Painless Thyroiditis With Hyperthyroidism Following External Irradiation To The Neck, J. David Fachnie, Sudhaker D. Rao

Two months after finishing 60Co external irradiation to treat squamous cell carcinoma of the larynx, a 47-year-old man became hyperthyrold. Although the thyroid gland was not tender, the radioactive iodine uptake was diminished. After subsequent periods of euthyroidism and hypothyroidism, the patient's thyroid function spontaneously returned to normal. The features of this case mimic the syndrome of painless granulomatous thyroiditis. Although hypothyroidism is frequently observed following radiation thyroiditis, hyperthyroidism from external radiation has not been reported. Despite this low frequency, diagnosis is important to permit prompt, symptomatic treatment of the thyrotoxic phase.

Graves' Disease Following The Occurrence Of Hypothyroidism, A. R. Guansing, D. D. Klink, N. Engbring, Y. Leung, J. Chakravarty, S. Wilson

Three patients with hypothyroidism of 15-48 months' duration developed Graves' disease while on thyroid hormone replacement therapy. In one patient, the hyperthyrold phase was preceded by ophthalmopathy and nonsuppresslble 24-hour radioactive iodine uptake (RAIU). Eventually, all had hyperthyrold findings with elevated thyroid hormone levels, high 24-hour RAIU, and a diffuse uniform uptake of RAI by the enlarged thyroid. All subjects had negative antithyroglobulin and antimicrosomal antibody titers. Two had demonstrable long-acting thyroid stimulator (LATS) in their sera. All three were followed for 5-36 months while on propylthiouracil therapy prior to surgery or RAI administration. Both LATS-positive patients underwent bilateral subtotal …

Insulin-Resistant Diabetes Mellitus And Insulin Receptor Antibodies: Variable Association With Acanthosis Nigricans, José Goldman

Two patients with severe insulin-resistant diabetes mellitus and anti-insulin receptor autoantibodies are reported here. A marked decrease in the number of accessible insulin receptors was found in the monocytes of one patient. Otherwise, the Insulin receptors were of normal affinity compared to those of normal controls. Acanthosis nigricans was not present in one patient, and in the other patient it preceded the diagnosis of diabetes by ten years. We conclude that although acanthosis nigricans and insulin resistance are frequently associated, they probably do not bear a pathogenetic relationship to each other. Since the enactment of the Federal Coal Mine Health and Safety Act1 (FCMHSA) in late 1969, occupational pneumoconiosis, "black lung disease," has been compensable under federal law. These federal black lung benefits are coordinated with state workmen's compensation or occupational disease laws to provide death or disability benefits for coal miners suffering from the disease. Black lung benefits are provided at federal cost under Part B of Title IV of the FCMHSA with respect to claims which arose before July 1, 1973 (Part B benefits)." This study examines the adoption and implementation of a decentralized budgeting model within a public mental‑health institution, focusing on the administrative, financial, and organizational consequences of shifting from a centrally controlled budget framework toward local unit responsibility. Drawing on archival records, budget data, and administrative narratives from the institution’s transition period, the analysis highlights how decentralization altered decision‑making dynamics, cost behaviour, service orientation, and internal accountability structures. The decentralization initiative empowered individual units by giving them budgetary control over expenditures previously managed at the centralized level, thereby increasing their autonomy over staffing, program purchases, and resource reallocation. The Effect Of Periapical Inflammation On Cellular Cementum : A Scanning Electron Microscope Study, Gary S. Yonemoto

The purpose of this study was to obtain information on the morphologic changes that occur in cellular cementum exposed to periapical inflammation.
Ten single rooted human teeth were collected. There were five specimens of normal cellular cementum and five specimens of diseased cellular cementum, which had been exposed to periapical inflammation. Each specimen was fractured longitudinally through the apical foramen, leaving the organic component intact. The cross-sectional morphology of cellular cementum was then examined using an ARM Model 1000 scanning electron microscope. Energy dispersive x-ray spectrometry was also conducted on samples representative of the two test groups.

