IMPORTANCE: The fragmented health care system presents significant challenges to promoting trust with hospitalized patients.

OBJECTIVE: To test the feasibility and perceptions of a digital connection tool for communication among patients, hospitalists, and primary care practitioners (PCPs).

DESIGN, SETTING, AND PARTICIPANTS: This quality improvement study was conducted as a feasibility pilot study from September 2022 to June 2023 on medical floors across 3 hospitals. Participants included adult inpatients, the hospitalists who managed their care, and the patients' PCPs. Data were analyzed from September 2023 to April 2025.

Several "primary atopic disorders" are linked to monogenic defects that attenuate TCR signaling, favoring T helper type 2 (TH2) cell differentiation. Patients with CARD11-associated atopy with dominant interference of NF-κB signaling (CADINS) disease suffer from severe atopy, caused by germline loss-of-function/dominant interfering (LOF/DI) CARD11 variants. The CARD11 scaffold enables TCR-induced activation of NF-κB, mTORC1, and JNK signaling, yet the function of CARD11-dependent JNK signaling in T cells remains nebulous. Here we show that CARD11 is critical for TCR-induced activation of JNK1 and JNK2, as well as canonical JUN/FOS AP-1 family members. Inflammatory bowel disease (IBD), including Crohn's disease and ulcerative colitis, is a chronic inflammatory disorder of the gastrointestinal tract with rising incidence and an unclear etiology. Innate lymphoid cells (ILCs) have recently emerged as key regulators of mucosal immunity and tissue homeostasis and are increasingly implicated in IBD. Unlike adaptive lymphocytes, ILCs do not require antigen recognition and clonal expansion to respond rapidly to environmental cues and shape immune responses. In a healthy gut, ILCs maintain intestinal homeostasis by guarding the epithelial barrier, protecting against pathogens, and mounting proper responses to external insults. Bacterial vaginosis (BV) is a polymicrobial condition of the vaginal microbiota associated with a variety of sexually transmitted infections, infections of maternal and fetal tissues during pregnancy, and even some infections outside of the reproductive tract, including the urinary tract and mouth. BV has also been associated with conditions in which the body generates prominent inflammatory reactions to microbes, including infections of the cervix and other upper genital tract tissues. Although some behavior-analytic studies have shown success with increasing these reports, the use of materials within activities that do not repeat has not been assessed (e.g., Shillingsburg et al., 2017). The purpose of this study was to determine whether different vascular phantoms, with and without a surrogate arm, could serve as effective analogs to the human brachial artery for assessing endothelial dysfunction, an early indicator of cardiovascular disease, particularly atherosclerosis. Atherosclerosis is characterized by plaque buildup and endothelial damage, which precede more severe clinical events and are often detectable through changes in vascular function. A 40” water tower was used to simulate diastolic pressure, enabling water flow through each phantom. This report evaluates Maine's capacity to support effective transitions of care across the substance use disorder (SUD) treatment continuum. Using data collected from a statewide survey of 65 SUD providers, conducted from February to April 2025, the study examines organizational practices, inter-provider communication, and coordination strategies related to transitions between levels of care: medically supervised withdrawal, inpatient, and outpatient treatment programs. Key findings reveal strong intra-organizational communication, widespread understanding of ASAM criteria, and a general commitment to patient-centered care. However, gaps persist in coordination between care levels, particularly in withdrawal management programs, which rarely conduct follow-ups or collaborate with outpatient …

Cerebral venous sinus thrombosis (CVST) is a multifactorial illness seen more commonly in females as compared to males. Its diagnosis is often delayed due to variable clinical presentations. Anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis is a form of autoimmune encephalitis that predominantly affects young individuals and often presents with neuropsychiatric symptoms. It is frequently misdiagnosed as a primary psychiatric disorder due to its initial presentation. Although first-line immunotherapy typically includes corticosteroids, intravenous immunoglobulin (IVIG), or plasmapheresis, a subset of patients shows limited response and requires escalation to second-line agents such as Rituximab. We report two adult male patients, aged 35 and 41 years, who initially presented with behavioral disturbances, confusion, memory deficits, and speech abnormalities. Cyclin-dependent kinase 4 and 6 (CDK4/6) inhibitors, such as abemaciclib, are approved for hormone receptor–positive (HR+) breast cancer, where they inhibit tumor cell cycle progression. While their antiproliferative effects are established, less is known about how they may reflect molecular changes and using cfDNA as a biomarker of treatment response. This study aimed to evaluate the effect of abemaciclib on breast cancer cell proliferation in vitro and characterize cfDNA released during treatment using next-generation sequencing. Health and wellness are foundational to thriving communities, yet many Americans face barriers that impact their access to essential health education and services. This study used a community-based participatory research (CBPR) approach to assess the perceived strengths and needs of health and wellness resources of rural and urban Texas communities. Data were collected through a statewide survey (n = 9,418), targeting key aspects of health and wellness. Background : From 2017-2018 the Michigan Anticoagulation Quality Improvement Initiative (MAQI2) implemented a multicenter quality improvement project to improve guideline concordant aspirin use among warfarin treated patients with atrial fibrillation and/or venous thromboembolic disease. After 48 months of follow-up, this intervention was associated with reduced bleeding but no increase in thrombosis. The cost savings of this intervention has not been assessed. Background : From 2017 – 2018, the Michigan Anticoagulation Quality Improvement Initiative (MAQI2) implemented a multicenter quality improvement project to reduce excess aspirin use among patients on warfarin. Analysis 24 months post-intervention showed significant reductions in aspirin use and the intervention was associated with improved bleeding outcomes. It was unclear if these outcomes would be sustained. Aims : To evaluate pre- and post- intervention aspirin use, bleeding, and thrombotic outcomes, now with up to 84 months of follow-up data.
