Secondary metabolites and extracts from plants have been widely studied and found to possess anti-aging activity. The completed quality-improvement project addressed inconsistent screening and incomplete electronic health record (EHR) documentation of low-income, limited family support, and transportation barriers in a primary care clinic. The nursing practice problem can disrupt access, adherence, chronic disease management, and continuity of care. The practice-focused question examined whether an ADDIE-based education program would improve social determinants of health (SDOH) screening, EHR documentation, staff knowledge and confidence, and referral or follow-up documentation during a 6- to 8-week period. Hypertension is a major modifiable risk factor for cardiovascular morbidity and mortality, yet timely follow-up after elevated blood pressure (BP) readings is not consistently documented in primary care. This Doctor of Nursing Practice (DNP) quality improvement project evaluated whether a standardized hypertension screening follow-up protocol improved provider initiation or documentation of guideline-concordant follow-up within 14 days. The faculty-approved PICOT population was primary care providers in a primary care clinic; we used eligible electronic health record (EHR) encounters as the analytic records to observe provider performance. Substance use is a widespread and complex public health challenge. Financial stress (e.g., unemployment, inadequate housing, transportation access, poor financial literacy) is pervasive among people with substance use disorder (SUD). However, few educational interventions exist to address financial stress for this group. Through the theoretical lens of recovery capital (an asset-based approach that describes the totality of resources that support SUD recovery), we summarize the association between financial stress and substance use, offering implications for Extension educators by highlighting PROFIT, an exemplary program that leverages Extension education as a tool to promote financial recovery capital among people with SUD.

Sertoli cell tumors (SCTs) are sex cord–stromal tumors of the testis that are common in dogs but rare in boars. This study characterized the pathological features of an SCT in a boar and compared its immunohistochemical profile with that of canine SCTs. Testicular tissues from boars (SCT, n = 1; normal testes, n = 2) and dogs (SCT, n = 3; normal testes, n = 5) were evaluated by histopathology and immunohistochemistry for proliferating cell nuclear antigen (PCNA), vimentin, and cytokeratin. Background: Pentosan polysulfate sodium (PPS, Elmiron®) is a widely used medication to reduce pain caused by interstitial cystitis, a bladder disorder most often affecting females. PPS maculopathy is a novel form of toxic retinal disease often not recognized or mistaken for other retinal conditions such as macular degeneration due to the initial similarities on fundus exam. However, when visualized using fundus autofluorescence, affected eyes show characteristic hyper/hypo-autofluorescent patterns.

Background: Outer foveal microdefects (OFMDs) are small focal photoreceptor interruptions that often present with minimal symptoms and are best identified using optical coherence tomography (OCT).

Case Series: This report describes three cases of outer foveal microdefects discovered incidentally during routine eye examinations.

Conclusion: These cases highlight the importance of OCT in detecting subtle macular abnormalities that may be missed on clinical examination.

Method: Seven hundred seventeen students from the San Antonio Northside Independent School District (NISD) received a comprehensive eye examination. The records of these students were retrospectively analyzed to determine the impact of the exam on this population.

BACKGROUND: Urinary tract and kidney infections are potential complications in pediatric chronic kidney disease (CKD), especially among patients with congenital anomalies of the kidney and urinary tract (CAKUT). We characterized the prevalence of self-reported infections from childhood through young adulthood; evaluated risk factors; and quantified the association with CKD progression.

BACKGROUND: Neonates with kidney failure require placement of a peritoneal dialysis catheter (PD-C) shortly after birth. PD-C malfunction and peritonitis, both common in this population, are associated with PD failure. The aim of this study was to describe our center's PD-C-related practices and outcomes in neonates with kidney failure.

METHODS: We performed a retrospective chart review of neonates who required kidney replacement therapy (KRT) within the first 30 days of life from 01/2018-12/2023. We evaluated for PD-C duration, PD-C replacement or revision, and peritonitis.

BACKGROUND: There are few prospective studies of mycophenolate mofetil (MMF) versus cyclophosphamide (CYC) for pediatric lupus nephritis (pLN) and none evaluating rituximab (RTX).

METHODS: The Prospective Pediatric Lupus Nephritis Registry (ProPeL-R) enrolled patients <  21 years within 4 weeks of an initial kidney biopsy diagnostic of pLN. Demographic, clinical, and laboratory data were collected prospectively at enrollment, 3 months, 6 months, and then every 6 months thereafter for up to 5 years of follow-up. For this study, we compared patients receiving initial therapy with corticosteroids (CS) and either MMF (n = 33) vs. CYC (n = 18), and those treated with CS, either MMF or CYC, with RTX (n = 20) vs. without RTX (n = 51).

RESULTS: Histology consisted of 18% class III; 35% class IV; 25% mixed class; and 22% pure class V. Eighty-two percent were female. No significant differences in response or infection rates were identified between those receiving MMF or CYC. BACKGROUND: In-vitro studies suggest that patients on extracorporeal membrane oxygenation (ECMO) may struggle to achieve therapeutic ceftaroline exposure to treat infections due to antibiotic sequestration within the ECMO oxygenator; however, there are no published data regarding ceftaroline concentrations from patients supported by ECMO.

PATIENT PRESENTATION: Here we present an analysis of ceftaroline pharmacokinetics in a 5-y-old patient supported on ECMO due to influenza-associated necrotizing pneumonia.

OBJECTIVE: To evaluate the diagnostic performance of ambulatory blood pressure monitoring (ABPM) thresholds for predicting left ventricular hypertrophy (LVH) in children aged 6-12 years.

Kawasaki disease (KD) and Multisystem Inflammatory Syndrome in Children (MIS-C) are systemic inflammatory diseases sharing clinical and laboratory features. Gastrointestinal (GI) symptoms are common presenting features of both conditions. We aimed to determine the association of GI symptoms and clinical features and outcomes for KD and MIS-C. The International Kawasaki Disease Registry enrolled contemporaneous KD and MIS-C patients <  18 years of age from 42 sites from January 1, 2020, through October 31, 2023. The association of GI symptoms (abdominal pain, vomiting and diarrhea), demographics, clinical and laboratory features, and cardiac involvement were determined using ordinal logistic regression incorporating interaction terms with diagnosis. We included 883 KD and 1827 MIS-C patients with confirmed diagnostic criteria. The presence of at least one GI symptom (vomiting, abdominal pain, diarrhea) was more prevalent for MIS-C patients (88% versus 57%; p <  0.001). A greater number of GI symptoms was significantly associated with older age and higher adiposity, presentation with shock, intensive care unit admission, inotropic support, and greater immunomodulatory therapy. Greater number of GI symptoms was significantly associated with higher peak markers of inflammation and organ dysfunction, and higher peak NT-proBNP and lower left ventricular ejection fraction, but not with coronary artery involvement. There were few differences related to diagnosis group. A greater number of presenting GI features is associated with features that may indicate a more severe form of KD and MIS-C, including cardiac involvement, and may merit increased attention.

College musicians face elevated risk for noise-induced hearing loss (NIHL) due to extensive rehearsal, headphone use, and performance activity, yet most objective sound-exposure studies rely on personal dosimeters that capture only brief windows (1–3 weeks) during the academic year, and overlook headphone-specific exposure. This study leveraged historical Apple Watch and AirPods sound data from participants in the ongoing AMI study (Skoe lab, University of Connecticut) to compare environmental and headphone noise exposure between academic-year and academic-break periods in college-age lifelong, former, and non-musicians.
