Aims: Airway remodelling is a characteristic feature of asthma and is a dynamic process involving extracellular matrix (ECM) production and its degradation. In this regard, the matrix metalloproteinases (MMPs) and their specific inhibitors known as tissue inhibitors of metalloproteinases (TIMPs) have been shown to be important in this process. Aim: In this paper, we present the hematological findings of 787 cases diagnosed with acute brucellosis. Materials and Methods: Records of the cases seen between 1994 and 2006 were retrospectively investigated regarding hematological changes. Results: Three hundred sixty-five (46%) patients were male and 422 (54%) were female. Mean age was 32 (range: 11-78 years). Mean hematological values were as follows: Hb 12 g/dl (4-19), Htc 36% (12-56), leukocyte 6.9 x 10^9/L (0.5-25) and platelets 213 x 10^9/L (9-617). According to differential counts, 34% and 14% of patients had mononuclear cell and neutrophil dominance, respectively. Fifty-six percent of patients were found …

Aim: The aim of this study was to establish a classification that will aid in the surgical treatment and prognosis of sigmoid volvulus (SV). Materials and Methods: The records of 420 patients with SV who were treated surgically during a 41.5-year period were reviewed retrospectively. The preoperative and operative criteria that were significantly correlated with mortality were determined. Results: Of 420 patients, 68 (16.2%) died. Age over 60 years (P < 0.001), associated disease (P < 0.001), presence of shock (P < 0.001), and presence of sigmoid colon gangrene (P < 0.001) were significantly correlated with mortality. A classification was made using these criteria. Conclusions: Recommended treatment options and potential mortality rates were determined for each class in the new classification based on the evaluation of the present series, our clinical experience, and literature findings. This classification may be helpful in determination of the surgical treatment and prognosis of SV.

Aims: To show the efficacy of oral contraceptive (OC) treatment for hospitalized adolescent patients with abnormal uterine bleeding (AUB). Materials and Methods: Thirty-one girls with AUB and anemia hospitalized within the last two years were included. The patients´ medical records were evaluated retrospectively. All patients were given combined OC containing 0.03 mg ethinyl estradiol plus 0.15 mg desogestrel according to a defined protocol. Results: Among 31 patients, 8 (25.8%) had bleeding disorders: 3 (9.7%) had factor VII deficiency, 3 (9.7%) thrombocytopenia, 1 (3.2%) factor VIII deficiency and 1 (3.2%) von Willebrand disease. Aim: To document the efficacy of low-dose long-term oral macrolide treatment in the management of sinonasal polyposis. The study was designed prospectively in a group of patients with sinonasal polyposis and negative history of surgery. Materials and Methods: Ten patients were administered roxithromycin (RXM) 150 mg/day single oral dose for eight weeks. All patients underwent Tc-99m rhinoscintigraphic evaluation of mucociliary activity and endoscopic evaluation before and after the treatments. Aims: The etiology of stroke in young adults may be challenging. After preliminary investigations, a cause is not established in about one-third of patients. Cardiovascular embolism has been recognized as the cause of unexplained stroke or transient ischemic attack in a relevant number of young stroke patients. Acute bacillary dysentery (shigellosis) occurs due to infections with shigella sp. The spectrum of acute shigellosis ranges from mild watery diarrhea to severe dysentery. Surgical complications of shigellosis described in children have included appendicitis with or without perforation, colonic perforation, intestinal obstruction, and intraabdominal abscess. Acute appendicitis and infective diarrheal illness can be difficult to differentiate, particularly in children. Thus, patients must be followed up closely until the differentiation between bacterial gastroenteritis and acute appendicitis can be made. We report herein a 10-year-old boy who had acute appendicitis due to Shigella flexneri.

