Current Trends And Future Implications In Sepsis Treatment, Bariş Doğu Yildiz, Kaya Yorganci

Turkish Journal of Medical Sciences

Sepsis is a systemic illness caused by invasion of tissues by pathogens. It is highly lethal, causing circulatory failure and death. There has been extensive research trying to identify possible therapies for sepsis. Apart from source control, and antibiotic and supportive therapy, glucocorticoids, anticoagulant agents, and immunoglobulins have been tried in both the experimental setting and in septic patients. Newer strategies include novel anticoagulant agents like ethyl pyruvate, statins, insulin receptor modulators and Toll-like receptor modulators. This review article summarizes both the current therapies and endeavors for future treatment in sepsis.

Aim: In this study, we aimed to assess the expression of CD117 in a large series of both Hodgkin´s and non-Hodgkin´s lymphoma, and to investigate the association between the expression patterns of CD117, IRF4/MUM1, and CD38. Materials and Methods: A total of 237 lymphoma cases were selected and examined within six tissue microarray paraffin blocks. Immunohistochemistry was performed using antibodies against CD117, IRF4/MUM1 and CD38. Results: CD117 and IRF4/MUM1 immunoreactivity were seen in a subset of high-grade and low-grade B-cell lymphomas, as well as T-cell lymphomas and classical type Hodgkin´s lymphomas. Aim: This study was performed to determine the effect of caffeic acid phenethyl ester (CAPE) treatment on levels of malondialdehyde (MDA) and nitric oxide (NO) and activities of antioxidant enzymes in retinas of streptozotocin (STZ)-induced diabetic rats. Aims: The aim of this study was to evaluate the oxidative stress measured by serum total antioxidant capacity (TAC) and malondialdehyde (MDA) in patients with community-acquired pneumonia (CAP), and to evaluate their possible correlation with the serum C-reactive protein (CRP) and pneumonia severity index (PSI). Materials and Methods: The PSI, chest X-ray (CXR) scores, and the serum TAC, MDA, and CRP levels were determined in 67 CAP patients on admission and compared to 45 healthy controls. Aim: The aim of this study was to evaluate the surgical approaches in retrosternal goiters. Materials and Methods: We reviewed 23 patients operated with the diagnosis of retrosternal goiter. Twelve (52%) of the cases were female and 11 (48%) were male. Mean age was 53.8±10.9 years. Shortness of breath was seen in 12 (52%) of the patients as the most frequent preoperative symptom. Aims: The incidence and severity of pain on injection of rocuronium and its pretreatment with saline, lidocaine or ketamine were evaluated. Materials and Methods: One hundred and twenty patients were randomized into three groups to receive intravenous (i.v.) lidocaine 30 mg (Group Lidocaine, n=40), ketamine 0.5 mg/kg (Group Ketamine, n=40) or saline 2 ml (Group Saline, n=40). Cystic hygroma (CH) is a neonatal abnormality that may cause respiratory problems especially due to pressure of huge masses located in the neck and thorax on the vital organs. A mother already known to have an intrauterine baby with CH underwent operation under general anesthesia. The newborn, who had cyanotic and respiratory problems, was immediately intubated and operated the day following delivery. This case report is intended to elaborate and discuss anesthesia in a newborn with CH and the mother.

We present the case reports of two children with a parasitic infestation associated with hypereosinophilia. One of those patients, who had been exposed to excessive second-hand tobacco smoke, was diagnosed as having acute eosinophilic pneumonia. The second patient had neither a history of exposure to second-hand smoke nor a prior diagnosis of acute eosinophilic pneumonia. We suggest that passive smoking may trigger acute eosinophilic pneumonia, a condition that develops frequently in children living in countries in which parasitic infestations and tobacco smoking are prevalent.

Aims: Acinetobacter species usually affect hospitalized patients and are involved in many infections associated with a high mortality rate. Management of infections caused by these strains is very difficult, as the strains often display multiple drug resistance, including carbapenem resistance. Tigecycline is the most recently developed semi-synthetic broad spectrum glycylcycline antibiotic. Splenic marginal zone lymphomas (SMZL) are rare tumors arising from B-cells, accounting for approximately 1% of all lymphomas. An accompanying lymphoma is uncommon in patients who have chronic renal failure and undergo hemodialysis. We present herein a 35-year-old male hemodialysis patient admitted with fever, abdominal pain, and splenomegaly, diagnosed with SMZL. In our investigations in the English literature, we did not observe any SMZL case among hemodialysis patients. Fever may emerge due to different causes in chronic hemodialysis patients, primarily infections. In hemodialysis patients presenting with fever, lymphoma and other malignant diseases should be considered.

Aims: To report a rare case of oropharyngeal anthrax. Materials and Methods: A case report of oropharyngeal anthrax is presented together with the related world literature. Results: A 70-year-old female patient with respiratory distress and extensive swelling of neck, soft palate and uvula is presented. There was ecchymosis on the neck circumferentially. Bacillus anthracis grew in the blood culture. The patient died of toxemia and sepsis. Conclusions: Oropharyngeal anthrax is the least common form of anthrax. Inflammatory pseudotumor (IP) is a rare pseudoneoplastic and lymphoproliferative disorder. The etiology of IP is still unclear. A 46-year-old male patient presented with a six-month history of dysphagia and diplophonia. Endoscopic examination of the larynx and hypopharynx revealed a 15 x 20 mm smooth-surfaced mass involving the right side of the hypopharynx. Computerized tomography (CT) scans of the neck showed a mass 3 cm in diameter at the level of the hyoid bone. The mass obliterated the pharyngeal lumen. Materials and Methods: The records of 420 patients with SV who were treated surgically during a 41.5-year period were reviewed retrospectively. The preoperative and operative criteria that were significantly correlated with mortality were determined. Results: Of 420 patients, 68 (16.2%) died. Age over 60 years (P < 0.001), associated disease (P < 0.001), presence of shock (P < 0.001), and presence of sigmoid colon gangrene (P < 0.001) were significantly correlated with mortality. A classification was made using these criteria. Conclusions: Recommended treatment options and potential mortality rates were determined for each class in the new classification based on the evaluation of the present series, our clinical experience, and literature findings. This classification may be helpful in determination of the surgical treatment and prognosis of SV.

Aim: Recurrence after a successful nonoperative detorsion or a nondefinitive operation is an important problem in the treatment of sigmoid volvulus (SV). This study was designed to review the outcomes of elective treatment of successfully detorsioned SV and to discuss the treatment algorithm. Materials and Methods: The records of 873 patients were reviewed retrospectively. Results: Nonoperative treatment was applied in 614 patients. The success, mortality, complication and recurrence rates were 77.5%, 0.8%, 2.8%, and 4%, respectively.
