Anterior open bite is a common and challengeable malocclusion. To manage Angle Class I molar relationship in adult cases with several problems, such as high mandibular plane angle, maxillary canting, anterior open bite, discrepancy of anterior Bolton ratio and bilateral posterior crossbite, we should choose therapeutic method carefully. It is not necessary to improve occlusion and profile with orthognathic surgery. A case of an adult woman with tongue thrust had the chief complaint of anterior open bite. Anterior open bite often combines with longer anterior facial height, higher mandibular plane angle, mandible clockwise rotation and mandible retrognathism. The facial profiles of patients with OSA ( obstructive sleep apnea syndrome) also often reveal mandible retrognathism. Therefore, it is an interesting subject that whether treating anterior open bite has the advantage of improving symptom of sleeping problem. This review article is to investigate the correlation of tooth movement in vertical dimension with airway, also investigate the influence of oral appliance height and surgery-treated open bite on airway.

To manage moderate mandibular prognathism combined with anterior open bite in adult cases,the dento-alveolar growth has to be guided and carefully utilized during orthodontic treatment, especially when the patients have acceptable profile before treatment and tend to deny the needs of orthognathic surgery. Two cases with similar diagnosis and treatment principle were reported. Extraction of mandibular molars provided spaces for lingual tipping movement of lower incisors, the lower occlusal plane showed a counter-clockwise rotation, the open-bite was accordingly corrected and the molar relationship was also normalized. A case of anterior open bite with facial asymmetry is presented. The un-expected anterior open bite and retrusive mandible occurred shortly following orthognathic surgery (LeFort I - 3 pieces + Bilateral sagittal split osteotomy + genioplasty). The anterior open bite was then successfully corrected with miniscrew orthodontics.Possible causes of post-surgical anterior open bite and the treatment options were discussed comprehensively.
