Two cosegregating single-nucleotide polymorphisms (SNPs) in human TLR4, an A896G transition at SNP rs4986790 (D299G) and a C1196T transition at SNP rs4986791 (T399I), have been associated with LPS hyporesponsiveness and differential susceptibility to many infectious or inflammatory diseases. However, many studies failed to confirm these associations, and transfection experiments resulted in conflicting conclusions about the impact of these SNPs on TLR4 signaling. Using advanced protein modeling from crystallographic data of human and murine TLR4, we identified homologous substitutions of these SNPs in murine Tlr4, engineered a knock-in strain expressing the D298G and N397I TLR4 SNPs homozygously, and characterized in vivo …

Population Pharmacokinetics Of Ribavirin In Lung Transplant Recipients And Examination Of Current And Alternative Dosing Regimens, Eliza Milliken
Theses
Ribavirin is used in the treatment of respiratory paramyxovirus infection in lung transplant recipients; however, its pharmacokinetic profile in the transplant population is unknown despite the potential for alterations due to underlying pathology. Furthermore, the ability of current regimens to meet exposure targets has not been established. This study examined the pharmacokinetics of ribavirin in a lung transplant population from which current and alternate dosing regimens were assessed. Population pharmacokinetic modelling was conducted in NONMEM using concentrationtime data from 24 lung transplant recipients and 6 healthy volunteers. Monte Carlo simulation was used to assess the ability of dosing regimens to …
Active Rehabilitation In Sports-Related Concussions: An Educational Intervention For Primary Care Providers, Lior Habinsky
Active Rehabilitation In Sports-Related Concussions: An Educational Intervention For Primary Care Providers, Lior Habinsky
Doctor of Nursing Practice Projects
Background and Review of Literature: Sports-related concussion (SRC) is a form of traumatic brain injury caused by a blow to the head and remains a prevalent issue across the 11-18-year-old age group within the United States. Historically, total physical and cognitive rest has been the preferred method to help individuals recover after suffering a concussion. However, recent data have shown that incorporating active rehabilitation or aerobic exercise therapy as part of acute post-concussion treatment is safe. Further investigation into these methods could provide valuable insight into prognosis as well as promote clinical recovery.
Purposes/Goal: This project involved delivering an educational …

Improving Provider Confidence And Partnership With Lgbt Patients Through Inclusivity And Education, Katherine Rowe
Doctor of Nursing Practice Projects
The focus of this project utilizes patient recommendations for LGBT-inclusive intake practices to increase provider confidence in communication with LGBT patients using pre-and post-survey data comparing provider confidence before and after a 20-minute video education session. LGBT-inclusive provider education has been shown to be deficient if not nonexistent in most educational settings, yet, LGBT individuals have increased risk of negative health disparities, violence, and discrimination. Research examining the ability for small changes in provider education to enable positive health outcomes for this vulnerable and marginalized population is imperative for the quality of life of these communities. A quasi-experimental design utilized …
Evaluating The Feasibility Of Practice Change: The Use Of The Phq-2 In The Urgent Care Setting, Helena Laubach
Evaluating The Feasibility Of Practice Change: The Use Of The Phq-2 In The Urgent Care Setting, Helena Laubach
Doctor of Nursing Practice Projects
Adolescents in the United States may go unscreened for mental health disorders, such as depression, despite current recommendations. This population has an increased use of urgent care clinics (UCC) for their acute and primary care needs, where they might not be screened for depression at regular intervals. This project looked at determining the feasibility of implementing a two-question depression screener in the urgent care setting – the PHQ-2. Additional aims were to increase screening rates and to determine barriers to screening. Healthcare providers at a pediatric urgent care setting used the PHQ-2 to screen all adolescent patients over a two-week …

Exploring The Use Of A Mindfulness Based Yoga Intervention For Children With Severe Asthma, Sharon Lack
Exploring The Use Of A Mindfulness Based Yoga Intervention For Children With Severe Asthma, Sharon Lack
Theses and Dissertations
Introduction: A diagnosis of severe asthma greatly impacts the physical and psychological health of children. Often experiencing increased stress, decreased physical activity, and increased exacerbations, children with severe asthma frequently exhibit poor health outcomes. Mindful yoga is a complementary health approach that may improve the physical and psychological well-being of children with severe asthma. The aims of this research were to evaluate the tolerability, feasibility, acceptability, and preliminary effects of a mindful yoga intervention in children with severe asthma. A novel theoretical framework encompassing concepts from three vulnerability nursing theories guided the research.
Methods: A mixed methods pilot …

In the pediatric population, femoral neck fractures are usually associated with high-impact trauma and often present with pain in the groin area. Regional anesthesia can offer adjunctive therapy for acute pain management. Various techniques have been employed to circumvent pain related to hip fractures and resultant hip surgery. Neuraxial, lumbar plexus, caudal, epidural, fascia iliaca, and femoral continuous nerve block techniques are advantageous in mitigating hip pain. However, these approaches require patient repositioning during placement and carry the potential for motor blockade with resultant weakness. A newly described method, the Pericapsular Nerve Group (PENG) block, allows for analgesia of the …

