Expandable Total Humeral Replacement In A Child With Osteosarcoma, Eric R. Henderson, Jidi Gao, John Groundland, Odion Binitie, G. Douglas Letson

Case. A right-handed 8-year-old female patient presented with a conventional, high-grade osteosarcoma involving her right humerus; through-shoulder amputation was recommended. After consultation, total humerus resection with expandable, total humeral endoprosthesis reconstruction was performed with a sleeve to encourage soft-tissue ingrowth. At three-year follow-up she has received one lengthening procedure and her functional scores are excellent. Conclusion. Total humeral resection and replacement in the pediatric population are rare and although early reports of expandable total humeral endoprosthesis outcomes demonstrate high failure rates, this patient’s success indicates that expandable total humeral replacement is a viable option.

Background: Previous studies investigating the windmill softball pitch have focused primarily on shoulder musculature and function, collecting limited data on elbow and forearm musculature. Little information is available in the literature regarding the forearm. This study documents forearm muscle electromyographic (EMG) activity that has not been previously published.

Purpose: Elbow and upper extremity overuse injuries are on the rise in fast-pitch softball pitchers. This study attempts to describe forearm muscle activity in softball pitchers during the windmill softball pitch. Overuse injuries can be prevented if a better understanding of mechanics is defined.

Background: Numerous studies have shown that there are better alternatives to log rolling patients with unstable spinal injuries, although this method is still commonly used for placing patients onto a spine board. No previous studies have examined transfer maneuvers involving an injured football player with equipment in place onto a spine board.

We tested if picosecond electric pulses (psEP; 190 kV/cm, 500 ps at 50% height), which are much shorter than channel activation time, can activate voltage-gated (VG) channels. Cytosolic Ca2+ was monitored by Fura-2 ratiometric imaging in GH3 and NG108 cells (which express multiple types of VG calcium channels, VGCC), and in CHO cells (which express no VGCC). Trains of up to 100 psEP at 1 kHz elicited no response in CHO cells. However, even a single psEP significantly increased Ca2+ in both GH3 (by 114 +/- 48 nM) and NG108 cells (by 6 +/- 1.1 nM). Background: Breast cancer treatments in women with breast cancer often result in physical impairments that lead to activity limitations and participation restrictions. These limitations and restrictions manifest in impaired functional mobility skills that may impact survivorship. Thus, evaluation of functional mobility is an important part of survivorship care.

Objectives: To quantify and determine the effects of Pilates on core endurance, hamstring flexibility, balance, body composition/mass and perceived stress level in healthy college age females.

Study Design: Randomized controlled trial design.

Background: Emerging research on the Pilates technique is inconclusive regarding benefits to core endurance, flexibility, balance, body mass, and perceived stress.

In the United States, ankle fusion is more commonly performed than total ankle arthroplasty (TAA) for treatment of degenerative joint disease of the ankle. Background: It remains unclear whether pediatric trauma, including fracture, occurs more frequently at a certain time of the year. We developed a database based on the University of New Mexico orthopaedics consult log (OCL) with the aim of determining whether pediatric fractures have a seasonal predilection.

Methods: The OCL for 2009 and 2010, representing a total of 2385 patient visits, was reviewed, and data on patients who were 17 years of age or younger who were treated for trauma were collected. The months and seasons of the year in which the trauma occurred were recorded.

Background: Although experience within the operating room can help surgeons learn simple bone-drilling techniques, outside training may be better suited for complex procedures. We adapted a rotary handpiece with dual trigger and drilling attachments to a haptic device to train and evaluate motor skills of orthopaedic resident physicians outside of the operating room.

Background: Partly threaded cannulated screws (PTCS) are currently recommended for management of slipped capital femoral epiphysis (SCFE), but no advantage of these screws over fully threaded cannulated screws (FTCS) has been demonstrated. We investigated whether PTCS are more difficult to remove than FTCS.

Methods: The records of 33 patients who underwent implant removal after operative treatment of SCFE were reviewed, and the following data were collected: age and sex of the patient, type of screw used (partly or fully threaded), length of removal operation, and any comments on the difficulty of screw removal.

Single- and dual-component revisions have typically been used to correct coronal malalignment after total knee arthroplasty (TKA). However, realignment of well-fixed components can result in complications such as bone loss and the need for increasing constraint of implants. Exchange to a custom-made angled polyethylene insert, although primarily implemented in cases of osteolysis, has been described with low complication rates and minimal loss of motion. Lipomas are benign tumors composed of adipose tissue and typically encountered in middle-aged patients. Although the etiology remains unknown, this lesion can occur throughout the body. However, it is rarely seen in the finger. Typical treatment has often involved observation, but excision techniques have also reported successful outcomes. • Adolescent idiopathic scoliosis (AIS) is the most common form of scoliosis. It affects 2-4 percent of adolescents aged 10-16 years. Although most adolescents diagnosed with scoliosis will not develop clinical symptoms, ten percent will progress and require medical intervention due to the potential for rib deformity and respiratory compromise as well as significant emotional distress due to aesthetic changes in appearance. Primary care providers should be prepared to evaluate and recommend treatment when idiopathic scoliosis is discovered or addressed by the patient. Study Design: Retrospective study. Objectives: To determine the reliability of measuring the perpendicular distance between the posterolateral acromion process and examination table to estimate passive shoulder protraction in student physical therapists.

Background: Previous research has identified scapular dyskinesis to be associated with musculoskeletal shoulder pain, and commonly addressed by physical therapists. Tightness of the anterior shoulder muscles may be associated with scapular movement abnormality. Adequate neuromuscular control of the lumbar spine is required to prevent lumbar injuries. A trunk postural control test protocol, controlling for the influence of body size on performance, was implemented to carry out between-subject comparisons. The enhanced differentiation of mesenchymal cells into chondrocytes or osteoblasts is of paramount importance in tissue engineering and regenerative therapies. A newly emerging body of evidence demonstrates that appendage regeneration is dependent on reactive oxygen species (ROS) production and signaling. Thus, we hypothesized that mesenchymal cell stimulation by nonthermal (NT)-plasma, which produces and induces ROS, would (1) promote skeletal cell differentiation and (2) limb autopod development. Bone grafting procedures are performed to treat wounds incurred during wartime trauma, accidents, and tumor resections. Endogenous mechanisms of repair are often insufficient to ensure integration between host and donor bone and subsequent restoration of function. We investigated the role that bone marrow-derived cells play in bone regeneration and sought to increase their contributions by functionalizing bone allografts with bioactive lipid coatings. Polymer-coated allografts were used to locally deliver the immunomodulatory small molecule FTY720 in tibial defects created in rat bone marrow chimeras containing genetically-labeled bone marrow for monitoring cell origin and fate.
