BACKGROUND: Tonic and atonic "drop attack" seizures are a classic and morbid semiology in Lennox-Gastaut syndrome, resulting in frequent injuries and emergency room visits, in addition to neurocognitive sequelae. Recent years have seen a growing interest in less invasive techniques for performing the classic surgical treatment for drop attacks in Lennox-Gastaut syndrome, that is, corpus callosotomy.
OBSERVATIONS: A 5-year-old boy with Lennox-Gastaut syndrome presented for surgical evaluation. He experienced up to 20 daily tonic seizures despite multiple antiseizure medications. Preoperative imaging revealed highly abnormal anatomy with severe ventriculomegaly and thinning of the cortex and corpus callosum. Open microsurgery or an …

TRIM44, a tripartite motif (TRIM) family member, is pivotal in linking the ubiquitin-proteasome system (UPS) to autophagy in multiple myeloma (MM). However, its prognostic impact and therapeutic potential remain underexplored. Here, we report that TRIM44 overexpression is associated with poor prognosis in a Multiple Myeloma Research Foundation (MMRF) cohort of 858 patients, persisting across primary and recurrent MM cases. TRIM44 expression notably increases in advanced MM stages, indicating its potential role in disease progression. Single-cell RNA sequencing across MM stages showed significant TRIM44 upregulation in smoldering MM (SMM) and MM compared to normal bone marrow, especially in patients with t(4;14) …

INTRODUCTION: The people who inject drugs (PWID) are attributed to high-risk groups for transmission of the Hepatitis C virus (HCV). This study assessed the prevalence and associated factors of current HCV infection (CHI) among U.S. general population and PWID of ages between 20 and 59 years old. METHODS: This study utilized cross-sectional data from the 2009-2018 National Health and Nutrition Examination Survey, conducting separate analyses for the U.S. general population, including PWID and non-PWID, as well as specific analyses focusing solely on PWID. The interplay between asthma and glucose metabolism disorders, such as hyperglycemia, has gained increasing attention due to the potential exacerbation of asthma symptoms and severity. This review explores the complex relationship between hyperglycemia and asthma, emphasizing the pathophysiological links, the impact of glucose metabolism disorders on asthma, and the effects of asthma medications on glucose levels. Hyperglycemia, often induced by asthma treatments like corticosteroids, has been associated with an increased risk of asthma exacerbations. AIMS: Data on mitral annular disjunction (MAD) in children with Marfan syndrome (MFS) are sparse. To investigate the diagnostic yield of MAD by echocardiography and cardiac magnetic resonance imaging (CMR), its prevalence and progression during childhood.
METHODS AND RESULTS: We included patientsMFS, defined by 2010 Ghent criteria and a pathogenic FBN1 variant or ectopia lentis. Two readers measured systolic separation between the mitral valve (MV) posterior hinge point and left ventricular (LV) myocardium on initial and subsequent imaging. MAD was defined as MV-LV separation ≥2 mm, MV prolapse (MVP) as atrial displacement ≥2 mm. Kappa coefficients evaluated echocardiogram-CMR agreement. Bland-Altman …

BACKGROUND: Prominent product placement is a core promotional tactic in retail food environments. How this practice has been adapted for online supermarkets, and the extent to which it is applied to healthier and less healthy food products in this setting, is largely unknown. We aimed to investigate placement-type promotions of food products in Australian online supermarkets.

