Purpose

The purpose of this scholarly project was to improve discharges by 11:00 a.m. within the Monitored Pod Units located in a regional hospital in Bryan, Texas.

Background

The current discharge rate by 1100 for the monitored pod units at the regional health center was below the health system goal of 50%. The impacts of delayed discharges are broad and can be seen throughout the regional health center. Delayed discharges have a direct effect on staff satisfaction, patient satisfaction, and decreased revenue.

This quality improvement (QI) project aimed to reduce hospital-acquired pressure injuries (HAPIs) by 10% in an intensive care unit through the implementation of a skin risk assessment form and a visual reminder system.

Hospital-acquired pressure injuries negatively impact patient outcomes and can increase costs associated with a hospital stay. Studies have shown that visual reminders can improve nursing intervention compliance and correlate with improved patient outcomes.

To test for sex differences in the impact of cannabis use on decision-making and brain correlates, we employed cortical thickness (CT) analysis of brain regions involved in intertemporal decision-making namely bilateral orbitofrontal cortex(OFC) and insula in young adult nondependent cannabis-users(CU) and non-users(NU) and their scores on delay discounting task. Neuroimaging analyzes of previously collected data were performed on 608CU and 503NU. CT analysis was performed on MRI images. OFC and insula thickness, scores on the delay discounting test were compared between groups and correlated. Controlling alcohol-use and intra-cranial-volume, CU exhibited sex differences in CT. This thesis presents the design, implementation, and mixed-methods evaluation of Systems Simulation, a reusable mixed reality (MR) application intended to help undergraduate nursing students explore human anatomy and pathophysiology using immersive 3D visualization.

This study examined the impact of a three-week Osher Lifelong Learning Institute (OLLI) health course, “Far Out” Medicine: The Science and History of Psychedelics, on midlife and older adult learners’ knowledge, attitudes, and perspectives regarding psychedelic and entheogenic substances. Eleven midlife and older adults (ages 40 - 80; M = 59.0, SD = 15.35) completed matched pre- and post-course surveys assessing demographic characteristics, factual knowledge, attitudes, personal willingness, and social or policy views. In rock climbing, climbers use their arms to regulate their posture on the wall, which can lead to localised muscle fatigue. Evidence shows fatigue is the primary cause of falls, but little is known about how fatigue specifically affects climbing rhythm and hand movements. The present study examined climbing fluidity and hand movements on an indoor climbing wall before and after a specific fatiguing protocol. Seventeen climbers completed three repetitions of a challenging climbing route (21 on Ewbank scale) with different levels of localised arm fatigue. Background: Diagnoses of labor dystocia, and subsequent labor augmentation, make one of the biggest contributions to childbirth medicalization, which remains a key challenge in contemporary maternity care. However, labor dystocia is poorly defined, and the antithetical concept of physiological plateaus remains insufficiently explored. Aim: To generate a definition of physiological plateaus as a basis for further research. Limited research has reported the reliability of rapid force generation characteristics during isometric assessments of the hamstrings. Therefore, the purpose of the present study was to determine the between-session reliability of rapid force generating characteristics of the hamstrings and relationship to maximal force production. Executive Summary

The SheSpeaks project successfully transformed how 223 Western Australian women understand and practice self-care, demonstrating  that targeted wellbeing literacy interventions can create lasting change  in women’s relationship with their own wellbeing. Funded by the Women’s Grants for a Stronger Future program, this innovative initiative combined evidence-based workshops with authentic podcasting to challenge the pervasive cultural narrative that positions women’s self-care as selfish  or impossible.

Professional reasoning, theory, and trial and error interventions can be demonstrated in case studies. Case studies are usually relegated to the lowest level in evidence hierarchies. Background: A breast cancer diagnosis can significantly impact an individual’s quality of life. This retrospective chart review explored the occupational therapy evaluation process with female patients in an inpatient acute care hospital with a breast cancer diagnosis.

Methods: This descriptive retrospective chart review analyzed 134 occupational therapy evaluations of female patients with breast cancer admitted to a southeastern US academic medical center. Data were extracted and categorized using the Occupational Therapy Practice Framework.

Objective: To explore the learning experiences of parents of children with sensory over-responsivity (SOR).

