Objectives: Trauma is the leading cause of death in children. The lack of an accepted definition of what constitutes a high-quality stabilisation of a traumatically injured child has limited the evaluation of direct interventions in simulation-based education and service-delivery models to improve trauma care. The aim of this study was to create a framework that delineates quality by exploring the perceptions of the multi-disciplinary team providing and improving this initial care.

Methods: Interviews were conducted with 36 experienced UK trauma team members and governance administrators (clinical directors to executive board level), from three standard UK trauma units. This study used …

In January 2020, under the NextGen initiative put in place by the Federal Aviation Administration (FAA), all aircraft flying in controlled airspace, both general aviation and commercial operations, will be required to have an Automatic Dependent Surveillance – Broadcast (ADS–B) system installed and operational. However, this is not required for Title 14 Code of Federal Regulations (CFR) Part 135 operations taking place outside of controlled airspace. Part 135 of the Federal Aviation Regulations (FAR) refers to aircraft operating as on-demand or air taxi services. BACKGROUND: Sepsis accounts for 30% to 50% of all in-hospital deaths in the United States. Other than antibiotics and source control, management strategies are largely supportive with fluid resuscitation and respiratory, renal, and circulatory support. Intravenous vitamin C in conjunction with thiamine and hydrocortisone has recently been suggested to improve outcomes in patients with sepsis in a single-center before-and-after study. However, before this therapeutic strategy is adopted, a rigorous assessment of its efficacy is needed.

METHODS: The Vitamin C, Thiamine and Steroids in Sepsis (VICTAS) trial is a prospective, multi-center, double-blind, adaptive sample size, randomized, placebo-controlled trial. It will enroll …

Lean Thinking was pioneered during the 1980’s by the Toyota Motor Company as a method of process improvement for their production lines. Since the early 2000’s, there have been published reports of using Lean to redesign healthcare systems. Its effectiveness as a quality improvement method for healthcare has been contested due, in part, to our limited contextual understanding of how Lean affects the working conditions, and clinical workflow, of healthcare professionals. The objective of this dissertation was to explore how a Lean intervention may impact clinical work, and within what contexts.

Timely feedback is critical in promoting learner self-reflection. When provided effectively, feedback can assist learners with the acquisition of new skills and knowledge in the ever-changing and complex landscape of healthcare. While the literature is replete on methods to provide feedback, faculty and supervisors receive little, if any, training on writing constructive feedback. Abbreviated comments (i.e., 'good job' or 'read more') provide little information on specific behaviors learners can change. Introduction: The emergency department plays a critical role in the trajectory of in-hospital patient care. The Center to Advance Palliative Care (CAPC) along with the American College of Emergency Physicians (AECP) recommend early referral to palliative care for patients with life limiting illnesses that present to the emergency department. Research demonstrates that early palliative care consultations result in patient directed goals of care, improved symptom management, decreased length of stay, and reduced direct costs for inpatient hospitalizations.

Road traffic injuries are a growing public health problem throughout the world. Kenya has is known for having unsafe roads with an estimated 13,463 road traffic fatalities in 2016 and 24% of road traffic fatalities coming from 2-and 3-wheeled vehicles (WHO, 2018). This study sought to analyze road safety measures and road traffic injuries amongst motorbike taxi drivers in Kisumu, Kenya. A qualitative survey was administered to 152 motorbike taxi drivers in Kisumu over a two-week period in April 2019. The results were then analyzed. Healthcare providers in emergency departments (EDs) face a daunting daily task: providing health care in a triage setting to a diverse group of patients many with complex medical issues. Many patients rely on ED services out of financial necessity, when their healthcare issues could be better suited for care from a primary care physician. Many of these already vulnerable patients – minorities, those health illiterate, low-income, uninsured and those with language barriers – must also deal with ED overcrowding and staffing conditions.
