Radiographic Features And Remodeling Of Proximal Radius Osteomyelitis In A 14-Month-Old Child With Long-Term Follow-Up: A Case Report, Christopher L. Shultz, Deana M. Mercer

Pediatric primary hematogenous osteomyelitis is uncommon. Studies have been mainly small case series with limited clinical and radiographic followup. We present a child with primary hematogenous osteomyelitis and osteolysis of the proximal radius. Inflammatory laboratory values were normal, and blood and elbow aspirate cultures were negative for osteomyelitis. Diagnosis was made by findings of magnetic resonance imaging. The patient was successfully treated with intravenous antibiotics. At 4-year follow-up, she had normal range of motion and near symmetric proximal radial morphological features radiographically. To our knowledge, this is the most long-term radiographic follow-up of a patient with spontaneous osteomyelitis. The reported …

Periprosthetic proximal and pertrochanteric femur fractures around a well fixed hip resurfacing implant present a challenge to orthopaedic surgeons, as they do not allow the most biomechanically favorable fixation constructs in patients with otherwise highdemand levels of physical activity. A 65-year-old man hit a tree while skiing. He presented to our emergency department with a right-sided pertrochanteric fracture of the proximal femur beneath an implant used in prior hip resurfacing. Postoperative complications of trigger finger, a type of tendon entrapment, are low, and there is little information regarding evaluation after a complication occurs. Recurrent trigger finger is a rare occurrence, but often requires additional procedures to relieve symptoms. Here we present a case of recurrent trigger finger in the immediate postoperative period after the patient developed a cyst distal to the A1 pulley and synovitis. The first procedure demonstrated a thickened and tight A1 pulley as well as synovitis around the tendon. Pantalar dislocation occurs when the talus is dislocated from the ankle, subtalar, and talonavicular joints. An extruded talus is seen when the talus protrudes out of an open wound with little or no soft-tissue attachment. We describe a 62-year-old man who underwent debridement and irrigation for treatment of a pantalar dislocation with an extruded talus. About 1 year postoperatively, he underwent a below-knee amputation. This case highlights the difficulties and complications associated with this injury.

Laurin-Sandrow syndrome (LSS) is a rare condition with only a few published cases in studies. This syndrome, originally described by Laurin in 1964 and again by Sandrow in 1970, can involve digit duplication with mirror hand and foot deformity, ulnar and tibial dimelia, and abnormalities of the knee, ankle, and nasal passages. A 7-month-old male infant presented to our clinic with limited function of his hands and feet due to multiple abnormalities. Acute gluteus medius and minimus tears are an infrequent and possibly underdiagnosed cause of lateral hip pain. When reported, they most commonly occur at the distal tendinous attachment at the insertion on the greater trochanter or at the musculotendinous junction. It is rare for muscle belly or proximal muscle tears to occur. They are more prevalent in the setting of degenerative joint disease, but cases have also been described in previously healthy athletes without preexisting joint or muscle disease. Background: Symptomatic stage 2 or 3 scapholunate advanced collapse (SLAC) wrist is aggressively treated with salvage procedures, such as proximal row carpectomy or partial wrist fusion with resultant pain relief but limited motion. We hypothesize that arthroscopic synovectomy, radial styloidectomy, and neurectomy will preserve wrist motion, relieve pain, and delay or avoid salvage procedures. Methods: We evaluated outcomes in 13 wrists through questionnaires and 11 of these through additional physical examination at a mean follow-up of 5.0 years. Methods:: A survey was distributed to members of the American Shoulder and Elbow Society (ASES), consisting of three sections—demographics, case scenarios, and general LHBT pathology management. OBJECTIVE: The purpose of this review was to identify risk factors, prognostic factors, and comorbidities associated with common spinal disorders.

METHODS: A scoping review of the literature of common spinal disorders was performed through September 2016. To identify search terms, we developed 3 terminology groups for case definitions: 1) spinal pain of unknown origin, 2) spinal syndromes, and 3) spinal pathology. We used a comprehensive strategy to search PubMed for meta-analyses and systematic reviews of case-control studies, cohort studies, and randomized controlled trials for risk and prognostic factors and cross-sectional studies describing associations and comorbidities.

Objectives

Little is known about patterns and correlates of Complementary Health Approaches (CHAs) in chronic pain populations, particularly in rural, underserved communities. This article details the development and implementation of a new survey instrument designed to address this gap, the Complementary Health Approaches for Pain Survey (CHAPS).

Design

Following pilot-testing using pre-specified criteria to assess quality and comprehension in our target population, and after feedback regarding face-validity from content experts and stakeholders, the final cross-sectional self-report survey required 10–12 minutes to com- plete. Given the high prevalence of anxiety in adults with arthritis and other rheumatic diseases (AORD) and the subsequent need for interventions to reduce anxiety, this brief report sought to determine if evidential value exists to support the role of exercise for reducing anxiety in adults with AORD. Methods. Utilizing data from a prior meta-analysis, a recently developed approach, P- curve, was used to determine evidential value by assessing for publication bias and p-hacking. Traditional above-the-knee amputation prosthetics utilize a stump-socket interface that is well-known for skin/socket problems, sitting difficulty, disuse osteopenia, and increased work of ambulation. As a result, we evaluated a novel osseointegrated transcutaneous implant in a large animal. The implant was designed to promote osseointegration at the bone-implant interface and minimize complications. As proof of concept, four Dorset sheep underwent a two-stage surgery for forelimb placement of an osseointegrated transcutaneous implant utilizing Compress�� technology (Biomet, Inc., Warsaw, IN). © 2018 Prof. PK Surendran Memorial Education Foundation Purpose: Our aim was to identify predictors of construct selection and recent trends for arthroscopic knotless rotator cuff repair (RCR). Methods: A manual review of 1617 operative reports was performed. Results: A medium-sized tear had a threefold increase in odds of single row (SR) knotless repair (OR, 6.91; p = 0.009) versus SR knotted (OR, 3.05; p = 0.003). Generalist orthopaedic surgeons were 79% less likely to perform SR knotless repairs versus sports medicine trained specialists (p < 0.001). Conclusion: There was a significant increase from 2009 to 2016 in SR knotless and double row medial row knotless constructs contrasting the declining use of the SR knotted technique.

Background: Disproportionate end-range plantar flexion weakness, decreased passive stiffness, and inability to perform a heel rise on a decline after Achilles tendon repair are thought to reflect increased tendon compliance or tendon lengthening. Since this was first noted, we have performed stronger repairs and avoided stretching into dorsiflexion for the first 12 weeks after surgery.

Hypothesis: Using stronger repairs and avoiding stretching into dorsiflexion would eliminate end-range plantar flexion weakness and normalize passive stiffness.

Plasma cell survival and the consequent duration of immunity vary widely with infection or vaccination. Using fluorescent glucose analog uptake, we defined multiple developmentally independent mouse plasma cell populations with varying life- spans. Long-lived plasma cells imported more fluo- rescent glucose analog, expressed higher surface levels of the amino acid transporter CD98, and had more autophagosome mass than did short-lived cells. Low amino acid concentrations triggered re- ductions in both antibody secretion and mitochon- drial respiration, especially by short-lived plasma cells. To explain these observations, we found that glutamine was used for both mitochondrial respira- tion and anaplerotic reactions, yielding …

Background: The purpose of this study was to investigate the influence of tranexamic acid (TXA) on functional outcomes in the immediate postoperative period after total knee arthroplasty (TKA). We hypothesized that the known benefits of TXA would confer measurable clinical improvements in physical therapy (PT) performance, decrease pain, and decrease hospital length of stay (LOS).
