This presentation by Mr. Peters and Dr. Steinhauer was given before the Henry Ford Hospital Professional Staff at a semi-annual dinner and staff meeting held on Tuesday, May 6, 1980. It offers a timely assessment of the position and prospects of the Hospital as we enter the new decade. On occasion, the journal features nontechnical articles like this presentation that we believe are of general interest to our readers.

Surgical Management Of Multicentricity, Nipple-Areola Involvement, And Bilaterality In Breast Carcinoma, F. Samhouri, M. Block, A. Kambouris

During a five-year period, 457 women who underwent partial, segmental, or total mastectomy were assessed for multicentricity (31%), nipple-areola involvement (17%), and bilateral breast carcinoma (9%). These factors determined the extent of surgery performed and the possibilities for later breast reconstruction. The frequency of nipple-areola involvement in breast cancer strongly indicates the need for alternatives to their preservation for breast reconstruction. The study concludes that total mastectomy for invasive carcinoma is the preferred method of treatment for early breast cancer until further clinical trials have assessed the value of segmentectomy.

Preliminary Experience With Endoscopic Papillotomy, Bernard Schuman

Endoscopic papillotomy is a non-operadve technique used to manage choledocholithiasis and papillary stenosis. Considerable experience in Europe and initial results in a few medical centers in the United States indicate that it is effective and safe. Six papillotomy procedures were performed at Henry Ford Hospital without complication, and with a successful outcome in five. Endoscopic papillotomy is the procedure of choice for retained common bile duct stones less than 2 cm in diameter.

Acute And Massive Intraparenchymal Hemorrhages Of The Posterior Fossa, Dennis E. Wilkins

Acute, massive cerebellar and pontine hemorrhages are discussed in the context of the routine availability of cranial computerized tomography. The diagnosis of these disorders is emphasized. An updated clinical review of the edologic, pathogenetic, and therapeutic aspects is also presented, with particular attention to the importance of hypertensive cerebrovascular disease as the most common underlying disorder.

Prenatal Diagnosis Of Fetal Cystic Hygromas Associated With Generalized Lymphangiectasis, Chang Y. Lee, Beatrice L. Madrazo, Daniel L. Van Dyke, Jewel Smith

Ultrasonography has made possible the prenatal diagnosis of many congenital fetal abnormalities. This report describes two cases of bilateral cystic hygromas of the neck associated with generalized lymphangiectasis that were diagnosed by ultrasound. Ultrasonic scans revealed moderate polyhydramnios: thick, edematous placenta and edematous fetus with large cystic mass occupying both sides of the neck and extending to the upper chest wall, ascites, and pleural effusion at gestational ages of 21.5 and 24 weeks, respectively. In one case, chromosomal study from amniotic fluid cell culture revealed X chromosome monosomy, often associated with lymphatic anomalies. The prenatal diagnosis was confirmed at birth: …

Three case histories of patients with malfunctions of Smeloff-Cutter mitral ball valve prostheses are presented here - one underwent successful replacement. The clinical diagnosis was made by observing a marked variation in the A2OC interval and intermittent absence of the opening click. Phonocardiograms were diagnostic in all three cases. while echocardiograms and even angiograms did not uniformly diagnose prosthetic valve dysfunction. Periodic phonocardiographic evaluations may be helpful in early detection of the prosthetic valve dysfunction. Once the diagnosis is established, immediate surgical treatment is imperative to prevent sudden death.

The purpose of this investigation was to determine the degree of healing which takes place in the periodontal tissues as a result of supragingival and subsequent subgingival plaque control attempts without any associated supra- or subgingival instrumentation and root planing.

Seven patients with moderate to severe periodontitis were finally selected for the study. Measurements were made on maxillary and mandibular incisors, cuspids and premolars to establish base line plaque scores, bleeding scores, probing pocket depths, and probing attachment levels. Stress of handling, partial or complete sialoadenectomy, and the administration of parotid hormone (PH), sodium fluoride (NaF), or guanidino-succinic acid (GSA) were examined for effects on fasting plasma glucose and insulin or effects on intravenous glucose tolerance in male Sprague-Dawley rats. Basal glucose and insulin were substantially elevated as a result of the stress of translocation in large animals, but were not altered in small animals. A rise in plasma insulin occured in control animals 20 minutes following anesthesia and the removal of a blood sample by cardiac puncture but not in animals that had been handled twice daily for …

The present investigation was initiated to establish cut-off scores for the shortened form of the Carrow Elicited Language Inventory (CELI). With such norms available, the assessment tool might be utilized as an expressive language screening device for the children ages five years nine months through six years nine months who are entering the first grade.

