Atrial fibrillation (AF) is the most common cardiac arrythmia impacting nearly six million Americans. Symptoms associated with AF significantly impact quality of life and can have serious health consequences. Risk factor optimization through lifestyle modification is a crucial component in AF care plans though often deferred to primary care providers.

This project assessed the impact of using a shared decision-making tool on patient knowledge regarding their AF care plan and assessed patient and provider perception of using the tool during routine AF visits.

Background
Hypertension is a major risk factor for cardiovascular disease, contributing to26% of annual U.S. deaths. Unreliable office-based blood pressure (BP) measurements have spurred the adoption of self-measured BP (SMBP). However, only half of adults aged fifty to eighty with hypertension regularly monitor their BP and share BP measurements with healthcare providers. Bluetooth-enabled SMBP (BT-SMBP) is a potential solution to promote at-home monitoring and information sharing.

Pancreatic ductal adenocarcinoma (PDAC) is the most common type of pancreatic cancer and among the most aggressive malignancies. It is projected to be the second leading cause of cancer related mortality in the United States by 2030. Due to its nonspecific early symptoms, PDAC often remains undetected until advanced stages, limiting treatment options and reducing survival rates.
Purpose
This project evaluated the impact of geographic distance on diagnostic staging and treatment initiation for patients with PDAC referred to the Providence Cancer Institute of Oregon (PCIO) from 2018-2023.

Breast cancer is the most common cancer across all ethnic groups with 270,000 women in the U.S. diagnosed yearly (Centers for Disease Control and Prevention [CDC], 2023). Breast cancer is the second causative factor of death in women with mortality estimates greater than 42,000 annually. Screening and early referral is essential. Mammography is the gold standard for detecting early, non-palpable, and preventable cancers. Primary care clinics can impact screening efforts by creating policies and utilizing electronic health records (EHR) to identify, track, and notify adult female patients who meet recommendations for breast cancer screening (BCS).

BACKGROUND: Personal statements are a required component of plastic surgery residency applications but can be extremely time- and labor-intensive. Artificial intelligence (AI) programs like ChatGPT can streamline personal statement writing, but their use, especially if undisclosed, can have ethical implications. This study elucidates the perspective of plastic surgery residency program directors (PDs) regarding the importance of personal statements in reviewing applicants and whether ChatGPT should be utilized.

Introduction: The number of tracheal intubation attempts required to reach proficiency in videolaryngoscopy with hyperangulated blades is unknown. Understanding this training requirement might guide training for clinicians who perform laryngoscopy. We therefore performed a planned sub-analysis of a randomised controlled trial comparing tracheal intubation success with videolaryngoscopy vs. direct laryngoscopy to determine the number of tracheal intubations with a hyperangulated videolaryngoscope blade needed to provide an acceptable first-attempt success rate.

Essential workers were at increased risk during the COVID-19 pandemic, including seafood processors who are often rendered invisible within the public sphere. To examine the health and safety concerns of seafood processors, many who are low income or im/migrant workers on H-2B visas, our team conducted qualitative research with 44 participants. We found that in addition to high occupational health hazards that existed before the pandemic, COVID-19 increased workers' financial risks, which put them in more dangerous health and safety positions, since they needed to work through physical and mental health illness. Introduction: The Global Observatory for Physical Education (GoPE!) builds on the Global Observatory for Physical Activity (GoPA!) to address gaps in surveillance, policy, and research on physical education (PE) and activity (PA). GoPE! is a worldwide surveillance system of quality PE and school-based PA that informs policy and interventions and allows comparative analysis so that each country and region can check their status and progress. Heart failure (HF) is a major contributor to global morbidity and mortality. While distinct clinical subtypes, defined by etiology and left ventricular ejection fraction, are well recognized, their genetic determinants remain inadequately understood. In this study, we report a genome-wide association study of HF and its subtypes in a sample of 1.9 million individuals. A total of 153,174 individuals had HF, of whom 44,012 had a nonischemic etiology (ni-HF). A subset of patients with ni-HF were stratified based on left ventricular systolic function, where data were available, identifying 5,406 individuals with reduced ejection fraction and 3,841 with preserved ejection fraction. …

