Register Of Multiple Endocrine Neoplasia Type 2 Syndromes In The United Kingdom, Bruce A. J. Ponder

A collaborative group for the study of multiple endocrine neoplasia type 2 (MEN-2) syndromes comprised of clinicians and laboratory scientists has been set up in the United Kingdom. Its aims are 1) to provide a basis for collaborative work on MEN-2; 2) to establish a register of patients; 3) and specifically to conduct studies aimed at defining the best policy for screening the families of apparently sporadic patients, to establish radioimmunoassays for family screening, and to identify large kindreds for genetic linkage studies using DNA polymorphisms.

Screening For Hereditary Medullary Cancer In Denmark, Kristian Emmertsen

Screening of first-degree relatives of patients with medullary thyroid cancer (MTC) gave normal values of pentagastrin-stimulated serum calcitonin and 24-hour urinary catecholamine levels in the relatives of 18 of 22 patients. This result is considered to be valid evidence for sporadic MTC. Absence of C-cell hyperplasia maybe another indication of sporadic MTC. Four hereditary MTCs were represented by one fully expressed MEN-2B patient without affected relatives, one fully expressed MEN-2A case in one family, and two first cousins with MTC as the only MEN-2A lesion in another family. Through nationwide surveys, we collected and analyzed 242 patients of medullary thyroid carcinoma (MTC). Included were 40 patients with multiple endocrine neoplasia type 2A (MEN-2A), six patients with MEN-2B, and 36 patients with only MTC having a positive family history (82 total patients in the hereditary group). Ten-year survival rates were 81.5% for all cases, and 97.5% and 76.1% for the hereditary and the sporadic group, respectively. Epinephrine/norepinephrine ratio in the urine was found to be a good indicator of the adrenomedullary hyperfunction in patients with hereditary MTC. We reviewed the scintigraphic images made after injections of 1-131 metaiodobenzylguanidine (MIBG) or 1-123 MIBG in patients with multiple endocrine neoplasia (MEN) types 2A and 2B. The information we obtained was applied to three questions about the treatment of pheochromocytoma in patients affected with these syndromes. Our first question "When should adrenal gland(s) be removed?" was not directly answered. However, adrenalectomy generally should not be contemplated unless distinct abnormalities are present in the scintigraphic images. With experience it may be possible to approximate, from the stage of pheochromocytoma depicted by scintigraphy, how many years will elapse before symptoms or hypertension …

Using recombinant DNA techniques, we analyzed the structure of human calcitonin (CT)-encoding DNA. The sequence of nucleotides derived from the messenger RNA (mRNA) predict the amino acid sequence of the entire human CT precursor protein. The complete CT gene could be isolated from a DNA library of the human genome. The CT precursor encoding sequences (exons) as well as the noncoding intervening sequences (introns) could be identified. Another sequence, called human calcitonin gene-related peptide (hCCRP), could be located near this CT-encoding DNA. 左大腦損傷與右大腦損傷之中風病人智能狀態的比較, 以雯 袁, 瑞棋 詹, 道昌 徐

本研究修改Comprehensive Mental Status Examination，對81名因為中風而接受榮總復健治療的病人進行測驗，比較左、右大腦損傷對智力所造成的影響。所有的病人都是第一次受傷，單側受損，以右手為優勢手，測驗時病人身體狀況均已穩定，神智清醒。病人年齡分佈為3i歲至75歲，平均60.08±7.68歲。教育程度小學以下者13人，小學程度12人，初中14人，高中21人，大專14人，另有7人為不能確定其教育程度者。病人從發病至接受測驗之時間間隔為6日至90日，平均26.26±16.71日。81名病人中有32人為右腦損傷，49人為左腦受損者，49人中有18入為失語症患者。兩半球受損的病人在教育程度、年齡及時間間隔上均無顯著差異。 測驗結果，右腦損傷者在注意力項目中的digit repitition，語言項目中的naming，記憶項目中的orientation，remotememory，new learning ability，建構能力中的drawing to command及高層次認知活動中的每一項目都是以右腦損傷的病人表現較佳，兩組的差異達統計上顯著水準。 對測驗題目作因素分析(factor analysis)，抽出語言、抽象思考，空間關係及記憶保存四大因素。這些因素的發現，並配合大腦功能的劃分，可以解釋兩組病人在測驗上的差異。失語症病人除去之後，兩組差異不顯著，對這種現象，文內有進一步的說明。

The Journal of Sociology & Social Welfare

It is estimated that between 500,000 and 1.5 million Vietnam veterans suffer from Post-Traumatic Stress Disorder (PTSD), the nature of which includes sociological and psychological manifestations. However, despite past research efforts, an adequate understanding of the social consequences of PTSD and its effect upon veterans and their families remains largely unexplored.

