Early Clinical Predictors Of Intractable Epilepsy In Childhood, Semra Saygi, İlknur Erol, Füsun Alehan
Turkish Journal of Medical Sciences
In this retrospective study, we evaluated the clinical responses to antiepileptic drug (AED) therapy in pediatric epilepsy patients treated at a single center. Materials and methods: We identified 28 children with intractable epilepsy and 213 patients with drug-responsive epilepsy. Results: Univariate analysis showed that age at onset, high (daily) initial seizure frequency, infantile spasm, history of neonatal seizures, abnormal neurodevelopmental status, neurological abnormalities, mental retardation, remote symptomatic etiology, and abnormal brain imaging results were significant risk factors for the development of intractable epilepsy (P < 0.05). Multivariate logistic regression analysis revealed that high (daily) initial seizure frequency and remote symptomatic etiology were significant and independent risk factors for intractable epilepsy (P < 0.05). Conclusion: Our study suggests that the risk of developing intractable epilepsy in childhood may be predicted, to some extent, by the early clinical course. Early identification of patients at high risk of developing intractable epilepsy will guide appropriate therapy and reduce exposure to ineffectual treatments.

To evaluate the necessity of leaving a pelvic drain after open radical cystectomy. Materials and methods: Between January 2007 and January 2010, 58 patients with muscle-invasive bladder carcinoma were included. All patients underwent radical cystectomy, extended lymph node dissection, and orthotopic bladder (Studer pouch) substitution. Patients were randomized to have a pelvic drain catheter (Group 1, n = 22) or not (Group 2, n = 36). Ureteral catheters were removed at postoperative days 7 and 8. The pelvic drain catheters in Group 1 were removed when daily production was

Reopening sternotomy to control bleeding after coronary artery bypass grafting surgery (CABG) has been reported in 2%-7% of cases. Platelet dysfunction and activation of fibrinolytic cascade are the common causes of bleeding after on-pump CABG. Different antifibrinolytic drugs have been used to reduce bleeding. In this study, we aimed to investigate the efficacy of topical tranexamic acid in reducing postoperative mediastinal bleeding after CABG. Materials and methods: This was a double-blind placebo-controlled randomized clinical trial on 40 patients as the control and another 40 patients as the study group. On completion of CABG before sternotomy wound closure, tranexamic acid (2 …

People are more frequently exposed to acute sleep loss nowadays due to extensive working. Epidemiologic studies suggest that sleep deprivation (SD) increases the risk of cardiovascular events. Aortic stiffness is also shown to predict cardiovascular events, stroke, and death even in healthy populations. The association between SD and aortic stiffness remains unknown. We sought to determine the impact of acute SD on aortic elastic properties in healthy workers. Materials and methods: The study population consisted of 34 healthy medical personnel working in shifts (17 men and 17 women). Less than half of the mean daily sleep time, or less than …

To evaluate the relationship between dental erosion (DE) and scintigraphically detected gastroesophageal reflux disease (GERD) in patients with cerebral palsy (CP). Materials and methods: Included in the study were 21 CP patients with dental erosion and a control group consisting of 16 CP patients without dental erosion (total of 37 children; 19 male, 18 female; mean age: 12.1 ± 2.8 years). The severity of DE was scored and all patients underwent gastroesophageal reflux scintigraphy. Results: The prevalence of GERD in patients with DE (78.9%) was found to be significantly higher than that in patients without DE (21.1%). The number of …
