Taiwanese Journal of Orthodontics

This 25-year-old male was characterized with mandibular prognathism and spaced dentition. We applied bilateral sagittal split osteotomy and genioplasty to set back the mandible and the prominent chin. The excess spaces of lower dentition were consolidated and then closed surgically by anterior subapical osteotomy. After treatment, his facial esthetics and occlusion were improved significantly. In the spaced dentition, the space closure by anterior subapical osteotomy after space consolidation may provide a more efficient alternative treatment.

Mandibular symphysis is the critical reference for facial profile and determines the lower incisor position indirectly. It also serves as the primary consideration for diagnosis and treatment plans. This essay reviews articles relevant to the embryology and growth, morphology of mandibular symphysis, and its restrain on orthodontic tooth movement of lower incisors. Discussion mainly focused on the effectiveness of genioplasty, mandibular symphysis distraction osteogenesis, periodontally accelerated osteogenic orthodontics on mandibular symphysis, which may breach its limitation on range of orthodontic tooth movement.

Patient with class III malocclusion and anterior crossbite combined with mandibular prognathism with deviation can be corrected through orthognathic surgery combined with conventional orthodontics. A proper diagnosis and careful treatment planning are the keys to success. This case report described a 30-yearold female, who complained of poor esthetics because of the square face and dark teeth. After a routine orthodontic examination, the diagnosis was skeletal class III malocclusion with mandibular prognathism and a deviated chin shifted to the right.
