Clinical Experience With Radioisotopic Powered Cardiac Pacemakers, Nicholas P. D. Smyth, Tomas Hernandez, Alvin Johnson

Henry Ford Hospital Medical Journal

Significant increase in the useful lifetime of the implantable cardiac pacemaker has been made possible by the development of a radioisotope power source. This paper reports experience with two models, the AEC-ARCO Nu-5 (fixed rate) and the Medtronic Model 9000 (ventricular inhibited demand). Five of the former models were implanted in 1973, and six of the latter more recently. Both types of units have functioned well.

The Relationship Of The Cold Nodule To Carcinoma Of The Thyroid, Douglas M. Evans

Fifty-two cases with histologically proven carcinoma of the thyroid were studied. With some variation as to the expected incidence of the several cell types of tumor, possibly due to the small size of this series, the distribution of these cases encompassed all histologic types. Fifty-three (53%) percent of the cases where pre-operative I131 scans were recorded showed a positive, or "cold" scan. Fifty-five (55%) percent of all lesions showed extension of disease beyond the confines of the thyroid gland. This experience indicates that, although thyroid scintiscans provide useful information to be correlated with other data, there are distinct limitations to …

Colon Interposition In The Management Of Total Gastric And Esophageal Stricture Following Lye Ingestion, Walter H. Janke, Ing-Sei Hwang

A case is presented in which ingestion of 300 CCS of liquid lye as a suicidal attempt resulted in caustic esophagogastritis, followed by a fibrotic process involving the entire esophagus and stomach down to the first portion of the duodenum. Successful colon bypass was performed from the cervical esophagus to the second portion of the duodenum.

Over a two-year period, 30 patients with chronic pancreatitis have been observed by the author. Thirteen had surgical treatment. Three had laparotomy and drainage only when recurrence of acute inflammation was encountered. One had excision of a cyst of the tail of the pancreas. Another had drainage of a retroperitoneal necrotic collection. A patient with duodenal stenosis from scarring of the head of the pancreas was treated by gastro-jejunostomy. Seven patients had subtotal pancreatectomies. There was no mortality in the latter group, and six are asymptomatic. Vagotomy With Pyloroplasty Or Antrectomy: A Comparison Of Results In The Treatment Of Duodenal Ulcer, Richard L. Collier, John H. Wylie Jr.

During the years 1970-73, 776 operations were performed for the relief of duodenal ulcer. Truncal vagotomy was done in all cases, and to this was added antrectomy in 92 cases and pyloroplasty in 24 cases. In the followup, three-fourths of the patients in both groups said they were satisfied with the result. There were four recurrences in the smaller pyloroplasty group. In this series, antrectomy with vagotomy appears to be the best operation for duodenal ulcer disease.

A case is reported of malignant lymphoma of the small and large bowel associated with ulcerative colitis. Primary lymphoma of the gastrointestinal tract is relatively uncommon. The dual association with colitis is rare. Prior to laparotomy, diagnosis of lymphoma is difficult because, of the many possible presenting complaints, none are specific. Treatment is basically surgical excision with postoperative radiation.

Of 23 patients with carcinomas less than two centimeters in diameter located in the rectum and distal sigmoid colon, evidence of invasion was present in seven. Resections were performed for six patients, with two eventually dying from the carcinoma. Individualized management should consider the presence or absence of invasive carcinoma, appropriate resection for invasive carcinoma, biopsy of the base of the lesion if feasible when observation is planned, repeated sigmoidoscopic examinations for noninvasive carcinoma, and later resection if recurrence with invasion develops. Metabolic Capabilities Of Sulfur Oxidizing Bacteria And Their Role In Water Pollution, M. I. H. Aleem

This report describes investigations into the physiology of microorganisms that are commonly involved in the oxidation of inorganic sulfur compounds. The metabolic activities of these bacteria play a potent role in several fields of economic importance such as strip mining operations, water pollution, corrosion, metallurgy, petroleum technology and soil fertility processes.

The oxidation of inorganic sulfur compounds was studied in the chemolithotrophs Thiobacillus denitrificans, Thiobacillus A2, Thiobacillus neapolitanus, and a photolithotroph Rhodopseudomonas palustris. Cell suspensions from all of the thiobacilli catalyzed completely the oxidation of sulfide, thiosulfate, and sulfite to sulfate. The oxidation of thiosulfate in R. palustris …

Among 143 women whose menarcheal age was documented during a longitudinal growth study, recall 39 years after the even gave the following results: menarche was recalled as 0.2 years after earlier than the actual date (p<0.05), the standard deviation of recalled age was 0.3 years larger (p<0.01), and the coefficient of correlation, r, between actual and recalled age was 0.60 +/- s.e. 0.05.

In this paper an attempt is made to determine what factors propel given individuals toward self destruction. Durkheim's typology of suicides is utilized and an analysis of the social and psychological components of each type undertaken. The social structure is viewed from the vantage point of how it influences and is internalized by members of society. The psychological aspects are handled by looking into what intrapsychic and external forces shape the individual's personality and behavior in such a way that he seeks his own death. This experimental study tested the effect of prenatal instruction on the success of a group of mothers who desired to breast feed their infants. It was hypothesized that mothers receiving information on how to nurse their infants, how to prevent complications and how to cope with common problems of breast feeding would have a higher success rate than those not so instructed.

Criteria for success was that the mothers would have an adequate supply of milk to meet the nutritional demands of their infant without supplementation, would have fewer and less severe complications, and would express a higher degree of …

A descriptive case study was conducted at one acute care medical center using one head injured patient who was unconscious for thirty-three days before testing. The study was done to provide descriptive information on the behavioral, physiological and electroencephalographic changes following a program of intermittent, repetitive, long-term auditory stimulation.

A patient who appeared to be unconscious thirty-three days, who had no evidence of extensive cortical damage and who had no evidence of hearing difficulties participated. The experimental stimulation was applied by earphones and consisted of a tape recording of eight Christmas songs which were separated by four seconds of silence, …

An exploratory study was designed to find out what questions mothers had about postnatal sexuality. A questionnaire was prepared by the nurse researcher to find out answers to the following questions.

  1. What specific questions do mothers have about sexual relations following delivery?
  2. Do mothers feel free to initiate discussion about resumption of intercourse following delivery?
  3. Do mothers like their husbands to be included in the discussion?
  4. When do mothers prefer to receive information regarding sexual relationships following delivery?

A descriptive-comparative survey using content analysis of medical records has been done to determine the frequency with which emotional disorders are identified by medical personnel in the population hospitalized in small general hospitals in north-central Nebraska.

A systematic, random sample of medical records was taken from two hospitals arbitrarily included in the study on the basis of bed capacity and distance from mental health facilities.

Each medical record that was reviewed has been placed in a category based on whether an emotional disorder was identified in it by the medical personnel, or whether it contained recorded behaviors that could indicate …

James G. Matthews was a jack-of-all-trades. He worked as a coal miner for James C. Coal Company, worked for United Fuel Gas Company, the Nickel Plant, and as a mail courier. He also performed as part of a singing quartet. Mr. Matthews was born on May 30, 1902. He grew up on a farm near East Lynn, WV. After the death of his mother, Mr. Matthews attended elementary school on a part-time basis in order to work on the family farm.
