Sickle cell disease (SCD), an autosomal recessive hemoglobinopathy, affects ∼100 000 people in the United States.1 The process of transitioning from pediatric to adult SCD health care systems can be disjointed and poorly coordinated, contributing to the high morbidity and mortality seen in this population. There is no universally accepted definition of a successful SCD care transition, nor are there existing standards and recommendations for SCD clinicians. Background: This study examines public awareness in Saudi Arabia regarding con- trolled drug abuse and dependence. It aims to evaluate public knowledge of controlled drugs, their complications, legal considerations, and exposure to related campaign. The study also explored predictors of public awareness of controlled drug abuse and dependence. Methods: A cross-sectional survey utilized a random sampling technique to recruit 529 participants aged 18 or older across Saudi Arabia. Objective:Toassessthebiomechanicalbehaviourofimplant-tooth-supported(hybrid)pros-theses,comparedwithanimplant-implant-supportedprosthesis,utilisingtwodifferentprostheticmaterials,employingfiniteelementanalysis(FEA).Method:Threedigitalmodels,derivedfromaCBCT-scannededentulousmandible,weredesigned:twohybridconfigurations(M1:distalimplant-firstpremolar;M2:mesialimplant-firstmolar)andanimplant-implant-supportedprosthesis(M3).Abiomechanicalmodellingworkflowwascarriedout(Mimicsforsegmentation,3-Maticforbone/mucosallayerrefinement,ExocadDentalCADandSolidWorksforimplants,abutmentandprosthe-sisintegration,andANSYSformeshingandFEA).Assumingisotropic/elasticmaterialproperties,zirconiaandpolyetherketoneketone(PEKK)wereselectedasprostheticmateri-als.Modelsweresubjectedtoastaticverticalload(100N).vonMisesstressandtotaldefor-mationwereanalysedtoevaluatestructuralresponses.Results:M3demonstratedthemostfavourablebiomechanicalprofile,exhibitingthelowestvonMisesstressmagnitudesalongsidethehighestoveralldeformation.Inhybridmodels(M1,M2),stressmaximawerelocalisedatthepontic−toothconnectorregionsand,inM1-PEKK,reached261MPainthescrew.Corticalboneandpreparedteethexhibitedhigherstressindis-tallyanchoredhybridsystems(M1)comparedtomesiallysupporteddesigns(M2),regardlessoftheprostheticmaterial.Comparativeanalysisrevealedthatzirconia-basedprosthesesgen-eratedhigherstressconcentrationsinbone(≤35MPa)comparedtoPEKK,whereasthePEKKprosthesisitselfexhibitedgreaterstructuraldeformation(70mminM1-PEKK).Conclusion:Hybridimplant-tooth-supportedprosthesescanactasareliablealternativetoimplant-implantonesandeliminatecantileverextensions;however,theiruseshouldbereservedforcarefullyselectedcasestoavoidbiomechanicalincompatibilities.

Background: Systemic right ventricular (SRV) cardiomyopathy commonly complicates d-transposition of the great arteries with previous atrial switch repair. Cardiac resynchronization therapy (CRT) is an effective therapy but may be difficult to achieve conventionally via coronary sinus (CS) lead implantation.

Case summary: A 58-year-old man with a history of d-transposition of the great arteries and prior Mustard procedure presented with progressive dyspnea and QRS duration >200 milliseconds. Blepharospasm (BSP) is characterized by excessive orbicularis oculi muscle activity leading to abnormal blinking and involuntary eyelid closure. Botulinum neurotoxin (BoNT) injections are the main treatment for BSP, but they only partially and transiently relieve symptoms, leading to a waxing and waning therapeutic response. A patient-centered outcome (PCO) tool that measures BSP symptoms in a simple and efficient way could inform the development of better treatments. Using a stepwise modified Delphi approach, potential PCO items were first identified using the Dystonia Coalition Database with data from over 200 individuals with BSP who had provided responses to existing clinical assessment scales. …

Due to the paucity of longitudinal DNA methylation data (DNAm), especially among Hispanic/Latino adults, the association between changes in epigenetic clocks over time and cognitive aging phenotypes has not been investigated. This longitudinal study included 2671 Hispanic/Latino adults (57 years; 66% women) with blood DNAm data and neurocognitive function assessed at two visits ~7 years apart. Objectives: Intergenerational games offer a potential channel to impact parent-youth sexual health communication. The "Secret of Seven Stones" (SSS) is an 18-level online adventure game and parent website designed to engage parents and youth (11-14 years) in conversations about healthy dating relationships and sexual behavior and to provide sexual health skills training to youth. Study hypotheses were that SSS exposure would increase sexual health parent-child communication, increase youth intentions to delay sexual debut, and reduce youth exposure to situations that promote sexual activity.

