In conclusion I would like to stress that these observations do not constitute direct evidence of the mechanism whereby the nitrates relieve angina pectoris. It is probable that they are capable of causing some reduction of myocardial oxygen consumption. However, there is increasing indirect evidence which suggests that the site of action of these drugs is principally on the large coronary arteries. Their administration may thus relieve coronary spasm, if this ever causes angina, and more importantly will increase collateral flow into ischemic areas of myocardium when collateral channels have become enlarged. I will use two recent cases to illustrate the following four points: 1. That high doses of irradiation, though not a desirable prelude to surgery, are not an absolute contraindication. 2. That the final criterion of radiation response is not measured by exfoliative cytology, but by the length of time the patient lives without further evidence of tumour formation. 3. That planned combined therapy ab initio is less traumatic for both patient and surgeon than salvage surgery after failed irradiation. 4. That teamwork involving radiotherapist and surgeon is essential for success.

A number of years ago I decided to attempt to measure the intracellular pH of skeletal muscle by a more direct method than those in current use, that is, by an electrode. I excluded all forms of pH electrodes except the glass electrode and then set up a certain number of characteristics or requirements that this electrode should have. First, the electrode should be insulated in such a manner that only the pH-sensitive portion is in the cell cytoplasm. All in all, the various methods, surgical with or without lymphadenectomy and irradiation, including supervolt therapy, permit marked individualization according to the age of the patient, her general condition, her operability, and according to the histological findings. It may be said for all methods that the best results are still obtained in the early stages of carcinoma. By surgery as well as irradiation, 90% or more permanent cures may be achieved. This, in view of the nature of cancerous disease, is an important fact.

Results Of 600 Wertheim Operations For Cancer Of The Cervix, Otto Käser

The 600 Wertheim cases are composed of three series; the first consisting of 405 cases (1950 to 1963) from Frankfurt under the chairmanship of Professor Naujoks; the second consisting of 85 cases (1953 to 1962) from St. Gall, Switzerland, where I was at the time head of the department; and the third consisting of 110 cases (1963-1965) from Frankfurt. In these 600 cases neither the technique nor the indications were identical. Before 1958, lymphadenectomy was not used in Frankfurt; however, since then it has been an essential part of the procedure, and has been used in all cases from St. …

I would like to conclude by citing two rather interesting observations we have made. Three-quarters of all the patients in all our surveillance who have allergic reactions to drugs have had peptic ulcer, ulcerative colitis, or neoplastic disease of the gastrointestinal tract. Of these patients, three-quarters of the drugs producing allergic reactions are administered orally. The rates of allergic reactions to individual drugs in patients with gastrointestinal disease as opposed to those without gastrointestinal disease receiving the same medication, are significantly increased. In conclusion, the antimetabolites have proven extremely interesting in the laboratory for the exploration of the mechanism of antibody synthesis and related problems. They have also proven to be extremely useful and interesting materials in the clinic. Whether they are going to replace any other standard forms of therapy, such as the corticosteroids, is, in my view, doubtful. Their use at the moment is experimental and their ultimate place in clinical medicine is by no means settled. They appear to have two important effects in man: 1) Suppression of antibody formation which can be, in many individuals, selective; and 2) …

It was the development of radar and sonar during the last World War which made the ultrasonic techniques I am going to talk about possible. Following the War, Dr. K. T. Dussik had the idea that, if one directed ultrasonic waves into the body, one might obtain echoes reflected from tissue surfaces in the same way that the Navy received echo information reflected from the submarine's hull. Continuing Education For What?, George E. Miller

Continuing education should mean continuing self-education, not continuing instruction. If this desirable goal is to be accomplished, there must be movement away from the content model, which encourages dependence upon teachers, to a process model, which demands a significant measure of self-reliance--a shift away from preoccupation with courses and methods, toward an augmented concern for educational diagnosis and individualized therapy. It does not mean an immediate abandonment of present program forms, but it is likely to be accompanied by a slow erosion of the faith which presently supports them.

Much of the background to the material present here has been reviewed extensively in the not too distant past (Jӧbsis '64), and I will take the liberty of letting that article provide most of the detailed justification of my remarks today.