BACKGROUND: Our institution carried out a multi-center, group-randomized controlled trial to evaluate the effectiveness of two community-based interventions on promoting the uptake of COVID-19 testing and vaccination in three regions with high levels of health disparities in Texas. We selected Census Block Groups (CBGs) with high disparity for randomization. In each study region, selected CBGs were randomized into two intervention groups and one control group. An important goal was to ensure balanced distributions of two continuous covariates, the disparity index and population size, across study arms. Chronic subdural hematoma (cSDH) presents a multifaceted challenge in contemporary neurological practice, necessitating innovative therapeutic strategies. This comprehensive review explores the convergence of two promising interventions: statins and middle meningeal artery (MMA) Embolization. With cSDH incidence on the rise, particularly among the aging population and those with long-term anticoagulant usage, the traditional surgical avenues face limitations in recurrence rates and associated risk factors. Statins, heralded for their anti-inflammatory, vasculogenic, and angiogenic properties, are emerging as potential allies in cSDH management. Physiological processes such as the sleep-wake cycle, metabolism, hormone secretion, neurotransmitter release, sensory capabilities, and a variety of behaviors, including sleep, are controlled by a circadian rhythm adapted to 24-hour day-night periodicity. Disruption of circadian rhythm may lead to the risks of numerous diseases, including cancers. Several epidemiological and clinical data reveal a connection between the disruption of circadian rhythms and cancer. On the contrary, oncogenic processes may suppress the homeostatic balance imposed by the circadian clock. Aims/hypothesis: DNA methylation studies of incident type 2 diabetes in US populations are limited and to our knowledge none include individuals of African descent. We aimed to fill this gap by identifying methylation sites (CpG sites) and regions likely influencing the development of type 2 diabetes using data from Black and White individuals from the USA.

Background: Acute kidney injury (AKI) is a common postoperative complication, and hypotension may contribute. We therefore tested the primary hypothesis that individualized intraoperative blood pressure regulation reduces postoperative AKI in older surgical patients.
Methods: We enrolled patients ≥60 years old scheduled for elective major abdominal surgery with invasive arterial pressure monitoring. All had goal-directed fluid management based on stroke volume variation cardiac output, and administration of a starch. This trial assessed the pharmacokinetics, pharmacodynamics, and safety of liposomal bupivacaine given via ultrasound-guided popliteal sciatic nerve block with or without immediate-release bupivacaine hydrochloride in adults having bunionectomies. Forty-five adults were enrolled into four sequential cohorts: (1) liposomal bupivacaine 266 mg with bupivacaine hydrochloride 50 mg; (2) liposomal bupivacaine 133 mg with bupivacaine hydrochloride 50 mg; (3) liposomal bupivacaine 266 mg; or (4) bupivacaine hydrochloride 100 mg. Background: Effect of apolipoprotein E (APOE) on Alzheimer's disease and related dementias (ADRD) risk is heterogeneous across populations, with scarce data on Hispanics/Latinos.
Methods: APOE genotype was studied in 12,221 Hispanics/Latinos (per cohort and via metanalysis): Caribbean-Hispanics, Mexicans, Mexican-Americans, and Peruvians/Bolivians. A subsample had longitudinal assessment and plasma p-tau. We tested the modifying effects of global and local ancestries. Results were replicated in an independent Peruvian cohort and brain samples.

Oral cancer is a major global health problem. It is commonly diagnosed at an advanced stage, although often preceded by clinically visible oral mucosal lesions, termed oral potentially malignant disorders, which are associated with an increased risk of oral cancer development. There is an unmet clinical need for effective screening tools to assist front-line healthcare providers to determine which patients should be referred to an oral cancer specialist for evaluation. Introduction: Despite global efforts to reduce adolescent birth rates, the problem persists in low- and middleincome countries. Sub-Saharan Africa has the highest rates (97.9 births per 1000 women). In Kenya, more than 15 % of adolescent girls become pregnant. Forty-three percent of these develop depression. This study investigated adolescent pregnancy in rural Kenya to understand the challenges and context-specific factors that can help adolescents maintain good mental health during pregnancy and the motherhood.

Pregnant and lactating women in sub-Saharan Africa are vulnerable to micronutrient inadequacies, with risk of adverse pregnancy outcomes. Adequate intakes of diverse foods are associated with better micronutrient status and recommended by the World Health Organization as part of healthy eating counselling during antenatal care. However, our understanding of community knowledge of dietary diversity within the context of maternal diets is limited. Despite the high disease burden, noninvasive cardiac output monitors have not been used extensively in low- and middle-income countries.
