that the …

Mechanical expansion of the maxilla, at the midline suture, is a widely accepted orthodontic procedure used to gain arch width in certain types of maxillary deficient arches.

In animal research on the mandible, techniques using surgical separation at the symphysis with immediate expansion up to 12 mm, fixation with a metal splint, and autogenous bone transplanted into the expanded area, have been reported to be successful.

To date, no experiment utilizing slow mechanical expansion of the mandible at the symphysis has been reported.

This study was designed to determine whether varying criterion for pass/fail on the Mini-Screening Language Test for Adolescents (Mini-STAL) would increase accuracy of predicting outcome of the Screening Test of Adolescent Language (STAL). The Mini-STAL was developed by Prather et al. (1981) to identify rapidly those students between grades six through twelve who are in need of language intervention. Using Prather's established criterion (one or more errors equal failure), the Phoenix school district (Prather, 1981) found too many of their school population (20 percent) were failing the Mini- STAL. This thesis presents an improved oscillotonometric system for the measurement of human blood pressure. The study included:

1. The design of a hydraulic occlusive cuff,

2. The investigation of the wave forms taken from the blood pressure measurements, and

3. The design of a mechanism for the simulation of human blood pressure pulse.

In this study, an experimental system consisting of a rigid shell occlusive cuff, a constant volume displacement pump, a transducer, and a chart recorder was designed and used for data collection.

A review of the literature on self-disclosure and small group communication showed that few investigations dealt with the effects of disclosing communication on small group process. The present study was conducted in order to assess the effects of the frequency of self-disclosure within small groups on the members' overall attraction to the group. 120 subjects were selected from the East Hill Church senior high youth department in Gresham, Oregon. Subjects were placed at random into ten small groups with twelve members each which met for one-half hour per week for six consecutive weeks. All group communications were audio tape recorded. …

Currently language is viewed as a multi-dimensional construct, consisting of three interrelated components; content, form and function (Bloom and Lahey, 1978). In recent publications, language function has been referred to as pragmatics (Prutting, 1979). The essence of pragmatic theory is that language is used functionally in social contexts, to accomplish specific types of communication (Wood, 1981). One such communicative function is referred to as the informative function, which occurs when a speaker provides ideas and information to others, as in describing, naming and giving examples {Wood, 1981).