Alcohol use is an independent risk factor for the development of bacterial pneumonia due, in part, to impaired mucus-facilitated clearance, macrophage phagocytosis, and recruitment of neutrophils. Alcohol consumption is also known to reduce peripheral natural killer (NK) cell numbers and compromise NK cell cytolytic activity, especially NK cells with a mature phenotype. However, the role of innate lymphocytes, such as NK cells during host defense against alcohol-associated bacterial pneumonia is essentially unknown. OBJECTIVES: To construct a new scoring system utilizing biomarkers, vitals, and imaging data to predict 30-day mortality in acute pulmonary embolism (PE).
BACKGROUND: Acute PE, a well-known manifestation of venous thromboembolic disease, is responsible for over 100,000 deaths worldwide yearly. Recent advances in spatial transcriptomics (ST) techniques provide valuable insights into cellular interactions within the tumor microenvironment (TME). However, most analytical tools lack consideration of histological features and rely on matched single-cell RNA sequencing data, limiting their effectiveness in TME studies. To address this, we introduce the Morphology-Enhanced Spatial Transcriptome Analysis Integrator (METI), an end-to-end framework that maps cancer cells and TME components, stratifies cell types and states, and analyzes cell co-localization. By integrating spatial transcriptomics, cell morphology, and curated gene signatures, METI enhances our understanding of the molecular landscape and cellular interactions within the tissue. This course is an introduction to the social, behavioral, and commercial factors that influence health. Theoretical and methodological approaches to the study of these determinants will be discussed from a social ecological perspective. Social determinants cover five domains: economic stability, education access and quality, health care access and quality, neighborhood and built environment, and social and community context. Behavioral determinants include such factors as smoking, physical activity, and alcohol use. Commercial determinants include economic and political domains with examples such as cigarette marketing and the placement of toxic waste sites in low-income communities.

Rural cervical cancer survivors, or individuals with a form of cancer that originated in the cervix of the reproductive system, may be uniquely impacted by socially constructed gender roles. The present study utilized the term “females” to describe those who self-identify as having a cervix, which may include cisgender, transgender, intersex, and non-binary gender identities. Infertility is a distressing experience for those diagnosed and often exacerbated by engagement with biomedical treatments. This thesis is an exploration of infertility “journeys” posted on YouTube. Through a narrative analysis of 8 YouTube channels, several key themes were identified, specifically embodiment of the infertility label, barriers to treatment, and agency. This work uses the mindful body framework to look at how the individual body, social body, and body politic shape and reinforce these themes in the context of biomedical infertility treatment. This thesis systematically optimizes and compares state-of-the-art supervised classification models for Louisiana Medicaid data targeting clinical services, COVID-19 infection, and tobacco use. These target variables are critically important as they represent key health outcomes and behaviors among Medicaid enrollees in Louisiana, a population often characterized by poverty and limited access to education. This study applies advanced machine learning techniques to identify the best model for multinomial and binary classification tasks. This article explores the influence of Electronic Word of Mouth (e-WoM) and hospital image on hospital patient loyalty, emphasizing the mediating role of patient commitment. Amidst the development of the healthcare industry sector in Indonesia, this study highlighted that e-WoM and hospital image significantly enhanced patient loyalty, with patient commitment playing an important mediating role. This article suggested practical strategies for hospitals, such as improving the quality of healthcare services, leveraging positive e-WoM, and implementing loyalty programs to elevate patient commitment and loyalty. Oral nutritional supplements (ONS) are nutrient-dense beverages that aid in meeting the nutrient needs of patients. Recent research has indicated that patient adherence to prescribed nutritional supplements may depend on palatability, tolerance, and taste fatigue. Registered Dietitian Nutritionists (RDNs) are healthcare professionals who may prescribe these supplements directly or make recommendations to the patient’s physician for a prescription, thereby influencing patient adherence. Twenty-five percent of people experiencing homelessness have pets, but few transitional housing programs allow pets (Rhoades et al., 2015); therefore, there is a gap in the literature about the experience of pet ownership during the transitional housing process. This study aimed to explore pet owners' experiences at a transitional housing site in San Diego County. The student interviewed two current dog owners and four recent dog owners. The capacity to initiate actions endogenously is critical for goal-directed behavior. Spontaneous voluntary actions are typically preceded by slow-ramping activity in medial frontal cortex that begins around two seconds before movement, which may reflect spontaneous fluctuations that influence action timing. However, the mechanisms by which these slow ramping signals emerge from single-neuron and network dynamics remain poorly understood. Here, we developed a spiking neural-network model that produces spontaneous slow ramping activity in single neurons and population activity with onsets ~2 s before threshold crossings. Chronic kidney disease (CKD) is a complex disease that poses various challenges to patients. Health Literacy (HL) is considered essential for the effective management of CKD. This phenomenological study explored health literacy in 12 patients with chronic kidney disease undergoing hemodialysis in Padang, Indonesia. Semi-structured individual interviews were conducted, and the data were analysed and interpreted using the hermeneutical phenomenological approach of Diekelmann. Consensual non-monogamy (CNM) and polyamory are types of relationship structures that differ in many ways from monogamy. CNM is becoming more prevalent (Moors, 2023; Scoats & Campbell, 2022). In the United States and Canada, 1 in 5 people have engaged in some form of CNM (Moors, 2023). Background: Both inflammation and insufficient physical inactivity contribute to individual-level risk of disease recurrence and death in stage III colon cancer. The extent to which increased inflammatory risk can be offset by sufficient physical activity remains unknown. Methods: This cohort study was nested within the CALGB/SWOG 80702 (Alliance) randomized trial. Inflammatory burden was quantified by high-sensitivity C-reactive protein, interleukin-6, and soluble tumor necrosis factor-α receptor 2 after recovery from tumor resection. Physical activity was measured during and after postoperative chemotherapy. The primary endpoint was disease-free survival. In two studies, this dissertation evaluated relations among early childhood education (ECE) teachers’ well-being and embedded social-emotional services. Study 1 focused on occupational stress, burnout, and mental health, exploring the relations among stress and mental health, and the associations with social-emotional services. Study 1 also explored group differences in stress, burnout, mental health, and social-emotional services for teachers who identified as Latinx. Results showed that occupational stress and social emotional services had significant main effects on mental health (burnout, depression, anxiety), with associations varying across mental health measures. Occupational stress was associated with adverse mental health outcomes, while social-emotional support …

