RATIONALE: Postoperative pneumonia is three to four times more frequent in patients with alcohol use disorders followed by prolonged intensive care unit (ICU) stay. Long-term alcohol use leads to an altered perioperative hypothalamus-pituitary-adrenal (HPA) axis and immunity. OBJECTIVES: The aim of this study was to evaluate HPA intervention with low-dose ethanol, morphine, or ketoconazole on the neuroendocrine-immune axis and development of postoperative pneumonia in long-term alcoholic patients. METHODS: In this randomized, double-blind controlled study, 122 consecutive patients undergoing elective surgery for aerodigestive tract cancer were included. Abstract

Carcinosarcoma of the lung is a malignant tumor composed of a mixture of carcinoma and sarcoma elements. The carcinomatous component is most commonly squamous followed by adenocarcinoma. The sarcomatous component commonly comprises the bulk of the tumor and shows poorly differentiated spindle cell features. Foci of differentiated sarcomatous elements such as chondrosarcoma and osteosarcoma may be seen. Aspergillus pneumonia is the most common form of invasive aspergillosis and occurs mainly in patients with malignancy, immunocompromizing or debilitating diseases. In practice, TTC is diagnosed almost exclusively either via (1) emergency cardiac catheterisation, primarily for apparent 'STEMI', or (2) recognition of a potentially precipating stress, primarily in patients with a non-ST-elevation (N-STE) ECG pattern. We therefore hypothesized that the greatest delays in diagnosis would occur in the subgroup with N-STE and no known stressor. A seasonality analysis was also undertaken, to test a possible association between heat stress and TTC.

Electrocardiographic changes previously described in Tako Tsubo Cardiomyopathy (TTC) include T-wave inversion with QTc prolongation, but the relationship of these changes to the evolution of TTC is uncertain. Furthermore, torsade de pointes (TdP) has been reported to occur, but its relationship to QT prolongation in TTC is unclear.

Pulmonary artery embolism (PE) is a rare and life-threatening complication in the neonate. We describe two previously stable infants who incurred respiratory failure from PE. The first case is a late preterm infant with gastroschisis with autopsy confirmation of PE presumably as a result of a central venous line (CVL) with thromboembolism. The second case is an extreme low birth weight infant with sudden onset of respiratory failure, abnormal chest x-ray, disseminated intravascular coagulation (DIC), and echocardiographic confirmation of a thrombus at the junction of the superior vena cava and right atrium (SVC/RA) at the site of a previous CVL. …
