Lung cancer, globally the second most common cancer, causes 1.8 million deaths annually with 2 million new cases. It's also one of Finland's deadliest cancers. Chemotherapeutic agents, targeted drugs and radiation therapy have been the mainstay in oncologic treatment for lung cancer patients for decades. Checkpoint inhibitors (e.g. nivolumab, pembrolizumab, atezolizumab) have changed the landscape also in lung cancer. These drugs awake patients own immune system to attack cancer cells. These drugs are widely used in different cancers and they have efficacy also in lung cancer, but only in a minority of patients. Checkpoint inhibitors also induce autoimmune side effects. …

Background: Milk exosomes are widely used to improve the performance of various small macromolecules, oligonucleotides, and imaging agents for delivery and imaging applications. Indocyanine green (ICG) based Near-Infrared (NIR) fluorescent imaging is an attractive and safer technique used for number of clinical applications. However, ICG tend to have poor photostability, short half-life, non-specific proteins binding, and concentration-dependent aggregation. Therefore, there is an unmet clinical need to develop newer modalities to package and deliver ICG. Understanding the true molecular evolution of viruses is critical for preventing future pandemics and developing the next-generation drugs and vaccines. Unfortunately, viruses exhibiting constant genomic substitution rates (GSRs) despite increasing vaccinations and human infections (SARS-CoV-2) and viruses under seemingly extreme conservation (ZIKV) paints a confusing picture of viral evolution, and the main evolutionary theories (Selectionist Theory, Kimura’s Neutral Theory) cannot explain their molecular evolution. Introduction: Syphilis screening, a common medical test in the US, is recommended annually in sexually active women younger than 25, annually for women older than 25, and annually for men(1). Standard tests include rapid reagin tests and Venereal Disease Research Laboratory tests, which react to membrane antigens of Treponema pallidum(2). Case reports of patients with autoimmune conditions have been described (3) but rarely considered in clinical practice. Black men continue to face health disparities due to systemic oppression. Research indicates that mind-body-spirit practices (MBSPs) can effectively improve mental and physical health, thereby reducing health disparities among Black men. However, MBSPs are often White-female-dominated practices, thus putting Black male practitioners at continued risks of under-representation and marginalization in MBSP spaces. OBJECTIVES: To examine changes in MRI rates between Pre-COVID-19 period (Feb-April 2019) and COVID-19 period (Feb-April 2020) among commercially insured patients aged 18-60 with a diagnosis of acute LBP, and to analyze differences in patient characteristics and outcomes between the time periods. Additionally, to examine the impact of eMRI on patient outcomes and health care costs for patients in the COVID-19 period.

DESIGN/METHODS: Using IBM® MarketScan® Commercial Database (MarketScan) we performed a quantitative pre-post comparative retrospective observational study of 117,150 total patients to examine adjusted differences in patient characteristics and rates of MRIs between time periods. Hospital readmissions during pregnancy nationwide impacts maternal health outcomes elevating the risk of maternal deaths after discharge. The accessibility and affordability of necessary prenatal care is integral to the field of health across the lifespan. Socioeconomic barriers within rural locations place expecting mothers at an adverse risk of developing conditions including preeclampsia and other complications that may result in hospital readmission. Bacterial antibiotic persistence is a phenomenon where bacteria are exposed to an antibiotic and the majority of the population dies while a small subset enters a low metabolic, persistent, state and are able to survive. Once the antibiotic is removed the persistent population can resuscitate and continue growing. Several different molecular mechanisms and pathways have been implicated in this phenomenon. A common mechanism that may underly bacterial antibiotic persistence is perturbations in protein synthesis. The purpose of this dissertation was to assess the relationship between childhood trauma and substance use problems as latent constructs among justice-involved adolescents (JIA). Additionally, racial differences and spatial analyses were examined. Previous research underscores the limitations of conceptualizing childhood trauma solely through Adverse Childhood Experiences (ACEs). This justified integrating child welfare involvement, or out-of-home placement, as an additional indicator to develop a comprehensive scope of childhood trauma among JIA. Furthermore, current literature established that childhood trauma is a key predictor of substance misuse among JIA. Alphaviruses are mosquito-transmitted pathogens that induce high levels of viremia, which facilitates dissemination and vector transmission. One prevailing paradigm is that, after skin inoculation, alphavirus-infected resident dendritic cells migrate to the draining lymph node (DLN), facilitating further rounds of infection and dissemination. Here, we assess the contribution of infiltrating myeloid cells to alphavirus spread. We observe two phases of virus transport to the DLN, one that occurs starting at 1 h post infection and precedes viral replication, and a second that requires replication in the skin, enabling transit to the bloodstream. Older Adults Are At Greater Risk Of Opioid Use Disorder In Communities With High Social Vulnerability, Tse-Chuan Yang, Seulki Kim, Stephen A. Matthews, Carla Shoff