Recently, downward trends have been reported in U.S. children’s math performance following school disruptions during COVID-19 amidst longstanding concerns for instruction and curricula within the subject. In support of efforts to remedy these declines, the current work presents two studies dedicated to identifying cognitive factors that are most strongly related to math performance, and which therefore offer promising potential targets for assessment and intervention. Phlebotomine sand flies are the vectors of leishmaniasis, a neglected tropical disease. High-quality reference genomes are an important tool for understanding the biology and eco-evolutionary dynamics underpinning disease epidemiology. Previous leishmaniasis vector reference sequences were limited by sequencing technologies available at the time and inadequate for high-resolution genomic inquiry. Here, we present updated reference assemblies of two sand flies, Phlebotomus papatasi and Lutzomyia longipalpis. These chromosome-level assemblies were generated using an ultra-low input library protocol, PacBio HiFi long reads, and Hi-C technology. Objectives: This paper explores parent coaching experiences supporting parents of young children newly diagnosed with type 1 diabetes in a clinical trial.
Methods: In a trial for 157 parents, those in the intervention arm (n = 116) were paired with a parent coach (n = 37; Mage = 37.9 years, SD = 3.9; 94.6% mothers, 81.1% White non-Hispanic). Parent coaches provided diabetes-specific social support. Parent coaches completed monthly surveys and satisfaction/feasibility surveys, with a subset (n = 7) undergoing qualitative interviews at the end of this study.
Results: There were 2262 contacts between participants and their parent coaches, averaging …

Objectives:
Atrial fibrillation after coronary artery bypass grafting is a relatively well known complication that has been observed for a long time. Though the management and drugs in the perioperative period have changed, their impact on the generation of postoperative atrial fibrillation remains unclear. Therefore, we investigated various perioperative management methods and the occurrence of postoperative atrial fibrillation.
Methods
