ลูดวิก แองไจนา (Ludwig's Angina) รายงานผู้ป่วย 2 ราย, ไพศาล กังวลกิจ

Ludwig's angina เป็นการอักเสบที่เกิดขึ้นอย่างเฉียบพลัน รุนแรง ที่บริเวณใต้ขากรรไกร (submandibular), ใต้คาง (submental) และใต้ลิ้น (sublingual) การอักเสบนี้มักกระจายหาขอบเขตไม่ได้ และเป็นทั้ง 2 ข้าง ปัจจุบันพบได้ไม่บ่อยนัก เนื่องจากมียาปฏิชีวนะที่ดี สาเหตุของการอักเสบนี้ส่วน ใหญ่เกิดจากฟัน ในรายงานฉบับนี้ได้กล่าวถึง ผู้ป่วย 2 รายที่มารับการรักษาที่โรงพยาบาลชลบุรี เนื่องจากมีอาการบวมที่บริเวณใต้ขากรรไกร คอและพื้นช่องปาก ได้ให้การวินิจฉัยว่าเป็น Ludwigs angina ซึ่งมีสาเหตุมาจากการติดเชื้อของ ฟันกรามล่างขวา ซี่ที่ 1 (# 46) ได้ให้การรักษาโดยให้ยาปฏิชีวนะในขนาดสูง เจาะคอ และใส่ท่อหายใจเพื่อป้องกันการอุดตันของทางเดินหายใจ กรีดระบายหนอง (Incision and drainage) และถอนฟันที่เป็นสาเหตุออก

มีรายงานการเกิดคราบสีบนฟันหลังจากการใช้ยาอมบ้วนปากคลอเฮกซิดีน โดยเฉพาะเมื่อมีสารอาหารบางอย่าง การทดลองนี้เพื่อต้องการศึกษาผลของการใช้ กาแฟ ชา แทนนิน โกโก้ และน้ำหวานสีแดง ร่วมกับ ยาอมบ้วนปากคลอเฮกซิดีน 0.2% ที่เป็นกรด เปรียบเทียบกับน้ำยาที่เป็นด่าง ในการเกิดคราบสี ผลปรากฏว่าไม่มีความแตกต่างกันระหว่างยาอมบ้วนปากทั้งสองชนิด

การใช้น้ำยาทําลายเชื้อในรอยพิมพ์ปาก, ประเวศ เสรีเชษฐพงษ์, ศรีวรพงษ์ พงษ์สถิตย์

เนื่องจากรอยพิมพ์เป็นสื่อที่อาจจะนําเชื้อโรคจากปากของผู้ป่วยมาสู่แบบจําลองได้ ดังนั้น ทุกรอยพิมพ์จึงควร ได้รับการทําลายเชื้อ รอยพิมพ์ชนิดโพลีซัลไฟด์ (polysulfide) ซิลิโคน (Silicone) โพลีอีเทอร์ (polyether) ซิงค์ออกไซด์ยูจินอล (zinc oxide eugenol) จะถูกทําลายเชื้อได้ดีโดยใช้น้ำยาสําหรับฆ่าเชื้อ และเมื่อได้ใช้น้ำยาฆ่าเชื้อแล้ว แบบจําลองยังคงมีขนาด รูปร่าง (dimensional stability) และรายละเอียด (surface detail sharpness) ที่ถูกต้องเหมือนเดิม ส่วนรอยพิมพ์ที่ใช้วัสดุชนิดไฮโดรคอลลอยด์ชนิดผันกลับได้และผันกลับไม่ได้ (reversible and irreversible hydrocolloid) คอมพาวด์ (impression compound) จะใช้น้ำยาฆ่าเชื้อไม่ได้ดีเพราะแบบจําลองที่ได้จะมีขนาดและรายละเอียดเปลี่ยนไป

This dissertation examined whether a hearing impairment of the auditory end-organ has the same or a differential effect on the place and periodicity processes. Differential sensitivities for four normally hearing listeners and for both ears of five patients with unilateral Meniere’s disease were measured for tonal frequency and rate of sinusoidally amplitude-modulated noise at common frequencies and rates of the stimulus.

We conclude from the analysis that the Reporting System, as presently administered, fails to achieve its primary statutory objective: it does not provide APHIS with information sufficient to demonstrate that researchers have used pain-relieving drugs "appropriately" and in accordance with "professionally acceptable standards." The chief reasons for this failing are (1) regulations and guidelines do not define "pain" or "distress," (2) regulations and guidelines do not adequately define "routine procedures," and (3) regulations and guidelines do not require meaningful explanations for the withholding of pain-relieving drugs in procedures acknowledged to cause pain.

The possibility of decreased prostacyclin (PGI2) in preeclampsia-eclampsia is currently being studies by several laboratories. Because of prostacyclin's various actions (platelet aggregation inhabitation, vasodilation, stimulation of renin release, and possible inhibition of the hypertensive effects of angiotensin II), an increase in the peripheral activity of this chemical could explain much of the altered physiology of normal pregnancy. A relative decrease in peripheral activity could tie together the various pathophysiological findings of preeclampsia-eclampsia. Therefore, plasma 6-keto, PGF1α, the stable nonenzymatic metabolite of prostacyclin, was measured in women with preeclampsia, and in those women of various gestational ages having a normal pregnancy. …