Population Health Research Brief Series

The prevalence of opioid use disorder (OUD) among older adults has increased and imposes a heavy burden on the health care system in the United States. This brief summarizes findings from a study that used data from the Centers for Medicare and Medicaid Services to examine how county characteristics contributed to the prevalence of OUD among Medicare beneficiaries age 65+ in U.S. counties in 2021. They found that the prevalence of OUD among these adults is higher in counties with more place-level social vulnerability (e.g., socioeconomic disadvantage, high housing cost burden).

Ruxolitinib reduces spleen volume, improves symptoms, and increases survival in patients with intermediate- or high-risk myelofibrosis. However, suboptimal response may occur, potentially because of signaling via the phosphoinositide 3-kinase (PI3K)/protein kinase B pathway. This phase 2 study evaluated dosing, efficacy, and safety of add-on PI3Kδ inhibitor parsaclisib for patients with primary or secondary myelofibrosis with suboptimal response to ruxolitinib. The WHO-recommended essential package of care (EPC) for filarial limb lymphedema consists of daily limb washing, entry lesion management, limb protection, exercises, and elevation. Decongestive therapy (DT) with compression bandaging by trained lymphedema therapists adds additional benefit but is unavailable for most in low- and middle-income countries (LMICs). To determine whether DT using self-adjustable, short-stretch compression garments (SSCG), prefitted using portable, three-dimensional infrared imaging (3DII), would be effective and feasible in LMIC settings, we conducted a pilot 6-week, interventional, single-group, open-label pilot study in Galle, Sri Lanka. This qualitative study was to determine clinical supervisors' perceptions of graduate students’ skill set of articulation disorders compared to motor speech disorders. Participants in this study included SLP clinical supervisors of Master’s level graduate students in Communication Disorders across various pediatric settings, including private practice, school-based or outpatient clinics in north central Arkansas. Face-to-face/phone interviews were recorded with a digital recorder and transcribed using the speech-to-text transcription service at Temi.com. The purpose of this research was to investigate if there was a relationship between hearing loss and length of employment in the steel industry. Participants included 11 males that worked in the steel industry. Participants completed questionnaires regarding demographics, employment, and noise exposure, as well as took part in an audiological exam. The results of the audiological evaluations showed that 45% of participants had hearing loss in the right ear, 36% of participants had hearing loss in the left ear, and 36% of participants had loss in both ears. Previous studies have shown the clinical benefit of rechallenging the RAF pathway in melanoma patients previously treated with BRAF inhibitors. 44 patients with multiple tumors harboring RAF alterations were rechallenged with a second RAF inhibitor, either as monotherapy or in combination with other therapies, after prior therapy with a first RAF inhibitor. This retrospective observational study results showed that rechallenging with RAFi(s) led to an overall response rate of 18.1% [PR in thyroid (1 anaplastic; 3 papillary), 1 ovarian, 2 melanoma, 1 cholangiocarcinoma, and 1 anaplastic astrocytoma]. The Veterans Affairs primary care delivery model uses health coaching as a catalyst to improve health behaviors in veterans. Health coaching is a complex skill that requires specialized training, repetitive practice, and professional confidence. Albert Bandura believed most people acquire skills and develop the confidence to use those skills through social learning. Neurological disorders are the leading cause of cognitive and physical disability worldwide, affecting 15% of the global population. Due to the demographics of aging, the prevalence of neurological disorders, including neurodegenerative diseases, will double over the next two decades. Unfortunately, while available therapies provide symptomatic relief for cognitive and motor impairment, there is an urgent unmet need to develop disease-modifying therapies that slow the rate of pathological progression. Following the authorization of the use of COVID-19 vaccines in babies age 6 months through children 4 years old in the United States, some individuals (parents, pediatricians, and communicators) framed COVID-19 vac-cination as an issue of access, while many others expressed hesitancy, and some resisted recommendations from the US Centers for Disease Control and Prevention. Amino acid formula (AAF) is increasingly consumed in infants with cow's milk protein allergy; however, the long-term influences on health are less described. In this study, we established a mouse model by subjecting neonatal mice to an amino acid diet (AAD) to mimic the feeding regimen of infants on AAF. Surprisingly, AAD-fed mice exhibited dysbiotic microbiota and increased neuronal activity in both the intestine and brain, as well as gastrointestinal peristalsis disorders and depressive-like behavior. Furthermore, fecal microbiota transplantation from AAD-fed mice or AAF-fed infants to recipient mice led to elevated neuronal activations and exacerbated depressive-like behaviors compared to that …