Methods: An embedded mixed-methods design was used. Sixteen parents of children with SOR, aged 3–8 years, were recruited using purposive sampling. All parents participated in group trainings; of these, nine participated in one-on-one interviews. Background: Vestibular dysfunction affects 15-20% of adults annually, often impacting daily activities and quality of life (Neuhauser, 2016). While occupational therapists and occupational therapy assistants (OTs/OTAs) can address functional limitations caused by vestibular disorders, their role in vestibular rehabilitation (VR) is under-studied.

Objective: This study explored the competence and confidence of occupational therapy practitioners (OTP) in VR and identified barriers to implementation.

Background: Wicked problems in healthcare, such as the complex health condition of head injury, demand interprofessional collaboration that transcends traditional disciplinary boundaries. These complex clinical challenges call for theoretical frameworks that integrate both disciplinary expertise and interdisciplinary processes.

Definitions and Concepts: Grounded in Repko and Szostak’s (2017) framework of interdisciplinary studies, this paper integrates the Science of Team Science (SciTS), the Interprofessional Education Collaborative (IPEC) competencies, and TeamSTEPPS® to construct a novel heuristic that informs the design of an interprofessional education (IPE) and practice model.

Gaps in HIV RNA monitoring persist globally impeding the ability to determine clinical progress and outcomes. This study systematically evaluated provider (e.g., guideline non-adherence), system (e.g., laboratory error) and participant-level (e.g., refusal) drivers of missed viral load (VL) monitoring measurements among people with HIV in Kenya. Adults aged 18-65 years were followed across five health facilities in Kenya as part of a clinical trial (NCT#02338739) where HIV RNA monitoring was done routinely. Objectives: To determine if psychological factors, such as anxiety, depression, fear of movement and fear of rupture are associated with increased tendon-related disability, quantified by the Tendinopathy Severity Assessment-Achilles (TENDINS-A). Design: Cross-sectional. Setting: Online Qualtrics survey. Participants: Sixty-eight participants (54% female) with Achilles tendinopathy and a mean (standard deviation) age of 40.1 (12.6) years. INTRODUCTION: Melanopsin is a photopigment with roles in mediating sleep and circadian-related processes, which are often disrupted in Alzheimer's disease (AD). Melanopsin also impacts cognition and synaptogenesis. This study investigated the associations between melanopsin genetic variants, sleep, and markers of brain health. METHODS: Linear regression analyses examined the relationship of single-nucleotide polymorphisms (SNPs) within the melanopsin gene (OPN4), with cortical amyloid beta (Aβ), cognition, brain volumes, and self-reported sleep traits in cognitively unimpaired older adults. Neoadjuvant immunotherapies have shown antitumor activity in melanoma. Substudy 02C of the global, rolling-arm, phase 1/2, adaptive-design KEYMAKER-U02 trial is evaluating neoadjuvant pembrolizumab (anti-PD-1) alone or in combination, followed by adjuvant pembrolizumab, for stage IIIB–D melanoma. Here we report results from the first three arms: pembrolizumab plus vibostolimab (anti-TIGIT), pembrolizumab plus gebasaxturev (coxsackievirus A21) and pembrolizumab monotherapy. Pathologic complete responses occurred in 10 of 26 patients (38%) with pembrolizumab plus vibostolimab, 7 of 25 (28%) with pembrolizumab plus gebasaxturev and 6 of 15 (40%) with pembrolizumab monotherapy. Approximately 50% of all adults with heart failure (HF) are classified as frail. Frailty is a clinical state of 'accelerated ageing' that complicates management and results in adverse health outcomes. Despite recommendations for frailty assessment in HF guidelines, its implementation into routine clinical practice has been slow. Further, evidence to inform models of care and pharmacological treatment for individuals with HF who are classified as frail is lacking. The complexity of management underscores the importance of tailoring models of care that can improve the focus on frailty through multidisciplinary care teams. Background: Our knowledge of the importance of flavonoid-rich foods in preventing unhealthy aging across its different domains is limited. Objectives: This study aimed to examine prospective associations between flavonoid-rich food and flavonoid intakes and indicators of unhealthy aging, namely frailty, impaired physical function, and poor mental health. Abdominal aortic calcification (AAC), a subclinical measure of cardiovascular disease (CVD) that can be assessed on vertebral fracture assessment (VFA) images during osteoporosis screening, is reported to be a falls risk factor. A limitation to incorporating AAC clinically is that its scoring requires trained experts and is time-consuming. We examined if our machine learning (ML) algorithm for AAC (ML-AAC24) is associated with a higher fall-associated hospitalisation risk in the Manitoba Bone Mineral Density (BMD) Registry.
