Distraction osteogenesis (DO) is a surgical procedure that uses a distractor through an osteotomy site, by applying mechanical stretching forces to produce new organization. These organizations contain skeleton, muscles, nerves, blood vessels and so on. This article reviewed the comparisons of DO with conventional guided bone regeneration (GBR) and conventional orthognathic surgery (OGS), as well as the introduction of some of the recent case reports, clinical studies and post-surgery stability in relation to the dentoalveolus, maxilla and mandible regions. Posterior complete buccal crossbite, also called scissors-bite or Brodie Bite, is usually difficult to be corrected. In most cases with the occlusion of complete buccal crossbite, the mandibular teeth are lingually tilted, and the maxillary teeth erupt past their antagonists. When using the conventional orthodontic treatment to correct the scissors-bite, extrusion of the molars and subsequent occlusal plane canting frequently occur. Applying miniscrew as direct skeletal anchorage can facilitate the scissors-bite correction without totally depending on patients’ compliance. The etiologies of skeletal Class III malocclusion include excessive growth of the mandible, insufficient growth of the maxilla, or the combination of both. The ideal treatment for severe mandibular prognathism is a combination of orthodontic treatment with orthognathic surgery to setback the mandible. However, nonsurgical approach using mini-implant anchorage could distalize the mandibular dentition and improve the occlusion if the skeletal discrepancy is mild to moderate. The aim of this study is to evaluate the shear bond strengths of metal brackets using two different fluoridereleasing adhesives (Light bond and Enlight) activated by two photoactivation systems [a conventional halogen light-curing system (HLC), and a plasma arc light curing system (PALC)]. Methods: Forty extracted premolars were divided into four groups of ten. The treatment timing is the most important factor in the correction of skeletal Class II malocclusion. The treatment effect in contribution of mandibular growth in the circumpubertal growth period is better than that in the pre-pubertal growth period in use of no matter what kind of orthodontic appliance. This report is a case of skeletal Class II malocclusion with mandibular deficiency. By using functional appliance (Activator) and fixed orthodontic appliance in the circumpubertal growth period, the malocclusion and retruded mandibular profile had been successfully improved.

Periodontal accelerated osteogenic orthodontics (PAOO) is a clinical procedure that combines selective alveolar corticotomy, particulate bone grafting, and the application of orthodontic forces. This procedure is theoretically based on the bone healing pattern known as the regional acceleratory phenomenon (RAP). Recently, partial evidences supported that PAOO had some advantages: 1. Supraerupted maxillary molars usually result from early loss of the opposing mandible molars. This may induce insufficient intermaxillary space and compromised prosthesis. The best way to deal with supraerupted maxillary molars is no doubt to intrude them. For the minor supraeruption of maxillary molars, simply wearing a posterior bite plate could apply vertical force to intrude them. But the larger supraeruption, combination with temporary anchorage device usage will supply more stable anchorage force to intrude maxillary molars back to the normal position. Overextrusion of maxillary molars usually results from early loss of antagonistic teeth due to caries or periodontitis. Conventional techniques for molar intrusion was used multiple teeth and extraoral devices, that depended highly on patient’s cooperation and usually result in extrusion of other teeth rather than posterior intrusion. This article reviews treatment consideration, force mechanics and side effects about miniscrews for intrusion of upper molars. Adult patients with skeletal Class III malocclusion may have several available treatment options: (1) a combination of orthodontic treatment and orthognathic surgery. (2) camouflaged orthodontic treatment with various extraction selections. In this case report, we use miniscrew anchorage to retract mandibular incisors and control labial tipping of maxillary incisors. Not only improving dental occlusion without extraction of teeth (except for wisdom teeth) but also achieving a balanced profile in only 14 months of treatment time.

The aim of this retrospective study was to examine the periodontal outcomes of impacted maxillary central incisors after surgical exposure and orthodontic extrusion compared to naturally erupted, contralateral incisors (control). Methods: Patients (N=80, 80 central incisors), who had been treated by a surgical-orthodontic approach, underwent a periodontal examination after 6 to 146 months. Purpose: the purpose of this study is to evaluate mandible bone density and gender difference through three-dimensional images. The Images was captured by cone-beam computer tomography (CBCT) and transfer into DICOM format for further examination. Material and method: CBCT images were obtained from 36 males and 34 females aged from 18 to 40 years. Mandible bone density was measured in 4 sites bilaterally. The surgery-first approach in surgical orthodontic treatment is re-emerging as the preferred technique when compared to the traditional orthodontic first-approach. Despite its growing popularity, no planning protocol that offers comprehensive and accurate predictions has been described for the surgery-first approach. In this article, the authors utilize computer aided surgical simulation (CASS) to provide two planning methods for the surgery-first approach. An adult female patient aged 34 years and 8 months with Wilson’s disease, a rare autosomal recessive genetic disorder, demonstrated poor chewing ability and facial asymmetry which were worsened gradually since the disease attacked her 10 years ago. She was treated by surgical orthodontics with minimal presurgical orthodontic decompensation. We designed an atypical maxillary anterior subapical osteotomy to close the maxillary incisor pontic space, retract anterior segment of the maxilla, and maintain the dental midline simultaneously. Class III malocclusion with mandibular protrusion and lateral displacement are commonly observed at orthodontic clinic. If patients don’t accept orthognathic surgery and belong to mild to moderate malocclusion, non-surgery treatment will be available. This was a 25-year-old female; her chief complaint was anterior crossbite with anterior spacing and mandibular protrusion. The diagnosis was dental and skeletal Class III malocclusion with mandibular prognathism. A 26-year-old female presented with Möbius syndrome. She came with chief complaint of excessive exposure her front teeth. She had Class III malocclusion with anterior cross bite. However she had a hyperdivergent face and backward rotation of the mandible without effective chin. After evaluation, she underwent orthognathic surgery and orthodontic treatment. The surgical techniques for correction her skeletal problems included maxillary Le Fort I osteotomy impaction and setback; mandibular bilateral sagittal split osteotomies setback; advancement and reduction genioplasty. The two-year follow up after treatment indicated that the surgical outcome and occlusion were stable.

The double-jaw surgery is frequently desirable to correct skeletal discrepancies and obtain better esthetic results for patients with severe skeletal Class III deformities. Instead of standard surgical principles, an alternative rotation of the maxillo-mandibular complex (MMC) is frequently required for patients with extreme degrees of skeletal discrepancies to achieve optimal facial esthetics.