Post-Traumatic Stress And Attributions Among Incest Family Members, Inger J. Sagatun

The Journal of Sociology & Social Welfare

This paper presents a discussion of the relationship between attribution models of self-blame and coping strategies among members of incest families. The findings suggest that incest victims are more likely to experience prolonged post-traumatic stress than are offenders and their spouses. The argument is made that by focusing on individual adjustment, therapy programs may be neglecting the interactive effect of attributions made by other family members, the legal system and outside observers. The author concludes by suggesting that a sociological model in which the relationship between social reactions to incest and attributions is addressed would enhance the effectiveness of self-help …

Investigation Of 500 Cerebral Palsies In Taiwan R.O.C., Alice May-Kuen Wong

Analysis of 500 cerebral palsies in National Taiwan University Hospital were done from July 1980 to June 1984.Among them, 292 cases were male (58.4%), 208 were female (41.6%). The age distributions that they were brought to our clinic were sequently as following: within 1/2 year old 22.0%, 1/2 to 1 year 31.8%, 1 to 2 year 38.0%, 2 to 5 year 8.8%, above 5 year group 0.4%. The proportions of causal factors were 31.0% in prenatal period, 47.0% in perinatal period, 15.0% in postnatal period, and 7.0% in untracable cases. 構音異常語言治療的結果, 百嘉 楊, 湘君 賴, 文玲 廖

4歲以上接受治療的構音異常患者，男性有70例，女性有37例，合計107例。其中有鼻音缺乏症狀者31.8%。錯誤音的個數由2到25個，平均為12.5個音。治療的方法為(1)聽覺區分訓練，(2)發言訓練，(3)節奏訓練。治療後患者有很大的進步，42.1%的患者完全治療，61.7%的患者治療率超過 50%以上。治療時間每次30分鐘，達到完全治療所需之治癒次數後6次到105次，平均為29．0次，女性較男性所需之平均治療次數少。治癒率愈高，所需之治療次數愈多。有鼻音缺乏症狀，在治療時對男性沒有影響，'對女性則會使治療次數增加很多。患者錯誤音的個數在 16個以下時，大多只錯字首音;超過17個錯誤音以上時，連字尾音也產生錯誤，治療困難，使超過50%以上治癒的人數顯著降低，並使所需之治療次數顯著增加。

為了研究中國語言構音異常的類型，利用 37個國語注音符號，分析其錯誤的分佈情形。經過構音測驗且年齡在4歲以上者共計146例，其中男性96例，女性50例。錯誤音按其發生率由高到低排列，共次序為ㄙ、ㄘ、ㄗ、ㄔ、ㄕ、ㄒ、ㄓ、ㄉ、ㄊ、ㄑ、ㄖ、ㄐ、ㄎ、ㄈ、ㄍ、ㄆ、ㄢ、ㄣ、ㄤ、ㄥ、ㄌ、ㄏ、ㄋ、ㄜ、ㄞ、ㄅ、ㄠ、ㄝ、ㄡ、ㄦ、ㄩ、ㄇ、ㄚ、ㄛ、ㄟ、ㄧ、ㄨ。其中發生錯誤的頻率最高的為ㄙ有86.3%，至曰為50%。四聲聲調的錯誤未曾發現。依發音方式而言，擦音及擦音錯誤多，爆音次之。依錯誤的型態來分，附加音的情形未發現，代替音和省略音較多，扭曲音較少。字尾音很少錯誤(0-10%)，但帶鼻音之字尾音則有27.4%的患者有鼻音缺乏的症狀，且女性的發生率高於男性。隨著年齡的增加，男性患者鼻音缺乏症狀的比例明顯降低，女性則不明顯。鼻音過重的現象極少發現。發生錯誤音的個數由2到29個，平均為11．9個錯誤音。各年齡層錯誤音的平均個數之間，無明顯差異。排行的位置，對構音異常的發生沒有明顯的影響。來接受檢查的患者，其父母親的教育程度，以高中以上者較多。