A review of the literature revealed few expressive language screening devices which are practical for use by the speech-language specialist in the schools. Three spices, nutmeg, mace and black pepper, and selected oil constituents, safrole, eugenol and myristicin, were tested for mutagenicity using the Aines test.

Safrole, an oil constituent, is a weak hepatic carcinogen in the rat and mouse. Safrole metabolites are more mutagenic than the parent compound. Myristicin and eugenol are structurally related to safrole and break down into similar metabolites as safrole. All three are present in nutmeg, mace and black pepper. These three spices were tested for mutagenicity in the Salmonella/microsomal mutagenicity test using strains TA1535, TA1537, TA1538, TA98 and TAlOO with and without metabolic activation. The tensile properties of .017 x .025" stainless steel. Multiphase, Blue Elgiloy, beta titanium, and Nitinol were tested and compared. All wires were tested in the "as received" condition. Fifteen samples of each type of wire were tested in order to determine the (1) modulus of elasticity, (2) .2% yield strength, (3) ultimate tensile strength, (4) percentage elongation and (5) maximum elastic deflection.

Recent investigations have shown decreased fetal weight in pregnant animals who were exercised regularly during pregnancy (Gilbert; Nelson; Longo, 1979; Terada, 1974). The purpose of this experimental study was to investigate fetal weight gain and measure blood flow in pregnant guinea pigs who were exercised 45 minutes per day; and thus answer the hypothesis that prolonged maternal exercise will cause decreased weight gain and decreased placental blood flow during pregnancy.

1. To present the deaf-cerebral palsied population with a more functional means of communication.
2. To test the validity of using a non-vocal means of communication, Blissymbolics, when other more traditional methods of teaching communication skills (sign language or verbal communication) have been ineffective.

Blissymbolics has been found to be an effective means of communication with cerebral palsy children who are unable to develop verbal communication skills. However, there is no research in the use of Blissymbolics with the deaf-cerebral palsy population.

Four periodontal parameters were examined to determine the effect of rapid and slow palatal expansion techniques on the supporting structures at the buccal aspect of the maxillary first permanent molars. The parameters examined included the levels of crestal alveolar bone and soft tissue attachment (relative to the cemento-enamel junction), the depth of the gingival sulcus, and the height of the attached gingiva. The area between the buccal roots was inspected for possible furcation involvement. สัตว์ป่วยเป็นสุนัขเพศเมีย อายุ 9 ปี พันธุ์ดอกเกอร์สเปเนียล นำมา รักษาที่โรงพยาบาลสัตว์เล็ก คณะสัตวแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย เจ้าของสังเกตพบก้อนโตแข็ง ใต้คอด้านซ้ายแต่สุนัขไม่ได้แสดงอาการเจ็บปวด การตรวจคลําพบก้อนกลมแข็งในช่องท้องได้เก็บเลือดตรวจทางโลหิตวิทยา เปลี่ยนแปลงเป็นนัยสำคัญ การตรวจโดยฉายรังสีเอ็กซ์พบตับโต ม้ามโต relative narrowing of intervertebral space of L1-L2 spondylosis of L1-L4-L5-L6 ในระยะหลังสุนัขมีสภาพทรุดลง มีอาการเบื่ออาหาร โลหิตจาง มีการ ติดเชื้อที่ก้อนแข็งใต้คอ เจ็บปวดบริเวณส่วนท้ายของลําตัว ส่องขาหลังไม่มีแรงและไม่เดินในที่สุดเจ้าของจึงอนุญาตให้ฉีดยาทําลายสัตว์ รวมระยะเวลาการรักษาประมาณ 3 เดือนครึ่ง จากการผ่าซากและตรวจทางจุลพยาธิวิทยาพบก้อนแข็งใต้คอเป็น malignant melanoma (spindle cell type) และมีการแพร่กระจายไปที่ปอด ตับและม้าม basilar cells ของ epidermis แล้วกระจายรงควัตถุเมลานินไปในชั้น epidermis และ hair follicles ดังนั้นเนื้องอกชนิดนี้จึงพบมากที่ผิวหนังในสัตว์เกือบ ทุกชนิด แม้กระทั่งปลา (Smith et al.,1972) นอกจากนี้ก้อนเนื้องอกเมลาโนมา อาจเกิดจาก chromatophores ของม่านตาได้ (Bloom, 1942)

