A Career Advancing Midwifery
Thomas Jefferson University Research Magazine
The year 2019 was a significant one—even for a clinician-researcher as accomplished as Barbara Hackley, PhD, CNM, associate professor of midwifery and women’s health and the Dorothea Lang Term Chair. Prior to joining Jefferson in 2017 to launch and direct the Jefferson Doctor of Midwifery Program— the nation’s first discipline-specific doctoral program for midwifery—Dr. Hackley served on the faculties of Georgetown, Columbia and Yale universities. And, during a career that has, thus far, spanned three decades, she received countless awards for her teaching and research—and for her clinical expertise in expanding mental health care, immunization, asthma care and obesity management …

The primary objective was to examine the role of pelvic fluid observed during transvaginal ultrasonography (TVS) in identifying ovarian malignancy. A single-institution, observational study was conducted within the University of Kentucky Ovarian Cancer Screening trial from January 1987 to September 2019. We analyzed true-positive (TP), false-positive (FP), true-negative (TN), and false-negative (FN) groups for the presence of pelvic fluid during screening encounters. Measured outcomes were the presence and duration of fluid over successive screening encounters. Of the 48,925 women surveyed, 2001 (4.1%) had pelvic fluid present during a TVS exam. The odds ratio (OR) of detecting fluid in the comparison …

Because the effects of age, menopausal status, weight and body mass index (BMI) on ovarian detectability by transvaginal ultrasound (TVS) have not been established, we determined their contributions to TVS visualization of the ovaries. A total of 29,877 women that had both ovaries visualized on their first exam were followed over 202,639 prospective TVS exams. All images were reviewed by a physician. While visualization of both ovaries decreased with age, one or both ovaries could be visualized in two of every three women over 80 years of age. Around 93% of pre-menopausal women and ~69% of post-menopausal women had both …

Do Empowered Women Receive Better Quality Antenatal Care In Pakistan? An Analysis Of Demographic And Health Survey Data, Muhammad Asim, Waqas Hameed, Sarah Saleem
Community Health Sciences
Introduction: Quality antenatal care is a window of opportunity for improving maternal and neonatal outcomes. Numerous studies have shown a positive effect of women empowerment on improved coverage of maternal and reproductive health services, including antenatal care (ANC). However, there is scarce evidence on the association between women's empowerment and improved ANC services both in terms of coverage and quality. Addressing this gap, this paper examines the relationship between multi-dimensional measures of women empowerment on utilization of quality ANC (service coverage and consultation) in Pakistan.
Methods: We used Pakistan Demographic and Health Survey 2017-18 (PDHS) data which comprises of 6,602 …

Background: Maternal undernutrition is critical in the etiology of poor perinatal outcomes and accounts for 20% of small-for-gestational-age (SGA) births. High levels of food insecurity, antenatal undernourishment, and childhood undernutrition necessitate the supplementation of fortified balanced energy protein (BEP) during pregnancy in low-income settings especially with scarce literature available in this subject. Hence, this paper extensively covers the protocol of such a trial conducted in an urban slum of Karachi, Pakistan.
Methods: The trial is community-based, open-labelled, four-arm, and randomized controlled that will include parallel group assignments with a 1:1:1:1 allocation ratio in low-income squatter settlements in urban Karachi, Pakistan. …

Purpose/Background: Homologous recombination deficiency (HRD) is a useful predictor of treatment response in patients with epithelial ovarian cancer (EOC). Reported data on the frequency of HRD in EOC is largely based on analysis of patients treated at academic medical centers or who participated in clinical trials. We sought to characterize the frequency of HRD based on mutations in homologous recombination repair (HRR) genes, genomic instability (GI) and loss of heterozygosity (LOH) scores in a large community-based cohort of EOC patients who received genomic testing in the context of routine clinical care.
Methods: Information including patient demographic, tumor stage and histology …

BACKGROUND: Simulation research that seeks to solve the problem of silence among interprofessional teams has focused almost exclusively on training subordinate team members to be more courageous and to speak up to team leaders using direct challenge scripts despite the great interpersonal cost. Consequently, the existing literature overemphasizes the responsibility of subordinate team members for speaking up and fails to consider the role and responsibilities of team leaders in sustaining silence. The purpose of this study is to identify and describe the subtle behaviors and actions of team leaders that both promote and discourage speaking up.
METHODS: This study used …

Background: We determined the impact of gestational age (GA) from near term to term to post-term on birth/placental weight ratio and cord oxygen values with implications for placental transport efficiency for oxygen, fetal O2 consumption relative to delivery or fractional O2 extraction, and oxygen margin of safety.
Materials and methods: A hospital database was used to obtain birth/placental weight ratios, cord PO2 and other information on patients delivering between Jan 1, 1990 and Jun 15, 2011 with GA > 34 completed weeks (N=69,852). Oxygen saturation was calculated from the cord PO2 and pH data, while fractional O2 extraction was calculated from …

Uterine Fibroid Prevalence And Health Care Disparities By County. Florida, 2010-2019, Saarah K. Sherifi

Also known as uterine fibroids, leiomyomas occur in 70% of women. A uterine leiomyoma is a benign growth in the muscular wall of the uterus which, if left untreated, can grow, and affect surrounding organs, leading to complications, including hemorrhage and death. Symptomatic women often experience pain and debilitating menstrual cycles which, in turn, result in poor health, poor quality of life, and loss of income. Preventative and treatment services in Florida could benefit from the characteristics of women affected the most by this disease in the state. Thus, we describe the demographic characteristics and estimate the prevalence rate of …

Background: Endometriosis, a medical condition in which endometrial tissue lining grows outside of the uterus, affects approximately 10-15 percent of women of reproductive age. While research has shown that symptoms of endometriosis negatively affect quality of life in women, few studies have examined the impact of this condition specifically in college students who may be overwhelmed with the stress of school, living independently, and managing the symptoms of their endometriosis. Purpose: To examine the impact of endometriosis on academic and psychosocial factors in university students. Methods: Four college females completed the Pain Self-Efficacy Questionnaire (PSEQ), a 10-item survey used to …

Obstetrical Nursing Care And Cesarean Section Delivery Rates In Laboring Women: An Integrative Review Of Literature, Ayana Cornelius
Honors Undergraduate Theses
Background: There is a documented overuse of C-sections (CS) in the United States. The majority of research surrounding CS overuse focuses on the physician’s role, location (state, city, hospital), hospital policies, and the reason why the CS is being performed. Little research focuses on the nurse’s influence on CS outcomes, although nurses spend the longest amount of time with the patient. Methods: Data for this literature review examined 13 research articles between 1992 to present, exploring factors relating to the overuse of CS, specifically, data relating to the correlation between CS and the nurse’s role. Results: Recent research has shown …

Problem: Vulvodynia, an unexplained vulvar pain, is a medical condition affecting women of all ages, races, and ethnicities and causes pain levels ranging from very mild discomfort to extreme suffering.
Purpose: The purpose of this study was to explore women’s knowledge toward seeking care for vulvodynia and to explore the different types of treatments prescribed after diagnosis. The secondary purpose of this review was to describe the treatments most effective in reducing pain caused by vulvodynia.
Methods: A review of the literature was conducted using articles from 2001 to 2021 that focused on diagnosis of vulvodynia and the relief of …
