Treatment of Class III malocclusion in non growing patients usually involves either orthodontic camouflage or orthognathic surgery (OgS). Aesthetic outcome has gained in importance in the treatment of orthognathic patients . The surgery options of Class III malocclusions have historically been treated by isolated mandibular setback and maxillary advancement, or bimaxillary osteotomy. This is a case of 17-year-old female patient with skeletal Class III with mandibular prognathism, anterior crossbite and open bite, and severe crowding. This 25-year-old male was characterized with mandibular prognathism and spaced dentition. We applied bilateral sagittal split osteotomy and genioplasty to set back the mandible and the prominent chin. The excess spaces of lower dentition were consolidated and then closed surgically by anterior subapical osteotomy. After treatment, his facial esthetics and occlusion were improved significantly. In the spaced dentition, the space closure by anterior subapical osteotomy after space consolidation may provide a more efficient alternative treatment.

Many researches had been proved that the effect of intraoral vertical ramus osteotomy (IVRO) to relieve the symptoms of painful temporomandibular joint dysfunction syndrome was significant by repositioning of the condyle head anteriorly and inferiorly. A case of mandibular prognathism with facial asymmetry was presented. The missing left central incisor was previously restored with a compromising cantilever bridge. Interdisciplinary approach including orthodontics, orthognathic surgery, and prosthetic dentistry was performed and gave patient a better esthetic and functional result. Ways to achieve better esthetics, evaluation and possibility of surgery-first orthognathic approach, and other possible treatment options were discussed.

A case of mandibular prognathism with facial asymmetry was presented. Surgery-first orthognathic approach was performed that no pre-surgical orthodontic treatment was done before surgery. Within this treatment modality, patient had major profile improvement in the beginning of the treatment and total treatment time was shortened. The etiology of mandibular deviation, treatment principles of facial asymmetry, and the advantages of surgery-first orthognathic approach were discussed.

The case report is presented on an adult patient who had surgical-orthodontic treatment to solve her skeletal Class III malocclusion with mandible deviation. Conventional approach of surgical-orthodontic therapy requires a varied period of pre-surgical orthodontic treatment, the most important work to do is dental decompensation. The case was treated by surgical-first approach with minimal pre-surgical orthodontics, i.e. most of the detail orthodontic works (leveling, alignment and interdigitation) were done after orthognathic surgery. The total treatment time was 12 months. The facial symmetry and optimal occlusion were achieved.

An alternative surgical treatment plan was presented to show how to efficiently correct mandibular prognathism without large overjet discrepancy presented. The protrusive chin was eliminated through clockwise rotation of the occlusal plane and maxillo-mandibular complex. Within this method, no complicated orthodontic treatment was needed and the total treatment time was therefore shortened. The smiling arc was also improved. The philosophy, clinical application, long term stability, and side effects of the occlusal plane manipulation in orthognathic surgery were discussed.