Orthodontic tooth movement was performed on five monkeys (Macaca nemistrina). Oral health was established and exploratory surgery was performed to assess the level of the connective tissue attachment and marginal bone. Measurements of the gingival margin and mucogingival junction were taken and orthodontic forces were applied. The central incisors were moved labially a mean distance of 3.05mm. Post therapy measurements were performed to assess the change which occurred as a result of tooth movement. Significant recession of the gingival margin, connective tissue level and marginal bone was found when compared to an amalgam marker located on the facial surface of …

A review of the laryngectomy literature, from the earliest mention of the laryngectomy surgical procedure to the present was conducted; with specific emphasis upon factors pertinent to laryngectomee rehabilitation and esophageal speech development. Psychological, idiosyncratic, social, therapeutic and physiological factors were reported as affecting esophageal voice development. There were a great diversity of variables that might be predictive in judging acquisition of esophageal voice. Plasma samples were obtained from seven hypertriglyceridemic (>150 mg/dl) chronic renal failure patients before and after regular hemodialysis. The effect of the plasma on the lipolytic activity of bovine milk lipoprotein lipase was investigated. The lipoprotein lipase activity, as measured by release of C14-labeled free fatty acid from triglyceride, was decreased 8-67% when incubated with uremic plasma as compared to the control plasma (44 mg/dl triglyceride). The inhibition noted prior to hemodialysis did not appear to be alleviated by the dialysis but subsequent treatment with an isolated ultrafiltration technique did appear to remove an inhibitory substance as …

A study was made to investigate the biological possibility of obtaining new connective tissue attachment to periodontally diseased root surfaces following full thickness flap procedure. Reconstructive surgery was performed on seven teeth involved in advanced chronic periodontal disease. A mucoperiosteal flap was raised and the apical extent of existing subgingival calculus was marked with a notch made into the root just apical to the calculus. Following thorough instrumentation, sterile saline was applied to the root surface for five minutes, and the flap was repositioned and sutured. In American society, convalescent facility residency is a fact of life for many of the aged. Typically, in these geriatric facilities, eighty to ninety percent of primary patient care services are delivered by unlicensed personnel. Unfortunately, this segment of the convalescent facility employee population tends to be transient, underpaid and overworked, and to feel very low esteem for themselves and their jobs. This situation has produced many service abuses and interaction difficulties in the convalescent facilities. A study was undertaken to determine the concentration at which a sealer reaches a point of nontoxicity. An in vitro model utilizing HeLa cells and human fibroblasts was used to observe the effect of various concentrations of eight freshly mixed and eight set root canal sealers. Evaluation of toxicity was determined by cell survival which was measured by incorporation of 3H-thymidine into the nucleic acid components of each cell type used. Each dilution tested was allowed direct contact with the cells for 24 hours before toxicity evaluation. The results indicate that all sealers at concentrations presently used are cytotoxic …

Expansion of the maxillae is a well recognized procedure in orthodontics. Little is known about Mandibular Expansion. This project is a pure research of the possibility of expanding the mandible laterally at the symphysis on a baboon.
The symphysis was cut A-P and expanded laterally 11 mm, then held and stabilized with a titanium mesh work screwed to the inferior border of the mandible. The defect area was filled with a bone graft of marrow and cancellous bone fragments taken from the greater trochanter and the crest of the ilium. New bone did fill in the defect and the expansion …

Preparations used in this study were obtained from an investigation which had undertaken the following:
Durapatite, a hydroxylapatite ceramic was implanted in 24 mature Beagle dogs to study its maintenance effect on the alveolar bone of the mandible.
After dividing the dogs into two groups, a fresh socket and a healed socket group, the posterior teeth were removed and alveolectomies were performed. Eight ceramic implants were then placed in the right mandible of each animal in each group. The healed group received the implants five weeks after tooth removal and the fresh socket group received implants immediately after extraction.