Objective: To identify neurobehavioral symptom profiles among persons with chronic traumatic brain injury (TBI) using the Behavioral Assessment Screening Tool (BAST) and to consider participant characteristics that differ between profile groups.

Setting: Community.

Participants: Participants (n = 615) were English-speaking adults (≥18) and had a self-reported history of at least one TBI of any severity.

Design: Secondary analysis of cross-sectional data.

Main measures: The BAST measures neurobehavioral symptoms in the domains of Negative Affect, Fatigue, Executive Dysfunction, Impulsivity, and Substance Misuse.

Deep brain stimulation (DBS) of the ventral capsule and ventral striatum (VC/VS) is an effective therapy for treatment-resistant obsessive–compulsive disorder (trOCD). DBS initiation often produces acute improvements in mood and energy. These acute behavioral changes, which we refer to as “approach behaviors,” include increased social engagement and talkativeness. The type IX secretion system (T9SS) is a protein secretion machinery unique to the Bacteroidetes-Chlorobi-Fibrobacteres superphylum, which plays crucial roles in bacterial pathogenesis and gliding motility. It is composed of >15 proteins, including the proton-motive force-dependent PorLM motor, the PorKN ring anchored to the outer membrane, and the Sov translocon. Here, we present the cryo-electron microscopy (EM) structure of the PorKN ring complex from Porphyromonas gingivalis at 3.2 Å resolution. Our structural analysis reveals that PorK contains a repurposed formylglycine-generating enzyme-like fold, which serves as a structural hub for complex assembly rather than enzymatic activity. After stroke, microglia and hematogenous macrophages, together referred to as MΦ, clear dead cells and cellular debris in the infarcted brain through phagocytosis as an essential part of the recovery process. However, the phagocytic capability of MΦ declines with age. Furthermore, aged MΦ become overactivated in response to stroke, enhancing secondary brain injury. In this study, we demonstrated that by reversing the age-related dysfunctions in MΦ through activating the retinoid X receptor (RXR), the recovery after stroke in the aged brain could be improved. Background: While highly efficacious for numerous cancers, immune checkpoint inhibitors (ICIs) can cause unpredictable and potentially severe immune-related adverse events (irAEs), underscoring the need to understand irAE biology.

Methods: We used a multidimensional approach incorporating single-cell RNA sequencing, mass cytometry, multiplex cytokine assay, and antinuclear antibody (ANA) profiling to characterize the peripheral immune landscape of patients receiving ICI therapy according to irAE development.

The β-barrel assembly machinery (Bam) is essential for assembling all transmembrane β-barrel outer membrane proteins in gram-negative bacteria. The Bam complex consists of the central β-barrel protein BamA and accessory Bam lipoproteins, including the widely conserved and essential BamD. BamD is assumed to be an essential regulator of OMP assembly, as its absence causes a global defect in OMP biogenesis. Here, we challenge this view by demonstrating that BamD essentiality is both conditional and substrate specific. Experimental studies suggest that animal species not previously described as naturally infected by prions are susceptible to prion diseases affecting sheep, cattle, and deer. These interspecies transmissions may generate prions with unknown host ranges. Pigs are susceptible to prions from different origins, including deer chronic wasting disease (CWD), sheep scrapie, and bovine spongiform encephalopathy (BSE). Here, we studied prions in brains and muscles from pigs previously infected with these different prion sources. This study evaluates the effects of ZOA Energy Drink, a commercially available energy beverage, on muscular strength, sustained attention, and mood in exercise-trained individuals. Using a randomized, blinded, two-arm design, 24 participants (15 males, 9 females; age = 31.8 ± 8.4 years) were assigned to consume either ZOA Energy Drink or a caffeine-matched placebo. The primary outcomes included psychomotor vigilance (PVT), mood state (POMS), handgrip strength, and push-up performance. Baseline body composition was also assessed using bioelectrical impedance.

Indoor air pollution from gas stove combustion remains a public health concern, given links to adverse cardiorespiratory health effects, yet few studies have characterized or compared the air quality impacts of different gas-based cooking fuels. We investigated kitchen-level concentrations of nitrogen dioxide (NO2) and fine particulate matter (PM2.5) concentrations in four homes in Central Pennsylvania that used natural gas and/or biogas fueled stoves. We conducted time-resolved kitchen monitoring and assessed pollutant concentrations during cooking and non-cooking periods. • There is growing interest in less invasive management options for primary and adjunctive treatment, such as middle meningeal artery embolization (MMAE).

Severe aplastic anemia (SAA) is a rare and life-threatening bone marrow failure disorder. Immunosuppressive therapy (IST) with antithymocyte globulin and cyclosporine has long been a frontline treatment option in SAA; however, its limited durability and risk of long-term complications such as secondary malignancies remain a drawback in this treatment modality. Allogeneic hematopoietic cell transplantation (HCT) is a potentially curative option with significantly improved outcomes over the long term, particularly with HLA-matched related donors.
