Bimaxillary protrusion is characterized by protrusive and proclined upper and lower incisors. Since patients with bimaxillary protrusion generally have perfectly good occlusion, orthodontic treatment is often solely sought to correct the protrusive profile and less concern about the airway space change. Typical orthodontic treatment includes extraction of the bimaxillary premolars and anterior tooth retraction, with maximum anchorage. Recently, the relationship between maximum anchorage and tooth displacement is well recognized, retrospective upper airway analysis after retraction of the large incisors remains to be established. Objective: Dentofacial abnormalities, such as bimaxillary protrusion, can be distressing both socially and psychologically. The aim of this study is to identify the cephalometric features of bimaxillary protrusion and highlight the measurements that characterize bimaxillary protrusion when compared to a normal occlusion group. Materials and methods: Lateral cephalograms of 60 pretreated bimaxillary protrusion young adults were compared with 60 untreated young adults with normal occlusion. Male and female were equal in each group. Thirty-two landmarks were identified for 61 skeletal, dental and soft tissue parameters. Bimaxillary protrusion is defined as convex profile with protrusive upper and lower lips. Therefore the orthodontic treatment goal is to correct the proclination of upper and lower incisors by retracting the incisors into the space usually resulted from four bicuspid extractions. Combined Anterior And Posterior Subapical Segmental Osteotomy Treatment Of Bimaxillary Protrusion With Chin Deficiency — A Case Report, Hsin-Lan Shen, Cheng-Ting Ho, Chen-Tah Huang

This case report presents a 39 years old female adult patient which diagnosis was skeletal Class II and dental Class III malocclusion with severe bimaxillary protrusion, chin deficiency and two maxillary supernumerary teeth over premolars area. The treatment plan were combined with orthodontic treatment and orthognathic surgery including bimaxillary anterior subapical segmental osteotomy, posterior subapical segmental osteotomy over right mandibular area and genioplasty. The treatment was finished in Angle’s Class I occlusion of both canine and molar relationships. Patient had normal overjet and overbite, lips competency and her facial esthetic have improved a lot.
