Introduction: Variations in nasion (N) could influence the measurement of ANB, thus affecting the interpretation of skeletal jaw relationship. Points sella (S) and N represent the anterior cranial base, a crucial role in determining the skeletal jaw patterns. Improper S-N line can affect results of lateral cephalometric analysis, as a highly tilted S-N line relative to the Frankfort Horizontal Plane (FHP) can lead to discrepancies between the analysis results and true morphology. This case report demonstrates an approach to correct an impacted MM2, specifically in a 21-year-old male patient who presented with a Class II division 2 malocclusion, deep bite, and bilateral maxillary second molars with buccal crossbite. The treatment processes included space re-opening for the tooth 21 to facilitate replacement with prosthesis, the application of Class III elastics combined with anterior retraction and active tie-back for space closure.
