Recent advances in artificial intelligence, machine learning, and deep learning are beginning to have an impact on everyday experiences, from natural language processing used in automated telephone call centers to semi-autonomous vehicles. These techniques have also been applied to medical care. In this editorial we discuss applications of AI to medicine and argue for a proactive approach to include rural medicine in this paradigm shift.

The applications of the tetracycline class of antibiotics extends beyond their antimicrobial activity to anti-inflammatory, immunosuppressive and neuroprotective applications making it a commonly used class of medication. Minocycline, a second generation tetracycline, has inherent characteristics that improve absorption and distribution. These benefits promote even more wide spread use. This familiarity of usage breeds prescriptive complacency toward the dermatologic complications including hyperpigmentation. The following case explores these adverse manifestations of Minocycline use.

Wilson’s Disease, a rare autosomal recessive genetic disease, is caused by a mutation in the ATP7B enzyme gene. Without this enzyme, copper builds up in the brain, liver, and cornea causing a multitude of symptoms. It is important to consider Wilson’s disease because the prognosis is dependent on timely diagnosis. This is an interesting case of a 19-year-old male who presented with suicidal thoughts and rapid weight loss. After many months and an extensive work-up, Wilson’s Disease was diagnosed. Hyperthyroidism is a very common endocrine disorder in both humans and cats. Radioactive iodine ablation therapy is considered the gold standard; however, the patient becomes a radiation source and poses a potential radiological risk to others. The regulations governing when the patient can be released differ between human medicine and veterinary medicine. A human patient can receive up to 33 mCi of I-131 and be discharged the same day; yet a feline patient can receive a dose as low as 2 mCi and require multiple days of hospitalization. As with most domains where machine learning methods are applied, correct feature engineering is critical when developing deep learning algorithms for solving the protein folding problem. Unlike the domains such as computer vision and natural language processing, feature engineering is not rigorously studied towards solving the protein folding problem. A recent research has highlighted that input features known as precision matrix are most informative for predicting inter-residue contact map, the key for building three-dimensional models. Selectable markers are widely used in transgenesis and genome editing for selecting engineered cells with a desired genotype but the variety of markers is limited. Here we present split selectable markers that each allow for selection of multiple "unlinked" transgenes in the context of lentivirus-mediated transgenesis as well as CRISPR-Cas-mediated knock-ins. Split marker gene segments fused to protein splicing elements called "inteins" can be separately co-segregated with different transgenic vectors, and rejoin via protein trans-splicing to reconstitute a full-length marker protein in host cells receiving all intended vectors. An Exploration Of Automated Narrative Analysis Via Machine Learning, Sharad Jones, Carly Fox, Sandra Laing Gillam, Ronald B. Gillam

The accuracy of four machine learning methods in predicting narrative macrostructure scores was compared to scores obtained by human raters utilizing a criterion-referenced progress monitoring rubric. The machine learning methods that were explored covered methods that utilized hand-engineered features, as well as those that learn directly from the raw text. The predictive models were trained on a corpus of 414 narratives from a normative sample of school-aged children (5;0-9;11) who were given a standardized measure of narrative proficiency. Performance was measured using Quadratic Weighted Kappa, a metric of inter-rater reliability. The results indicated that one model, BERT, not only achieved …

Increased longevity coupled with age-related declines that compromise driving safety and fragility render older adults as vulnerable road users (Oxley & Whelan, 2008). To address this public health concern, researchers continue to investigate interventions to improve older driver safety. The current dissertation consists of two papers examining: a) the state of the literature on older driver interventions and b) the efficacy of one approach, Useful Field of View cognitive training, to reduce at-fault crash involvement. Intensive efforts are being made to eliminate the raccoon variant of rabies virus (RABV) from the eastern United States and Canada. The United States Department of Agriculture (USDA) Wildlife Services National Rabies Management Program has implemented enhanced rabies surveillance (ERS) to improve case detection across the extent of the raccoon oral rabies vaccination (ORV) management area. We evaluated ERS and public health surveillance data from 2006 to 2017 in three northeastern USA states using a dynamic occupancy modeling approach. Research has shown that instruction in morphological awareness improves students' understanding and use of affixes. However, morphological awareness instruction, typically, is not provided in the general education classroom, and there are few documented collaborations between teachers and speech-language pathologists (SLPs), the latter of whom have a unique expertise in language. BACKGROUND: Patients with relapsed or refractory acute myeloid leukemia (AML) with mutations in the FMS-like tyrosine kinase 3 gene (FLT3) infrequently have a response to salvage chemotherapy. Gilteritinib is an oral, potent, selective FLT3 inhibitor with single-agent activity in relapsed or refractory FLT3-mutated AML.
METHODS: In a phase 3 trial, we randomly assigned adults with relapsed or refractory FLT3-mutated AML in a 2:1 ratio to receive either gilteritinib (at a dose of 120 mg per day) or salvage chemotherapy. Neglected tropical diseases, defined as a group of parasitic diseases affecting developing countries in the tropics (Hotez et al. 2007), are of particular concern because these diseases occur in areas that are also expected to experience rapid population growth and agricultural development in the coming decades. This article explores the role of therapist self-disclosure in clinical settings. Distinctions are made between the enmeshed concepts of privacy, secrecy, and confidentiality to elucidate the role of ignorance in maintaining the power dynamics in therapeutic relationships. While some measure of privacy is essential to counseling practice, secretive behavior (in which the counselor divulges too little about themselves) can have a negative impact on the therapeutic relationship and the client’s therapeutic outcomes. There is, therefore, an under-appreciated and delicate balancing act between withholding information to protect the client and the counselor and revealing enough personal details to empower the client’s …

Unregulated new psychoactive substances (NPS) in “pure” or “preparation” forms are designed to mimic the effects of controlled substances, and are introduced and reintroduced in the market as a cheap substitute of established regulated drugs in quick succession to loophole the law enforcement efforts on combating drugs. For example, carfentanil, a synthetic opioid activates the opioid receptors similar to morphine but it is astoundingly potent compared to a typical drug of abuse (100 times more potent than fentanyl and 10,000 times more powerful than morphine). Copyright © 2019 Billig, Herrmann et al. The functional organization of human auditory cortex can be probed by characterizing responses to various classes of sound at different anatomical locations. Along with histological studies this approach has revealed a primary field in posteromedial Heschl's gyrus (HG) with pronounced induced high-frequency (70-150 Hz) activity and short-latency responses that phase-lock to rapid transient sounds. Low-frequency neural oscillations are also relevant to stimulus processing and information flow, however, their distribution within auditory cortex has not been established.