BACKGROUND: Reducing the rising health-care burden associated with shoulder surgical site infection (SSI) is of paramount importance. The purpose of this study was to investigate the antimicrobial efficacy of protocatechuic acid (PCA) as a topical reagent for surgical skin antisepsis surrounding the shoulder joint.
METHODS: This was a 2-phase skin-disinfection trial involving the human shoulder. The shoulders of healthy volunteers were randomized to topical treatment with PCA (a 10% concentration of PCA in Phase I [11 subjects] and a 17% concentration in Phase II [12 subjects]), with a control of isopropyl alcohol (IPA) applied to the contralateral shoulder. Mechanical scraping …
College Campus Outreach To Increase Chlamydia Screenings, Elizabeth Kinney
College Campus Outreach To Increase Chlamydia Screenings, Elizabeth Kinney
DNP Projects
A common sexual health concern around the world is sexually transmitted infections (STI). Chlamydia is the most reported sexual health problem. The highest prevalence of chlamydia rates is among ages 15-24, which includes college age students (CDC, 2019). The lack of education provided to students prior to starting college by parents, health care providers and high school sources as well as inaccurate information obtained from peers and erroneous internet sources may lead to misconceptions about STIs and consequently the avoidance of routine screening among this high-risk population (Canan & Jozkowski, 2017).
The purpose of this project was to determine if …

DNP Projects
Abstract
BACKGROUND: Oversedation in the mechanically ventilated patient can cause long- and short-term adverse side effects. Light sedation practices provide patients with relief from agitation and pain and still allows them to actively participate in care. A light sedation protocol was introduced to bedside ICU registered nurses (RNs) at the University of Kentucky Medical Center (UKMC), but deep sedation is still commonplace. Reeducating bedside RNs and increasing their knowledge and confidence is a proposed strategy for increasing adherence to light sedation protocols.
PURPOSE: The purpose of this quality improvement project was to gain understanding of bedside RNs knowledge and perceived …

Multifaceted Interventions To Improve Hand Hygiene Compliance Among Nurses, Christopher Hoffman
Multifaceted Interventions To Improve Hand Hygiene Compliance Among Nurses, Christopher Hoffman
DNP Projects
The WHO has identified hand hygiene as the single “most important measure to avoid the transmission of harmful germs and prevent healthcare-associated infections” (World Health Organization [WHO], 2009) and subsequently created guidelines specifically focused on hand hygiene and prevention of hospital-acquired infections (HAIs). Nurses know and understand the importance of performing hand hygiene before and after patient contact, but often compliance is subpar. An extensive literature review was conducted for methods and interventions to improve hand hygiene compliance. The review of literature found seven systematic reviews which helped shape this study. The purpose of this study is to examine the …
Individualizing Patient Care During Percutaneous Coronary Intervention To Reduce Operator Risk Adjusted Bleeding Rates: A Pre/Post Interventional Study, Sarah Brown
DNP Projects
OBJECTIVE: It is well documented that the most common problems associated with diagnostic and interventional angiography are major bleeding and vascular complications. While previous research and the American College of Cardiology (ACC) recommend the use of bleeding risk stratification tools, there is little evidence related to the use of bleeding avoidance strategies in the high-risk for bleeding population. This study aims to determine if individualizing access site and anticoagulation strategies based on bleeding risk stratification would positively impact NCDR Risk-Adjusted Bleeding Rates.
METHODS: This was a single-center pilot study utilizing retrospective chart reviews with pre/post design. Data was collected on …

Increasing Medication Compliance In Pediatric Asthma Patients By Implementing A Home Medication Dosing Chart, Lindsey K. Shader
DNP Projects
Background: There are 6.2 million children under the age of 18 years who have an asthma diagnosis (Center for Disease Control and Prevention, 2017). Only 60% of children ages 7-17 years with persistent asthma maintain their prescribed inhaled corticosteroid (ICS) controller medication regimen (Arnold, Bixenstine, Cheng, & Tschudy, 2018). The purpose of this paper is to discuss a practice inquiry project that incorporated the implementation of an asthma medication dosing chart to increase compliance in pediatric asthma patients.
Method: The quasi-experimental pretest-posttest design evaluated the daily use of a medication dosing chart, which was to serve as a physical reminder …
Neonatal Abstinence Syndrome: Effectiveness Of Targeted Umbilical Cord Drug Screening In Determining Risk Of Withdrawal, Alecia Karr
DNP Projects
Neonatal Abstinence Syndrome (NAS) is a condition in which an infant experiences withdrawal from uterine exposure to various substances such as caffeine, nicotine, amphetamines, THC, opioids, benzodiazepines, and other types of substances. Depending on the severity of the symptoms, these infants may experience a longer hospital stay and may need treatment and monitoring in the neonatal intensive care unit (NICU), resulting in an increase in healthcare costs. The objective of this project was to determine if targeted drug screening of newborns was effective in determining infants at risk of NAS based on a positive screening result. This study utilized a …
