Dental And Skeletal Correction Of Möbius Syndrome: A Case Report, Yea-Ling Yang, Ellen Wen-Ching Ko, Chiung-Shing Huang, Yu-Ray Chen

A 26-year-old female presented with Möbius syndrome. She came with chief complaint of excessive exposure her front teeth. She had Class III malocclusion with anterior cross bite. However she had a hyperdivergent face and backward rotation of the mandible without effective chin. After evaluation, she underwent orthognathic surgery and orthodontic treatment. The surgical techniques for correction her skeletal problems included maxillary Le Fort I osteotomy impaction and setback; mandibular bilateral sagittal split osteotomies setback; advancement and reduction genioplasty. The two-year follow up after treatment indicated that the surgical outcome and occlusion were stable.

In clinical, the maxillary transverse development insufficiency of the patients often have the posterior crossbite and arch narrow circumstances, there are some impact in the pronunciation, chewing functions, and even for appearance. In childhood, these problems can be improved by rapid palatal expander correction device to stimulate the maxilla suture zone growth and development. However, the adult’s palate suture zones have been closed; if the maxilla needs to do a significant expansion, it often has to assist by the way of surgery to resolve. The double-jaw surgery is frequently desirable to correct skeletal discrepancies and obtain better esthetic results for patients with severe skeletal Class III deformities. Instead of standard surgical principles, an alternative rotation of the maxillo-mandibular complex (MMC) is frequently required for patients with extreme degrees of skeletal discrepancies to achieve optimal facial esthetics. The purpose of this study was to investigate and analyze the situation of medical disputes or controversies in clinical orthodontic practice in Taiwan. Materials and methods: The investigation was done by the questionnaire survey among the dentists who perform orthodontic treatment as either part or all of his/her clinical practice. This 16-year-old female presents with Class II Division 1 subdivision right malocclusion, complicated by gummy smile, large mandibular plane angle and the mandibular right first molar missing. Treatment consisted of 4 temporary anchorage devices (TADs) application and extraction of bilateral maxillary first premolars and the mandibular left first premolar. Bimaxillary anterior and posterior teeth were intruded by miniscrews for counterclockwise rotation of the mandible. A very favorable result was achieved due to reduction of gingival display and the mandibular plane angle was decreased successfully.

Facial asymmetry is a common feature in orthodontic patients. Mild skeletal facial asymmetry can be treated effectively by camouflaged orthodontic technique. The purpose of this research is to compare the arch changes after non-extraction orthodontic treatment on the mandibular crowding dentition between self-ligating and conventional bracket systems. Seventy-one samples that satisfied the criteria were selected from the patient database, including forty samples in the conventional bracket group and thirty-one in the self-ligating bracket group. Pre-treatment and post-treatment intercanine width, interpremolar width, intermolar width, arch perimeter and arch length were measured from the study cast, and angulation and position of mandibular incisors were measured from the cephalometric radiograph. This is a case report about an adult female patient with bilateral idiopathic condylar resorption. Extraoral check revealed obvious convex profile with retruded chin, lip incompetence; intraoral check revealed class I malocclusion with moderate crowding. Owing to the bilateral condylar resorption, the condyle position may not be easily hold during operation; besides, trying to maintain the integrity of whole mandible to avoid post treatment relapse. In treating patients with facial asymmetry, orthognathic surgery is often needed in combination with orthodontic treatment. Conventional surgical treatment planning uses dental casts and two-dimensional radiography to predict the amount of movement needed. The iatrogenic injuries, which often lead to medical tangles, have been widely discussed in recent years.The areas of intraoral iatrogenic injuries in orthodontics include both hard tissues and soft tissues: tooth, alveolar bone, gingiva, oral mucosa, nerve and so on. Almost every procedure during orthodontic therapy could result in unpredictable injuries. To avoid negative effects, the clinicians should understand all causes of various iatrogenic injuries, and take measures during orthodontic treatment. This article discusses the etiologies, prevention and management of intraoral injuries, and it may help for clinicians.

A case was presented with an adult male patient diagnosed as skeletal class III and dental class I malocclusion, and complaining of his dental crowding. Undergone orthodontic related examinations and periodontal evaluations, the treatment plan was to combine atypical extraction with insertion of mini-implants into mandible, in order to facilitate control of anchorage. After the treatment, lateral profile and occlusion were both apparently improved. Discussions were made about usage of mini-implants in this case and other possible ways to treat the patient.

A case of a 14 y/o male patient with a lingually impacted mandibular right 2nd molar was presented. After the third molar was extracted, a closed coil spring was used to connect the impaction to a miniscrew inserted in the nearby buccal shelf. Throughout the treatment process, all orthodontic appliances were hidden underneath the flap. The advantage of this closing flap technique is that it improves patient comfort while minimizing irritation to the surround muscle and soft tissue. Treatment options for uprighting lingually impacted mandibular 2nd molar, clinical considerations of orthodontic miniscrew placement, and design of biomechanics were also discussed.

Patients with impacted maxillary canine are considered to be more difficult to treat than the average orthodontic patients because successful treatment requires proper surgical technique and orthodontic mechanisms. This case report describes the treatment of a 10-year-old boy with a palatally impacted canine and rotated premolar. Using closed exposure with placement of an attachment, following by distal orthodontic force to bring the canine crown away from the root of lateral incisor, then buccal traction into normal occlusion. Anterior crossbite is a common clinical problem that requires early diagnosis and proper treatment. Here we report on a pair of siblings with anterior crossbite. They have a similar dental and facial profile at the start of treatment, but developed different types of dental facial patterns on follow-up. Summary: An adult male (20 years old) came to our clinic with a chief complaint of lower posterior crowding. Clinical examination found mild spacing over the lower anterior teeth and lingually blocked-in lower second premolars. Because the patient didn’t want to receive extraction treatment, ISW (improved superelastic Ti-Ni alloy wire) leveling was performed to relieve lower arch crowding. Posterior crossbite is a common malocclusion in the deciduous or mixed dentition. Various techniques of posterior crossbite correction have been suggested, including rapid maxillary expansion, slow expansion with a quad-helix appliance or removable expansion plate, grinding, and composite onlay. A young male (14 years old) came to our clinic with a chief complaint of maxillary protrusion and not being able to chew food well with his right posterior teeth. Clinical examination revealed Class II division 1 malocclusion with unilateral scissors bite on the right side. With successful molar upright on the right lower arch by bracket upside-down, adding crown buccal torque on the archwire and ISW(Improved Super-elastic Ti-Ni alloy wire, developed by Tokyo Medical and Dental University) Expansion Arch technique, scissors bite was corrected quickly.