ประสิทธิภาพของยา Canex Plus ต่อพยาธิในทางเดินอาหารสุนัข, นิคม เสวกาพานิช, งามตา ศรีสกุล, นาลละออ สงวนดิสกุล, มานพ ม่วงใหญ่

การทดลองใช้ยา Canex Plus (R) ซึ่งประกอบด้วย Oxantel pamoate a Pyrantel pamoate เพื่อกำจัดพยาธิในทางเดินอาหารของสุนัขซึ่งเป็น สาเหตุที่สำคัญยิ่งปัญหาหนึ่งโดยการให้ยาครั้งเดียวในขนาด 1 เม็ด/น้ำหนักตัว 7 กิโลกรัม ในสุนัขทดลองจำนวน 15 ตัว แล้วดูผลของยาถ่ายพยาธิ 3 วันหลังให้ยาเปรียบเทียบ กับสุนัขอีกกลุ่มจำนวน 8 ตัวที่ไม่ได้ให้ยา พบว่า Canex Plus (R) สามารถลดจำนวน ไข่พยาธิแส้ม้าได้ 100% ใน 3 วัน แต่ลดจำนวนไข่พยาธิปากขอและพยาธิไส้เดือนได้ ไม่หมดทุกตัว และเมื่อคํานวณหาค่าประสิทธิภาพของยานี้ต่อตัวแก่ของพยาธิแส้ม้าได้ 94.44% ต่อพยาธิปากขอได้ 62.39% และต่อมาพยาธิไส้เดือนได้ 100% แต่ไม่ได้ผล ต่อพยาธิตัวจี๊ดและพยาธิเม็ดแตงกวา

438 day-old-piglets from 45 litters and 454 piglets from 127 litters were used for the prevention and treatment purposes in this study which E-coli (70.03%) was the dominant pathogen on this experimental farm. For the first trial, each piglet in each litter was randomly divided into three groups. A single oral dose of Linco-Spectin Pig Pump was administered to the group of 144 piglets compared to another two group of 144 piglets and 150 piglets which were treated with Biosol-M pump and negative control respectively. ศึกษาการแก้ไขปัญหาการไม่เป็นสัดในแม่สุกรหลังหย่านม ซึ่งพบมีตั้งแต่ 16-53% ต่อเดือน ในฟาร์มโครงการหมู่บ้านเกษตรกรรมกำแพงเพชร ตั้งแต่เดือน พฤษภาคมถึงเดือนสิงหาคม 2523 แม่สุกรที่ไม่แสดงอาการเป็นสัดหลังหย่านมระยะต่าง ๆ กัน จำนวน 61 ราย ถูกแบ่งออกเป็น 3 กลุ่ม ๆ แรก จำนวน 25 ตัวได้รับการฉีดสาร โปรสตาแกลนดินเอฟทอัลฟ่า จำนวน 125 - 200 ไมโครกรัม ในวันที่ 26.56 ± 10.80 วัน (ระยะ 6-48 วัน) หลังหย่านมแม่สุกรทุกตัวแสดงอาการเป็นสัดภายใน 7.04 ± 6.00 วัน ได้รับการผสมและติด 68% กลุ่มที่ 2 จำนวน 11 ตัว ได้รับ gonado- trophic hormone (FSH ขนาด 400 IU และ LH 400 IU) ในวันที่ 37.64 ± 22.03 วัน (ระยะ 13-78 วัน) แสดงอาการเป็นสัดทุกตัวใน 6.64 ± 5.77 วันหลัง ฉีดและผสมติด 72.70% ส่วนในกลุ่มที่ 3 ใช้ฉีด E.C.P. ขนาด 2 มก. แก่แม่สุกร 15 ตัว ซึ่งไม่เป็นสัดเป็นเวลา 24.57 ± 18.15 วัน (ระยะ 10-70 วัน) หลังฉีดเกิด การเป็นสัดภายใน 4.29 ± 4.63 วัน ผสมติด …

Male Buffalo rats were fed a control basal diet and diet containing a. α -BHC (500 ppm); b. AFB1 (1 ppm); and c. α -BHC (500 ppm) plus AFB1 (1 ppm) for period of 5,10,15,35, weeks. The diets were then replaced with Chow pellets for 30 weeks. The livers of the fed α-BHC or α-BHC plus AFB1 were markedly increased in size and exhibited centrolobular hepatic hypertrophy in comparison with the livers of the rats receiving AFB1 alone or those of the control. By Week 35, α-BHC produced large hepatic cytoplasmic inclusions and these changes were reversible at week 65. …