The present study had been designed to determine whether Blissymbolics - a visual-graphic communication system based on meaning rather than phonetics - could be used as a means of communication re-training with adults having expressive aphasia. Questionnaires returned by 328 physical therapists were analyzed to 1) establish a role definition for public school therapists and 2) determine the educational needs of therapists preparing for public school service. The therapists ranked 15 skills in importance to the role of the public school physical therapist and in urgency of their need to develop each skill. They also indicated which educational format they believed would be most appropriate for developing each skill. The respondents were divided into four groups matched by experience in treating children and by exposure to public school physical therapy. Rank-order correlations within each of these …

ตั้งแต่เดือนธันวาคม พ.ศ. 2520 เป็นต้นมา ภาควิชาพยาธิวิทยา คณะสัตวแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย ได้พิสูจน์โรค Aujeszky's Disease สุกรซึ่งเกิดระบาดขึ้นเป็นจำนวน 6 ครั้ง โดยใช้การพิสูจน์โรคหลายวิธี และพบว่าวิธี ฟลูออเรสเซนท์แอนติบอดี้เทสท์จาก impression smears ของสมองและ cryostat sections จากทอนซีลให้ผลรวดเร็วและมีความแม่นยําสูง เหมาะที่จะใช้ในกรณีที่เกิด โรคระบาด เพราะสามารถนำผลไปใช้ในการวางแผนป้องกัน และควบคุมโรคได้ทันท่วงที การศึกษาจุลพยาธิวิทยาของสมอง ทอนซิลและอวัยวะอื่น ๆ ของลูกสุกรป่วย และ การฉีดเชื้อไวรัสเข้าใต้ผิวหนังกระต่าย ถึงแม้จะใช้เวลาในการปฏิบัติมากกว่าแต่เป็น การปฏิบัติที่ได้ผลและเป็นการสนับสนุนผลทางฟลูออเรสเซนท์แอนติบอดี้เทสท์ได้เป็นอย่าง ดี ในการศึกษาครั้งนี้พบว่าลูกสุกรป่วย (อายุ 3-15 วัน) จำนวน 21 ใน 22 ตัว จาก การระบาดทั้ง 6 ครั้งมีการอักเสบของสมองแบบ non-suppurative encephalitis และพบ Cowdry type A intranuclear inclusion bodies ในเซลล์ประสาท และ glial cells ด้วย และพบหย่อมเนื้อตาย (focal necrosis) ที่ตับ ปอด ต่อมน้ําเหลือง ม้าม ทอนซิล ซึ่งหย่อมเนื้อตายเหล่านี้เกี่ยวข้องกับ intranuclear inclusion bodies ในการฉีดเชื้อไวรัสเข้ากระต่าย จะเกิดอาการคันมาก (intense pruritus) หลังจากฉีด 2-4 วัน อาการป่วยที่สำคัญของสุกรที่เป็นโรคนี้จาก ทั้ง 6 ครั้งคืออาการทางประสาท อาการทางระบบหายใจ และพบว่าสุกรที่กำลังท้อง มีอัตราการแท้งสูงในช่วงเวลาที่มีการระบาด

Aujeszky's Disease virus แยกได้จากสมองของสุกรที่ตายด้วย โรคที่ส่งสัยว่าเป็น Aujeszky's Disease โดยใช้ PK-15 cell line. ซึ่งต่อมาได้ทําการพิสูจน์โดยใช้วิธีฟลูออเรสเซนท์แอนติบอดี้ยเทสท์และไวรัสนอยทราลไลเซชั่น เทสท์ และยืนยันได้ว่ามีโรค Aujeszky's Disease ระบาดอยู่ในสุกรในประเทศไทย

ฟาร์มเลี้ยงสุกรฟาร์มหนึ่งตั้งอยู่ในท้องที่ จังหวัดราชบุรี มีปัญหาการผสมติดยาก (35%) แท้งลูก (24.40%) และลูกสู่กรที่คลอดออกมาไม่แข็งแรงทําการ ตรวจโรคแท้งติดต่อทาง Seroagglutination test พบว่าสุกรในฟาร์มเป็นโรคนี้ ถึง 72.09% จากสุกรทั้งหมด 86 ตัวจึงได้คัดสุกรเหล่านี้ออกจากฝูง
