Background: Heart failure (HF) patients who follow the treatment regimen and attend to symptoms before they escalate are assumed to have better health-related quality of life (HRQOL) than those with poor self-care, but there are few data available to support or refute this assumption.
Objective: The objective of the study was to describe the relationship between HF self-care and HRQOL in older (≥65 years old) adults with moderate to advanced HF.
Methods: Self-care was measured using the 3 scales (maintenance, management, and confidence) of the Self-care of Heart Failure Index. Scores range from 0 to 100, with higher numbers indicating …

The patient-healer relationship has an increasing area of interest for complementary and alternative medicine (CAM) researchers. This focus on the interpersonal context of treatment is not surprising as dismantling studies, clinical trials and other linear research designs continually point toward the critical role of context and the broadband biopsychosocial nature of therapeutic responses to CAM. Unfortunately, the same traditional research models and methods that fail to find simple and specific treatment-outcome relations are similarly failing to find simple and specific mechanisms to explain how interpersonal processes influence patient outcomes. A preliminary subnational statistical analysis of violence against Christians in contemporary India, this article suggests that whereas the data provide very little support for simple, demographic explanations of this violence, they do more robustly support theories emphasizing the relative status of ethnic and religious minorities (vis-à-vis majorities) and the perception, among Hindus, that Christians (and other minorities) represent a threat to their numerical, political and economic strength.

The idea of the Guantánamo detainee as a Muselmann, the lowest order of concentration camp inmates, contains within it important implications for the new understanding of sovereignty in the era of Guantánamo, in an age of exception. The purpose of this article is to explain the status of those who are detained at Guantánamo Bay. Stated broadly, in assessing that status, we will emphasize the connection between the altered meaning of sovereignty that has accompanied the placing of prisoners in an American penal colony in Cuba and the biopolitical status of the prisoners who reside there. Matching for several nominal covariates with many levels has usually been thought to be difficult because these covariates combine to form an enormous number of interaction categories with few if any people in most such categories. Moreover, because nominal variables are not ordered, there is often no notion of a "close substitute" when an exact match is unavailable. Background/Aims: Most heart failure (HF) hospital discharges involve people > 65 years, many frail. The purpose of this study was to determine if frailty explains variability in health related quality of life (HRQOL) in older adults with HF over and above known correlates.
Methods: A frailty index score was developed by weighting age, number of comorbid conditions, and symptom severity. A multivariate hierarchical regression analysis of known predictors of HRQOLgender, income, ethnicity, health perception, NYHA class — were entered first and then the frailty index was entered and regressed on HRQOL in 2 unique samples.

Background
Most heart failure (HF) patients have other comorbid conditions. HF self-care requires medication and diet adherence, daily weight monitoring, and a thoughtful response to symptoms when they occur. Self-care is complicated when other chronic conditions have additional self-care requirements. The purpose of this study is to explore how comorbidity influences HF self-care.
Methods
Using qualitative descriptive meta-analysis techniques, transcripts from 3 mixed methods studies (n = 99) were reexamined to yield themes about perceptions about HF and self-care and to explore the influence on HF self-care. The Charlson Comorbidity Index identified comorbid conditions.
Results

In January 1876 William Osler, a young Canadian physician, was recovering from a mild case of smallpox contracted while attending patients at the Montreal General Hospital (Figure 1). In a letter written that same month to an old schoolmate (ArthurJarvis), Osier described his illness and noted in closing, "You need not be afraid of this letter. I will disinfect it before sending." Concern about disseminating smallpox via the letter was well founded. In his medical textbook of 1892, Osler would later write that smallpox can be conveyed by fomites: "the dried scales [of variola scabs] ... as a dust-like powder …

Joseph Hersey Pratt (1872-1956) was a member of the second class of the Johns Hopkins Medical School, where he became a devoted student of William Osler and life-long disciple. Pratt received his medical degree in 1898 and spent his professional career in Boston. He maintained a close association with Osler until the latter's death in 1919, when Osler's deification as a secular medical saint began. When Pratt died in 1956 at age 83, the Boston Globe eulogized him in an editorial which read, "Dr. Pratt earned a place in the group of Boston medical immortals who have done so much …

Intergenerational disparity in income and health violates the norm of equal opportunity and deserves the attention of researchers and policy makers. To understand changes in intergenerational disparity, we created the intergenerational mobility index (IMI), which can simultaneously measure changes in income rankings and in health outcomes across two generations. We selected obesity as one health outcome to illustrate the application of IMI due to its severe health and financial consequences for society and the significant changes in the distribution of obesity across income groups. Heart failure (HF) afflicts nearly 6 million Americans and the prevalence continues to rise as the population ages. Hospital use for HF is high and about half of readmissions are believed to be preventable. Promoting self care through telehomecare is an emerging strategy for managing chronic illness. Hospitalized heart failure patients were randomly assigned at discharge to receive usual home care or telehomecare, nurse visits and daily use of monitoring devices such as blood pressure cuffs, scales, glucometers, and pulse oximetry to monitor their symptoms. Nurses at the home care agency monitored the results and interacted with patients as needed …
