Project SOAR (Supporting Operational AIDS Research) conducts operations research (OR) with the aim of producing a large, multi-faceted body of high-quality evidence that can guide the planning and implementation of programs and policies for HIV prevention, care, and treatment. OR provides the needed evidence to make sound policy and program decisions at national, subnational, and service-delivery levels by identifying practical solutions to challenges delivering program services. Translating OR findings into action at these levels requires two-way information exchange between people who can apply research findings and researchers. The purpose of this study was to examine examples of task-shifting programs in Uganda with the aim of generating data that could inform the development of task-shifting policies, guidelines, and practices. The overall purpose of the study was to contribute to the growing knowledge base about task-shifting by describing the service delivery approaches of three purposively selected major AIDS service organizations (ASOs) in Uganda that have adopted task-shifting. The study showed that in the three ASOs, almost all key tasks in antiretroviral therapy (ART), including determining eligibility, initiating ART, and dispensing antiretrovirals, can be feasibly shifted to lower-level cadres or …

In February 2016, Project SOAR convened a Technical Advisory Network meeting, attended by more than 50 international and domestic thought leaders in adolescent HIV care research and a small group of youth from southern Africa who are living with HIV. To prepare the groundwork for the meeting, SOAR produced a background paper synthesizing the literature on HIV continuum of care outcomes among adolescents living with HIV (ALHIV) in sub-Saharan Africa, where most adolescent cases of HIV are concentrated. Strengthening Community-Based Services For Children And Families Affected By Hiv: An Evaluation Of The Zambia Family Program, Project Soar

As the US President’s Emergency Plan for AIDS Relief (PEPFAR) pivots its focus to care and treatment, there is strong interest in learning to what extent community-based programs contribute to achieving the public health goals of getting people—including children and adolescents—tested, enrolled in care if infected, and adherent to treatment. This brief describes Project SOAR’s evaluation of the Zambia Family (ZAMFAM) program, which will generate critical evidence to help fill knowledge gaps. The evidence will also guide country and global decisionmakers in strengthening community-based services to better meet the care, support, and treatment needs of vulnerable children and their families.

HIV/AIDS has brought the connections between care and relatedness into sharp relief. In the midst of social change driven largely by the AIDS epidemic, the house has emerged as the most stable element connecting kin in Lesotho. Houses provide spaces that frame human actions, transform relationships, and reflect the social order. The house is a key crossroads for human movement. It is also the site where physical connections, emotional bonds, and feelings of love and affection are nurtured. Most significantly, it is the site where physical acts of caring take place. In this article we discuss the role that fathers and paternal families play in acknowledging and caring for children born outside of a recognised union in two southern African communities – Nyanga East, South Africa and Mokhotlong, Lesotho. While these communities are geographically and culturally close, there are important differences in the responses to the care of children born outside of a recognised union. In Nyanga East, despite not paying damages, the genitor and the paternal family are increasingly becoming involved in the care of children, even when they are no longer in a relationship with the mother; whereas in …

The 2.5 generation refers to individuals who have one parent born in the United States and one born in another country. The presence of both native-born and foreign-born parents has the potential to enhance bicultural adaptation. Across two studies with Latino young adults, we examine the extent to which the 2.5 generation is distinct from members of other generations with regard to cultural orientation, acculturative stress, and parent ethnic socialization. Results suggest that the 2.5-generation individuals report greater native cultural orientation, ethnic identity, and parental socialization compared with third-generation individuals, along with greater American orientation than first-generation individuals. Youth's experiences with romantic relationships during adolescence and young adulthood have far reaching implications for future relationships, health, and well-being; yet, although scholars have examined potential peer and parent influences, we know little about the role of siblings in youth's romantic relationships. Accordingly, this study examined the prospective longitudinal links between Mexican-origin older and younger siblings' romantic relationship experiences and variation by sibling structural and relationship characteristics (i.e., sibling age and gender similarity, younger siblings' modeling) and cultural values (i.e., younger siblings' familism values). This study examined prospective associations between the family context and adolescents’ romantic relationships as moderated by adolescents’ gender and culture among Mexican American families (N = 189). Adolescents at Time 1 (early adolescence) were on average 12.29 years of age (SD = .50) and 54% female. Mothers and fathers reported on family structure and dynamics during early adolescence, and youth reported on their romantic relationship involvement and quality during middle and late adolescence. Results from path analyses indicated that family structure and dynamics (supportive parenting, consistent discipline, parent-adolescent, and interparental conflict) were associated with adolescents’ romantic involvement and …

This study assesses the impact of nonstandard employment schedules (shift work) on parenting among U.S. fathers of young children in dual-earner couples. The outcomes examined include total caregiving, caregiving without the mother present, and the elements of father involvement proposed by Pleck: positive engagement, warmth, and control. Models with latent variables and with lagged dependent variables are estimated using three waves of nationally-representative data from the Early Child Longitudinal Study – Birth Cohort (ECLS-B). When incarcerated youth – those with and those without disabilities – face the prospect of reentering the community, they have many obstacles to overcome. Employment requirements are often associated with terms of parole or aftercare. Those who fail to obtain and maintain employment often reenter the juvenile justice system instead of successfully reentering society. Research shows employment is critical for successful transition from incarceration back in to the community. Limited information is available about programs that positively impact post-incarceration employment for juveniles, however. The overall objective of this study is to provide an evidence-based description and interpretation of the HIV-related risks and vulnerabilities faced by different subgroups of youth in Egypt. The report is part of a six-country study that was conducted by the Population Council in Egypt, Kenya, Nigeria, Senegal, South Africa, and Uganda. The study examines social, economic, cultural, and legal factors that may contribute to youth’s increased vulnerability to HIV and documents the situation of the policy and programmatic responses. The overarching goal of this study was to pilot an approach to HIV testing services (HTS) that addresses intimate partner violence (IPV) and sexual relationship power (SRP) inequalities, and explores the intervention’s potential for improving uptake and retention of antiretroviral therapy and prevention of mother-to-child transmission services. The project was implemented in Kenyatta National Hospital’s (KNH) antenatal care (ANC) clinic, and examined intermediate indicators of effect, as well as operational issues around incorporating attention to IPV in hospital procedures, especially in high-volume, high-need settings like KNH’s.
