BACKGROUND: Patient decision aids have the potential to lower decision conflict for patients and to improve patient-physician communication. However, uptake of decision aids has been poor, in part because the time required to incorporate these into clinical practice is not well understood.
OBJECTIVE: To estimate the time required for a rheumatology clinic to implement a validated decision aid for patients with lupus.
METHODS: Using a cohort of eight implementation sites, study investigators identified the activities required to administer a decision aid. Site coordinators embedded within the clinics timed the duration of each activity. To estimate the effect of viewing the …

Introduction: In December 2019, the novel SARS-CoV-2 triggered a global pneumonia outbreak, leading to millions of deaths worldwide. A subset of survivors faces increased morbidity and mortality, particularly due to subacute lung injury evolving to chronic fibrosing interstitial lung disease. While nintedanib, a tyrosine-kinase inhibitor, shows promise in treating progressive fibrotic lung disease, limited randomised trial data exists for post-COVID-19-induced lung injury. We hypothesise that treatment with nintedanib may attenuate advancement to the fibrotic stages, offering a potential avenue for improving outcomes in this specific patient subset.

Phelan-McDermid syndrome (PMS) is a neurodevelopmental disorder caused by haploinsufficiency of the SHANK3 gene. Approximately 25% of individuals with PMS have epilepsy. Treatment of epilepsy in PMS may require multiple anticonvulsants, and in a minority of cases, seizures remain poorly controlled. Converging lines of evidence in different experimental models indicate that the Ras-ERK pathway is implicated in the pathophysiology of seizure generation and neurobehavioral symptoms in PMS. The goal of this study was to evaluate the safety, tolerability, and efficacy in treating seizures in adults and adolescents with PMS using AMO-01, a Ras-ERK pathway inhibitor. Angiotensin-converting enzyme 2 (ACE2) and the amino acid transporter B0AT1 are essential for blood pressure regulation, amino acid absorption, and viral entry. Their interaction is crucial in both normal physiology and disease, including hypertension, Hartnup disease, and SARS-CoV-2 infection. However, the effects of genetic variants on their biogenesis, trafficking, and function remain poorly understood. This PhD thesis investigates how specific ACE2 and B0AT1 variants influence their subcellular localization and interactions. A multidisciplinary approach, incorporating site-directed mutagenesis, confocal microscopy, Western blotting, and in silico modelling, was used to examine 39 ACE2 variants and 18 B0AT1 mutations. There is a significant amount of racial and gender bias in sports media, which can impact the way minority and female athletes are perceived by the public. This study closely looks at the way these professional athletes are talked about and treated by the media compared to white athletes. This study highlights the language used by commentators and other members of sports media that reinforces negative stereotypes that minority and female athletes deal with throughout their careers. Some of these stereotypes imply black athletes owe their success to natural physical ability, while white athletes are “smart” and more “skilled. This …

Research has suggested that birth order or environmental factors affect siblings' oral health status during childhood. However, more recent research suggests that the strongest predictor of children's oral health status is the mother's oral health status. This project investigated the association between the oral health status of the mother during pregnancy and bacteria (biofilms) identified in cavitated lesions (dental caries) of their children’s exfoliated primary teeth. Five consenting maternal participants anonymously donated their children's primary teeth and completed a questionnaire about their oral health status during each pregnancy. Boglands are iconic landscapes and important carbon sinks, extending across the northern hemisphere, covering approximately 20% of Ireland's land area. These ancient water-logged ecosystems are reservoirs of a unique biodiversity adapted to survive in nutrient-poor yet strikingly beautiful environments. Irish mythology is deeply connected to boglands, featuring stories about healing plants, fairies, and ancient curses. Irish communities historically used bogland plants for their medicinal properties. Many uses are recorded in the Schools' Manuscript Collection (SMC) (1937-1939), a treasure-trove of Irish folklore, customs, and ethnographic data, collected as part of the Irish Folklore Commission's effort to preserve Ireland’s oral traditions (Dúchas.ie). …

It is mandatory in 39 European countries and the European Union. The Ph. Eur. establishes quality specifications for active ingredients, excipients, dosage forms, and finished products. Objectives: This study aimed to evaluate the sealing ability of mineral trioxide aggregate (MTA) and 2.25% sodium hypochlorite (NaOCl) gel mixture in repairing furcal perforation in primary molars using methylene blue dye penetration.

