Objectives: Atrial fibrillation (AF) is a common comorbidity in patients undergoing mitral valve surgery (MVS), significantly increasing the risk of thromboembolism, heart failure, and mortality. Surgical ablation has recently been given a Class IA recommendation for concomitant MVS, reinforcing its role in restoring sinus rhythm and improving outcomes in this population. However, concerns about procedural complexity, extended operative time, and postoperative risks have limited its widespread adoption. Total knee arthroplasty (TKA) is becoming increasingly prevalent, with projections of a significant rise in cases by 2050. Optimal perioperative management is essential for enhancing postoperative recovery, minimizing complications, and supporting early discharge. Spinal anesthesia, particularly with long-acting agents like bupivacaine, is commonly used; however, it may lead to prolonged motor blockade and postoperative urinary retention (POUR), impacting recovery. Recent research suggests that mepivacaine, an intermediate-acting spinal anesthetic, may offer improved recovery outcomes for TKA patients compared to bupivacaine. COVID-19 has put immense pressure on healthcare facilities, especially in managing patients with comorbidity. This study addressed the association between two important coinfections, congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD), and their effect on hospital length of stay (LOS) among COVID-19 patients. The study also explored the predictive role of the inflammatory biomarker C-reactive protein (CRP) in prolonged hospitalization, also considering the influence of age and race. Background: Cardiopulmonary bypass (CPB) is an essential part of cardiac surgery, but it leads to SIRS (systemic inflammatory response syndrome) which in turn increases postoperative complications. There are many proposed approaches to reduce inflammation, but there is no clear best practice on the approach to use. Westminster Home Connection is a non-profit agency serving low income adults 60+ or individuals with a disability by providing home repair and modifications to allow individuals to safely live in their home. The primary focus of this project was to create educational materials and bring an OT perspective to the assessment and modification process. Resources provided included a guidebook for home modifications and fall prevention education. The overprescription and misuse of opioids like oxycodone has played a significant role in the ongoing opioid crisis. While effective painkillers, drugs like oxycodone are also reinforcing and often misused. While sex and ovarian hormones can impact the analgesic effects of these drugs, the impact of these factors on oxycodone reinforcement are not fully understood. This relationship between ovarian hormones and opioid reward is further complicated by the fact that opioid use can disrupt ovarian hormone levels and produce reproductive cycle irregularities. IMPORTANCE: Driving under the influence of cannabis increases the risk of motor vehicle collisions. In some jurisdictions, deterrence rests on the ability to detect delta-9-tetrahydrocannabinol (THC) in blood. Recent evidence suggests that there may be a nuanced relationship of blood THC to driving.

OBJECTIVE: The purpose of this systematic review was to summarize all published papers investigating the presence of a linear relationship between blood THC and driving, primarily measured by simulated driving in the lab.

This capstone project explores the role of occupational therapy (OT) in managing pain and enhancing quality of life (QoL) for individuals following lower limb amputation (LLA). Despite OT's recognized potential in addressing physical and psychosocial challenges, there is a lack of research specifically examining its impact on pain management outcomes post-amputation. This mixed-methods study involved quantitative surveys and qualitative interviews with 11 participants to assess their experiences with OT services, pain strategies, and QoL. Particulate matter containing environmentally persistent free radicals (EPFRs) is formed when organic pollutants are incompletely burned and adsorb to the surface of particles containing redox-active metals. Our prior studies showed that in mice, EPFR inhalation impaired vascular relaxation in a dose- and endothelium-dependent manner. We also observed that activation of the aryl hydrocarbon receptor (AhR) in the alveolar type-II (AT-II) cells that form the air-blood interface stimulates the release of systemic factors that promote endothelial dysfunction in vessels peripheral to the lung. Here, we present a multi-omics study of type 2 diabetes and quantitative blood lipid and lipoprotein traits conducted to date in Hispanic/Latino populations (nmax = 63,184). We conduct a meta-analysis of 16 type 2 diabetes and 19 lipid trait GWAS, identifying 20 genome-wide significant loci for type 2 diabetes, including one novel locus and novel signals at two known loci, based on fine-mapping. We also identify sixty-one genome-wide significant loci across the lipid/lipoprotein traits, including nine novel loci, and novel signals at 19 known loci through fine-mapping. Obesity is a major public health crisis associated with high mortality rates. Previous genome-wide association studies (GWAS) investigating body mass index (BMI) have largely relied on imputed data from European individuals. This study leveraged whole-genome sequencing (WGS) data from 88,873 participants from the Trans-Omics for Precision Medicine (TOPMed) Program, of which 51% were of non-European population groups. Summary Hospital-acquired pressure injuries (HAPIs) have a significant negative impact on patient safety and the quality of care. This doctor of nursing practice (DNP) staff education project focused on improving staff nurses’ knowledge in preventing pressure injuries. Recognizing the high incidence of pressure injuries at the project site, this staff education project addressed the critical need to reduce pressure injuries for at-risk patients by providing ongoing education and training to enhance pressure injury management, reduce hospital costs and length of hospitalization, and improve patient care quality. Intestinal atresias are a common cause of neonatal intestinal obstruction. An extremely rare variant is total intestinal atresia, where no lumen is appreciable from the duodenum through the colon. We describe a case of a 33-week female who was born at an outside hospital and underwent laparotomy for concern for intrauterine perforation and proximal intestinal obstruction. Laparotomy revealed a duodenal perforation and an atresia in the proximal jejunum. Resection of this segment revealed no intestinal lumen distally, and the patient was left in discontinuity and transferred to our tertiary children's hospital.