Virologic suppression is an essential part to a successful regimen for Persons with HIV (PWH) and the 95-95-95 initiative. Yet, PWH experiencing episodes of low-level viremia (LLV) may be more prone to experience virological failure (VF). Currently, studies debate the clinical relevance of LLV and its effect on virological outcomes, but none have explored its interconnectedness with drug resistant mutations (DRM) and antiretroviral treatment (ART) adherence – all integral elements in predicting virological outcomes.

Background: Community based surveillance (CBS) is the organized and rapid collection of information from health events identified in the community and includes the systematic detection and reporting of events of public health significance. While used globally to enhance the detection of priority diseases, CBS has not been routinely deployed in Iraq, where a once-robust health system has been devastated by decades of war, international sanctions, and large-scale population displacement. Importance: Inguinal hernia repair in preterm infants is common and is associated with considerable morbidity. Whether the inguinal hernia should be repaired prior to or after discharge from the neonatal intensive care unit is controversial.

Objective: To evaluate the safety of early vs late surgical repair for preterm infants with an inguinal hernia.

Design, setting, and participants: A multicenter randomized clinical trial including preterm infants with inguinal hernia diagnosed during initial hospitalization was conducted between September 2013 and April 2021 at 39 US hospitals. Follow-up was completed on January 3, 2023.

Maximizing the potential of human liver organoids (LOs) for modeling human septic liver requires the integration of innate immune cells, particularly resident macrophage Kupffer cells. In this study, we present a strategy to generate LOs containing Kupffer cells (KuLOs) by recapitulating fetal liver hematopoiesis using human induced pluripotent stem cell (hiPSC)-derived erythro-myeloid progenitors (EMPs), the origin of tissue-resident macrophages, and hiPSC-derived LOs. Remarkably, LOs actively promote EMP hematopoiesis toward myeloid and erythroid lineages. Moreover, supplementing with macrophage colony-stimulating factor (M-CSF) proves crucial in sustaining the hematopoietic population during the establishment of KuLOs. Non-invasive early cancer diagnosis remains challenging due to the low sensitivity and specificity of current diagnostic approaches. Exosomes are membrane-bound nanovesicles secreted by all cells that contain DNA, RNA, and proteins that are representative of the parent cells. This property, along with the abundance of exosomes in biological fluids makes them compelling candidates as biomarkers. However, a rapid and flexible exosome-based diagnostic method to distinguish human cancers across cancer types in diverse biological fluids is yet to be defined.