Metabolic syndrome is increasing in prevalence in the United States. To be diagnosed, three of the five diagnostic criteria must be met. There is a higher risk of metabolic syndrome due to lifestyle and social factors, with increased criteria occurring as an individual has unmet social determinants of health. This quantitative quasi-experimental/quasi-comparative study examined the association between education level, with sleep quality and depression as moderators, on a diagnosis of metabolic syndrome with all five criteria versus three criteria using a binary logistic regression while controlling for age, race, gender, income, employment, and marital status. This case report is centered on an atypical presentation of Hereditary Vitamin D-dependent Rickets 2A (VDDR2A), a rare disorder caused by defects in the gene encoding the vitamin D receptor (VDR). While this disorder is primarily autosomal recessive in inheritance, this case demonstrates that a single heterozygous variant in the VDR gene could be linked to both phenotypic and laboratory manifestations of this condition. Substance Use Disorder (SUD) is a relapsing disorder characterized by repeated seeking and use of psychoactive substances despite negative consequences to the individual. Epigenetic changes within the medium spiny neurons (MSNs) of the nucleus accumbens (NAc) are thought to underly the lasting propensity to relapse in SUD. The epigenetic enzyme histone deacetylase 5 (HDAC5) in NAc MSNs limits the formation of reward-cue associations in rodent models of drug seeking, but not sucrose seeking, leading to decreased relapse-like behavior. Background: Quality improvement (QI) initiatives aim to systematically improve the efficiency and safety of healthcare provided to patients. These data-driven projects also seek to improve processes within the healthcare industry. Potential goals for quality improvement projects (QIPs) can include improving patient outcomes, improving healthcare access, increasing efficiency, and enhancing patient safety. The Ready to Shine: A Framework for Coaching K-12 Educators to Support Diverse Learners is meant to guide occupational therapy practitioners in training and coaching educators to implement strategies to support students with social-emotional, behavioral, sensory, or other unique needs. Through this framework, practitioners will aim to equip educators with the confidence, self-efficacy, and competence to use tools and/or techniques that promote participation and performance among their students.

Direct Support Professionals (DSPs) serve an important role in assisting people with intellectual and developmental disabilities (IDD) in many aspects of their lives through the provision of care and support. Despite lacking in knowledge and training to support the potentially complex and varied presentation of people with IDD, DSPs are in a position to help their clients address and improve on their independent living skills that are important for autonomy and overall well-being. Building Bridges: A Community Health Worker Training Course is a comprehensive 20-hour continuing education course designed for community health workers (CHWS) who work with Black pregnant women. This training course was developed for and in collaboration with Allure Alliance Incorporated, a non-profit organization whose mission is to improve the lives of Black women through community programming, education, and advocacy. There are limited educational resources available on concussion management for occupational therapy practitioners, coaches, and student-athletes which can limit access to comprehensive occupational therapy services for clients who have sustained a concussion, ultimately hindering participation in sport and overall wellbeing. The purpose of this capstone was to develop a comprehensive guidebook for coaches and student-athletes and a comprehensive handbook for occupational therapy practitioners to raise awareness in concussion education and clarify occupational therapy's role on a multidisciplinary concussion management team. Informal caregivers (care partners) are people, such as family members, who provide care to those who cannot care for themselves, including people with dementia (PwD). Care partners assist PwD in their daily occupations, often without training. Caring for PwD can lead to emotional, physical, financial, and occupational challenges for care partners. This capstone project addressed the critical gap in psychosocial support for pediatric oncology clients by developing an evidence-based guidebook for occupational therapy (OT) practitioners. Grounded in the Model of Human Occupation (MOHO), the guidebook provides therapists with developmentally appropriate strategies to enhance emotional regulation, social participation, and engagement in meaningful activities for children aged 3 to 12 undergoing cancer treatment. The project identified key psychosocial challenges through qualitative interviews, needs assessments, and real-time clinical feedback and designed targeted interventions aligned with OT principles. Significance and Background: Heart failure with reduced ejection fraction (HFrEF) remains a major cause of hospital readmissions, increasing both healthcare utilization and patient morbidity. Guideline-Directed Medical Therapy (GDMT) has been proven to reduce mortality and hospitalizations; however, adherence to these evidence-based guidelines remains inconsistent (Yancy et al., 2022).
Purpose: This quality improvement project aimed to evaluate whether enhancing GDMT adherence in an outpatient heart failure clinic could reduce emergency room (ER) admissions among patients with HFrEF.
Methods: A structured intervention was implemented at a local healthcare system Cardiology Clinic, focusing on optimizing adherence to GDMT in 60 patients with HFrEF. …
