Factors That Lead To Hospital Readmissions And Interventions That Reduce Them: Moving Toward A Faith Community Nursing Intervention, Deborah J. Ziebarth

Abstract

Hospital readmissions affect over 80 percent of all Medicare enrollees. Hospitals have a responsibility to their Medicare patients to keep them safe after discharge in their homes and communities. With changes in the Medicare reimbursement model, hospitals are examining efficient methods of decreasing avoidable re-admissions. A Faith Community Nurse Transitional Care Program may be just the answer to improve patient’s discharge experience, ensure post-discharge support and reduce re-hospitalization.

Methods

This report presents the results of a research process in which 59 young adults who had received residential mental health treatment in the past were sorted into descriptive profiles based on the information they shared about their lives and personal functioning with researchers. Five different groups of young adults emerged from this process and represent the clearest categorizations for understanding this particular sample of young adults from across Southern Ontario who received residential treatment.

It is estimated that over 7 million orphans are institutionalized worldwide (sos-usa.org). Research has shown that while orphanages are often able to meet physical needs better than alternative forms of care, orphanage life can be harmful for the wellbeing of a child. This study evaluated orphan care at one institution in Malawi, Africa, to determine the effectiveness of care and suggest interventions that could position the children for success. Through interviews with caretakers and administrators, it was found that residential care in Malawi could be an effective form of care. The purpose of the study was to conduct a preliminary efficacy evaluation of the Better Futures model, which is focused on improving the postsecondary preparation and participation of youth in foster care with mental health challenges. Sixty-seven youth were randomized to either a control group that received typical services or an intervention group, which involved participation in a Summer Institute, individual peer coaching, and mentoring workshops. Findings indicate significant gains for the intervention group on measures of postsecondary participation, postsecondary and transition preparation, hope, self-determination, and mental health empowerment, as compared to the control group. This article reports findings from three qualitative studies exploring supports for positive transitions of American Indian/Alaska Native (AI/AN) youth to adulthood. Community-based participatory methods were employed through a research partnership involving a culturally based community agency, the Native American Youth and Family Center (NAYA), the National Indian Child Welfare Association, and Portland State University. Studies utilized a Relational Worldview (RWV) framework, where well-being is understood as a balance among the domains of mind, body, spirit, and context. Collectively, findings demonstrate that NAYA employs culturally grounded interventions to overcome the traumatic histories and current oppressive conditions affecting low-income urban AI/AN youth …

Background Human papilloma virus (HPV) is the most common sexually transmitted infection (STI) globally, with a high burden of anogenital warts (AGW) among men who have sex with men (MSM) and transwomen (TW).

Methods Six-hundred HIV negative MSM and TW (300 with AGW, 300 without) were recruited for a prospective cohort study to examine HPV outcomes and HPV vaccine knowledge. Participants completed a self-administered online questionnaire. Logistic regression was used to assess the association between sociodemographic and behavioral characteristics with HPV vaccine knowledge.

Results The average participant age was 25.5 years. Most (67%) were single and 41.2% self-reported STI symptoms. …

Evidence-based practice with pregnant clients who are opioiddependent can be especially challenging because pregnant women are rarely included in clinical trials. The paper synthesizes systematic reviews on the outcomes of medication-assisted treatment for opioid dependent pregnant women and compares the effectiveness of methadone and buprenorphine. We explore evidence on maternal and neonatal outcomes, the safety of breastfeeding, and discuss the implications for social work practice. Searches were conducted in 6 databases. Ten reviews met the inclusion criteria. Results suggest medication-assisted treatment with either methadone or buprenorphine are equally effective in reducing maternal substance use, although methadone may offer slightly higher …

The issue of posttraumatic stress disorder (PTSD) following combat deployment among American military veterans has created significant barriers to successful reintegration. However, positive outcomes also exist in the form of posttraumatic growth. The purpose of this study was to examine Posttraumatic Growth in U.S. military combat veterans by identifying significant relationships among variables leading to successful reintegration of military personnel within the community following combat deployment.