Hemophagocytic syndrome (HPS) is a disorder that might be transferred genetically due to an autosomal recessive genetic defect of the long arm of chromosome 9 and chromosome 10 (9q21.3-22 and 10q21-22), termed as primary hemophagocytic syndrome (PHPS) or familial hemophagocytic hemophagocytosis (FHPS) and it may be associated with a variety of infections, malignant neoplasms, drugs, autoimmune diseases and various immuno deficiencies, termed as secondary hemophagocytic syndrome (SHPS). Aim: Regular physical exercise is associated with reduced risk of cardiovascular diseases. In this study, the hypothesis that acute submaximal exercise has similar effects on rheological parameters of smokers and non-smokers was tested. Materials and Methods: Thirty-three male university undergraduates comprised of 18 smokers and 15 non-smokers were studied. All the subjects underwent submaximal exercise on cycloergometer for 30 minutes. Blood for hemorheological parameters was collected 30 minutes before and after exercise. The Lanz® endotracheal tube is a new kind of tube with a pressure-regulating valve. Aim: While there have been efforts to slow the recent declining trend in autopsy practice in the world, in Turkey there is already a low level of medical autopsy audit, practically nearing zero. In this study, we determine and evaluate the autopsy status in Turkey; reveal pathologists' attitudes towards autopsy practice; propose several factors to explain current autopsy audit; and discuss differences in Turkey's autopsy practice with that of the rest of the world. Aim: The aim of the study was to investigate nasopharyngeal colonization of respiratory tract pathogens and nasal carriage of methicillin-resistant Staphylococcus aureus (MRSA) in healthy schoolchildren (6-14 years old). Materials and Methods: The present study was conducted at Konuralp Primary School by Düzce University and 484 healthy children were included. Isolated tuberculous inguinal lymphadenitis is a rare entity and is usually bilateral. A 16-year- old male presented with anorexia, nocturnal sweating and painless soft right inguinal lymph nodes. The patient underwent excision of lymph nodes and a Ziehl-Neelsen stain revealed the presence of acid-fast bacilli, but cultures of the lymph nodes were negative. Histological sections showed the presence of caseous necrosis and epithelioid cell granuloma. He had a positive tuberculin skin test (Mantoux) with 22 mm diameter. The patient was diagnosed as tuberculous lymphadenitis with these findings. Tuberculosis should be included in the differential diagnosis in any case of isolated …

Aim: The purpose of this study was to compare the water sorption and solubility of four provisional, three permanent luting cements and five restorative cements. Methods: A split ring mould was fabricated for the preparation of specimen discs which were 15.0 mm in diameter and 1.5 mm thick. All specimens were manipulated according to the manufacturer's instructions and then subjected to water sorption and solubility tests. Data were analysed with Wilcoxon Signed Ranks Test. Aicardi syndrome is a rare X-linked dominant genetic disorder characterized by infantile spasms, agenesis or hypogenesis of the corpus callosum, and chorioretinal lacunae. There remain many unresolved questions regarding the clinical features of this disorder. Recent advancements in neuroimaging techniques have allowed us to obtain information about brain development and perfusion. The commonest and most serious complication of neonatal ovarian cysts is torsion. Ovarian torsion leads to loss of the ovary. We report a case of a torsioned and autoamputated right ovarian cyst presenting as a duplication cyst of the intestine in a newborn. An abdominal cystic mass 4.8 x 3.4 cm in diameter was diagnosed in a 36-week-gestation fetus using antenatal ultrasonography (US). Metformin is an oral hypoglycemic agent that is commonly used in the treatment of type 2 diabetes mellitus. While metformin-associated metabolic acidosis is a widely recognized side effect of this drug, metformin-induced hepatotoxicity has been rarely reported in the literature. We present herein the case of a 52-year-old male in whom metformin-associated lactic acidosis and metformin-induced hepatotoxicity developed.

Schwannoma of the cervical sympathetic chain is a rare nerve tumor. These lesions typically present as an asymptomatic neck mass and are easily mistaken for a carotid body tumor during the initial work-up. In this report, a rarely seen huge cervical sympathetic chain schwannoma case, who experienced partial Horner´s syndrome postoperatively, is presented. We report a case of schwannoma on the cervical sympathetic chain, which to our knowledge is the largest reported in the current literature.

Aim: In this study, we aimed to investigate the effect of in-and-out urethral catheterization on serum total and free prostate specific antigen (PSA) levels and free/total PSA ratio. Materials and Methods: This is a prospective study including 24 patients who admitted with lower urinary tract symptoms and underwent in-and-out urethral catheterization with 18 F Foley catheter inserted for postvoiding residual urine measurement. Congenital gastrointestinal duplications are rare, and gastric duplication cysts are even rarer. Most cases are seldom diagnosed preoperatively. We present a case of gastric duplication cyst, which in preoperative imaging studies mimicked a hydatid/pseudocyst of the pancreas. The diagnosis was made only intraoperatively and was confirmed by histopathological examination of the completely excised cyst.
